Some epidemiologic studies have indicated that uterine cancer risk may be increased after use of fertility drugs. To further assess this association, the authors used data from a large cohort of 54,362 women diagnosed with infertility who were referred to Danish fertility clinics between 1965 and 1998. Some women fail to become pregnant after tubal reversal surgery because they do not ovulate regularly. The cause may be that their pituitary glands don't produce enough follicle stimulating hormone (FSH) and luteinizing hormone (LH) at the right time during the cycle. For these women, the medication most doctors prescribe is clomiphene citrate (Clomid). We have always started medroxyprogesterone acetate (MPA) on day 16 of the cycle—that is, after menstruation, whether this menstruation occurred naturally or was triggered by medication. We measure the serum human chorionic gonadotropin (hCG) level on the last day of MPA administration to rule out pregnancy before starting the clomiphene citrate (CC) protocol. Our study corroborates the impact of age on donor insemination outcome. Nevertheless, even in some older age subgroups, acceptable expected cumulative delivery rates were observed. CONCLUSIONS: Further trials comparing aromatase inhibitors with clomiphene citrate are necessary before aromatase inhibitors can be recommended routinely for ovulation induction in women with PCOS and infertility. However, aromatase inhibitors may be considered in a subset of this population, specifically women who are clomiphene citrate resistant or those who, after discussion of risks and benefits, are not candidates for clomiphene citrate, gonadotropins, or GnRH analogs.

in reducing the time to ovulation and often improves the ovulation rate in women with polycystic ovarian syndrome

Polycystic ovary syndrome is a condition present in approximately 5 to 10 percent of women of childbearing age. Diagnosis can be difficult because the signs and symptoms can be subtle and varied. These may include hirsutism, infertility, menstrual irregularities, and biochemical abnormalities, most notably insulin resistance. Treatment should target specific manifestations and individualized patient goals. When choosing a treatment regimen, physicians must take into account comorbidities and the patient's desire for pregnancy. Lifestyle modifications should be used in addition to medical treatments for optimal results....

CONCLUSIONS
IUI treatment requires ovarian stimulation to achieve modest results, but the high multiple pregnancy rates mean that it is no more than a poor substitute for IVF treatment.
