Calcium and phosphate usually form part of a 'bone' profile of tests that also includes measurement of serum albumin and alkaline phosphatase levels. Most laboratories report total calcium concentration in serum. This is influenced in particular by serum albumin concentration, and the total calcium is therefore also adjusted for the serum albumin concentration (ACa = serum Ca + (0.02(40-alb) (g/litre))). However, the adjustment is an approximation which may be inaccurate, particularly at extreme albumin concentrations. Prolonged exposure to raised parathyroid hormone and serum calcium can lead to a multitude of systemic symptoms (Table 1). It is important to consider hyperparathyroidism in the differential diagnosis of patents with hypercalcaemia. Parathyroid surgery can cure patients with primary hyperparathyroidism and lead to prolonged life expectancy. Surgery can be challenging but safe, with low morbidity and mortality when undertaken by surgeons who specialize in parathyroid surgery. This article reports the case of a 37-week gestation neonate delivered by emergency caesarean section for fetal bradycardia. At birth the baby was pale and floppy and needed resuscitation. On further examination, she was anaemic with an abdominal mass. On further investigations and laparotomy, this turned out to be a haemorrhagic luteinized follicular cyst. This case highlights that early recognition and appropriate management of neonatal haemorrhagic ovarian cysts is crucial for successful outcome. Intensive care physicians are increasingly aware of the need to appropriately manage limited resources, so the admission of patients with haematological malignancy can be controversial because there is a perception of poor prognosis regardless of intervention. While this was true in the 1980s - one study showed an in-hospital mortality of 78% (Lloyd-Thomas et al, 1988), another an intensive care mortality of 90% for those requiring ventilation and renal replacement (Brunet et al, 1990) - survival rates are improving. Traditionally, doctors have been encouraged to prescribe drugs using their generic names. Increasing cost constraints in both primary and secondary care have resulted in the Department of Health considering the introduction of 'automatic generic substitution' from 2010. Thus pharmacists receiving a prescription for a brand-named drug would automatically substitute it for a generic preparation where that was available. It is unlikely that the prescribing clinician would be notified of the substitution, and the patient would not be able to choose to receive his/her original medication. Look at the old faded group photographs of the student classes and 'firms'; the numbers are much smaller, the sex entirely male in most schools, the vast majority are white and the dress uniformly a suit and tie. Pick up any bound volume of a general medical journal of 1934 (75 years ago - within the memory of old members of the profession), and there are likely to be papers concerning the dreaded effects of the Gram positive cocci. The pneumococcus, causing often fatal lobar pneumonia in previously fit young people, and the beta-haemolytic streptococcus, producing erysipelas, septicaemia and puerperal sepsis, were especially dangerous. Volume one of the Lancet for that year, for example, contains no less than four papers on different trials of treatment for streptococcal infection as well as a long and learned leading article....

Sepsis and septic shock are common syndromes occurring in the intensive care unit and are associated with a high mortality. Levels of endogenous activated protein C are reduced in sepsis and linked with poor outcome.
