From November 2004 to July 2005, we observed 29 meetings at the Tours Hospital and 324 case presentations, 80 in orthopaedics, 151 in gastroenterology and 93 in chest medicine. Social deprivation did not demonstrate an association with levels of pelvic recurrence, metastasis or death. We reported in this retrospective study the clinical outcome of 56 chronic lymphoïd leukemia of black African in Ivory Coast and the predicting prognosis factors. The mean age was 62 years old with average of 38 to 84 years. According to Binet staging, 29 patients with stage A, and respectively 11 and 16 patient for stage B and C. The present op-ed article reports on a thesis study which examines the emergence of ethics committees in health care institutions. These committees have developed rapidly over the past few years in order to address ethical issues related to the management and care of patients. However, the question is raised of whether the ethics committees established in hospitals are an appropriate and effective response to the demands of the law (bill L. 6111-1), which states that institutions should implement ethical policies but does not provide any guidance on what should be done in practice.

With the introduction of the new prospective payment system considerable concern is being expressed that with the dominant emphasis on coasts, the principals of access and quality might be compromised. A new concept now emerged in care: productivity. How can we conjugate with the preset price for services to its beneficiaries based on average coasts of hospital care for patients' quality of care, patients and nursing team expectations and ethic? Which room will have ethic in that new way of redefining care? Will it be a risk that some patients may become "outliers"? Will the service take over the care?

In medical practice, habits and ethics, on one hand, means, on the other hand, have evolved and induce the addition of autonomy to benevolence principle. In usual cases, traditional paternalism should be abandoned for allowing the patient to express his/her good and his/her choice for medical decision. This changes deeply doctor-patient relationship. Therefore, the physicians have to improve their practice in order to decrease inconveniences and increase advantages of this new relation, to finally better respect patients and their will.
