CONCLUSION: Significant differences in in vitro parameters were observed between RBC prepared within 8 hours and 18-24 hours after blood collection. The results were consistent irrespective of the blood container used. New alkaline solutions may decrease the differences.

Comparison of JAK2V617F mutation assessment employing different molecular diagnostic techniques.

CONCLUSION: Detection of the JAK2(V617F) mutation may depend on the molecular technique used. Considering that detection of this mutation will not only have a diagnostic value, but also a role in treatment given the development of JAK2(V617F) pathway inhibiting drugs, indications on a reference molecular diagnostic technique for JAK2(V617F) assessment and quantification of its allele burden from a panel of experts are warranted.

CONCLUSIONS: The Securblood system guarantees complete traceability of the transfusion process outside the SIMT and eliminates the possibility of mistaken identification of patients or blood components. The use of fingerprinting to identify health care staff (nurses and doctors) and patients obliges the staff to carry out the identification procedures directly in the presence of the patient and guarantees the presence of the doctor at the start of the transfusion.

CONCLUSION: Although a significant drop in complete blood count was observed in all donors, none manifested features of thrombocytopenia or anaemia. Nevertheless, more prospective studies on this aspect are required in order to establish guidelines for donor safety in apheresis and also to help in assessing donor suitability, especially given the present trend of double product apheresis collections.

CONCLUSIONS: Since the median cost of each apheresis procedure is 900 USD, the fact that two apheresis procedures was spared in 72 cases and one apheresis was spared in another 65 cases, led to a total saving of approximately 188,100 USD. It can be concluded that, in many cases, allogeneic transplants can be completed with a single apheresis session and that there are considerable financial benefits from this practice.

CONCLUSIONS: Desmopressin (DDAVP) is the treatment of choice for patients with type 1 VWD with FVIII and VWF levels of 10 U/dL or more, while VWF/FVIII concentrates are indicated for those who are unresponsive or insufficiently responsive to DDAVP (severe type 1, type 2 and 3 VWD). VWF concentrates devoid of FVIII, not yet licensed in Italy, may be considered for short-term prophylaxis in elective surgery or for long-term secondary prophylaxis.

CONCLUSIONS: The present study suggests that apparently healthy subjects with repeatedly high levels of Hct may have the acquired mutation in JAK2. Laboratory screening tests for JAK2 may be offered to blood donors at transfusion services with expertise in molecular genetics.

Determinants of transfusion decisions in a mixed medical-surgical intensive care unit: a prospective cohort study.

CONCLUSION: RBC transfusion decisions are predominantly based on Hb levels rather than on patients' characteristics. Residents transfuse at a higher Hb level compared to more experienced physicians. The major determinant for physicians to transfuse RBC is bleeding. However, the majority of patients were transfused in the absence of bleeding, and many of these patients received multiple units. The need for RBC may be overestimated, resulting in wasted orders.
