Nontuberculous mycobacterial pulmonary infection in renal transplant recipients

We report on a Mycobacterium avium complex pulmonary infection in a KTR with underlying chronic obstructive pulmonary disease. Chest computed tomography showed an excavated lesion of the right upper lobe, similar to a typical lesion of pulmonary tuberculosis. Evolution was favorable with multiple-drug therapy including rifampicin, ethambutol, and clarithromycin, along with a slight reduction in immunosuppression. We review the literature and discuss the epidemiology, diagnosis, management, and follow-up of this uncommon post-transplant complication. (Source: Transplant Infectious Disease)

We describe a case of borderline CD30+ CLPD with cytotoxic phenotype, presenting in a 22-year-old male patient as an ulcer on the forearm. He reported having had similar ulcers on the buttock and thigh that spontaneously regressed over the course of 1 year. The lesion resolved with a single course of clarithromycin; a subsequent lesion, too, responded to clarithromycin, and no recurrences or systemic involvement have been documented in the 9-month follow-up. The omission of the name 'Mycobacterium paraffinicum' from the 1980 Approved List of Bacterial Names was due to phenotypic confusion surrounding a close relationship with Mycobacterium scrofulaceum. Correspondingly 'M. paraffinicum' strains grew slowly in >7 days, stained acid-alcohol fast, produced yellow-pigmented smooth waxy colonies in the dark at an optimal temperature of 35 degrees C. Antimicrobial susceptibility patterns and beta-lactam resistance mechanisms of 544 H. influenzae isolates through the nationwide Acute Respiratory Infections Surveillance (ARIS) network in Korea during 2005 and 2006 were determined. Resistance to ampicillin was 58.5%, followed by cefuroxime (23.3%), clarithromycin (18.7%), cefaclor (17.0%), amoxicillin/clavulanate (10.4%), and chloramphenicol (8.1%). We present a case of acute lower limb ischemia with characteristic angiography findings of diffuse arterial spasm resulting from use of ergotamine and an erythromycin derivative, clarithromycin. The history of the patient and classic features seen on angiography helped us establish the diagnosis. Conclusion: Bronchogenic cyst should be included in the differential diagnosis of a child with cough, dyspnea, and fever. Although rare, we stress the importance of keeping mycobacterial infection in mind in cases of an infected cyst. Acid-fast culture should be done on sputum and cyst contents. Due to the frequency of negative cultures, stains should also be performed on resected cyst specimens. Antibiotic therapy should be considered and administered based on the extent of infection. Annual Change of Primary Resistance to Clarithromycin among Helicobacter pylori Isolates from 1996 through 2008 in Japan

Conclusions: The resistance rate of H. pylori infection to clarithromycin in Japan has increased gradually to approximately 30% from 1996 through 2004, and remained unchanged since 2004. Elderly and females were at high risk of having resistance to clarithromycin. Our results suggested that the level of clarithromycin resistance in Japan has now risen to the point where it should no longer be used as empiric therapy. (Source: Helicobacter)

Conclusions: The eradication rate with a PPI, metronidazole and amoxicillin triple therapy was unacceptably low in South China. These results suggest that studies of the eradication of H. pylori infection in China must take into account antibiotic resistance as well as patient occupation, gender, and protocol compliance. Gastric cancer and peptic ulcer disease are both important in China making methods to effectively and efficiently eradicate H. pylori a priority. (Source: Helicobacter)

Only limited information regarding the antimicrobial susceptibilities of Mycoplasma genitalium is available because of difficulties in isolating M. genitalium strains from clinical specimens. Antimicrobial susceptibilities of 15 clinical isolates, 7 ATCC strains and an early-passage of the M30 strain were examined by the broth-dilution method. Azithromycin, clarithromycin, sitafloxacin and moxifloxacin were the most active drugs to M. genitalium and their MIC90 values were 0.002, 0,008, 0.125 and 0.125 mg/liter, respectively.

These data indicate that cholic acid-based antimicrobial agents such as CSA-13 resist proteolytic degradation and inhibition by mucin and have potential for treatment of H. pylori infections, including those caused by the clarithromycin and/or metronidazole-resistant strains. A 28-year-old woman with community-acquired pneumonia was treated with sulbactam/ampicillin and clarithromycin, but failed to show any improvement after 4 days. The antibiotic regimen was changed to pazufloxacin and rapid clinical improvement was seen. Mycoplasma pneumoniae was identified as the causative agent, and adenine (A) to guanine (G) mutation at position 2063 in domain V of the 23S rRNA was noted in the isolate. The minimum inhibitory concentration of macrolide antibiotics, including clarithromycin, of this isolate was greatly elevated. (Source: Respirology)

We present a case of a patient with advanced AIDS, not on antiretroviral therapy, who presented with massive pleural empyema and splenic and liver abscess secondary to Mycobacterium avium-intracellulare complex. She was a 42-year-old female who was admitted with fever, chills, weight loss, and productive cough of 3 weeks duration. Her pleural fluid cultures were positive for Mycobacterium avium-intracellulare complex, and no other organism was identified. Our patient had good outcome after treatment with antitubercular agents (isoniazid, rifabutin, and clarithromycin).

AUC(F) is completely ineffective for evaluating variations in CYP3A activity.
