Treatment-related malignancies are widely reported in the medical literature. They are usually chemotherapy-related leukemias or radiation-induced solid tumors. Here we present a case of a 47-year-old woman treated with mantle radiation for Hodgkin's lymphoma who later developed a thymoma in the setting of a venous thrombosis. A review of the literature including surveillance recommendations for secondary malignancies will be discussed. PMID: 19860271 [PubMed - in process] (Source: Connecticut Medicine)

In this report we describe the clinical presentation, diagnosis, treatment and outcome of a 45-year-old woman with thoracic endometriosis. Four clinical presentations have been described. The majority have presented with catamenial pneumothorax, followed by hemothorax, hemoptysis and lung nodules. Our patient presented with right-sided hemothorax and lung nodules. Video-assisted thoracoscopic aurgery confirmed the presence of endometrial tissue embedded in the diaphragmatic pleura. Talc pleurodesis alongwith atotal abdominal hysterectomy and bilateral salpingo-oophorectomy led to a clinical and radiological resolution. ...

When the diagnosis is a zebra: pulmonary artery aneurysm.email this articleEmail this article to a colleague. save this article to My ClippingsSave this article to My Clippings. discuss this articleDiscuss or comment on this article.
We describe a case of a patient who presented to our institution with recurrent hemoptysis. The clinical features were suggestive of an underlying infection or malignancy. When these were ruled out, the search for a zebra had begun. The patient underwent sixty four-slice computerizedtomographic angiography (CTA) which revealed a right pulmonary artery aneurysm (PAA). Timely involvement of interventional radiologists and thoracic surgeons prevented a potentiallylife-threatenninghemorrhage. This case underlines the importance of awareness of this condition in the formulation of a differential diagnosis forhemoptysis. PAAis a...

We reviewed trauma registry data (2004-06) from a Level 1 trauma center to describe the epidemiology of injury. There were 5,900 trauma patients. Most admissions were among white men, age 20-29 years (mean age = 48 years). Most admissions occurred in the summer, on Saturday and Sunday evenings. Blunt injuries (89%) and fractures (49%) predominated. Mean ISS = 9.7. ISS = 9-15 was highest among those 70+ years and lowest among patients 30-39 years (P < 0.01). Motor vehicle crashes and falls were the most common mechanism of injury. Violent assaults ranked highest in the injury prevention priority score. This research ...

Becker's muscular dystrophy (BMD) is one of the most common muscular dystrophy syndromes. The heart is always affected by myocardial fibrosis with early involvement of the right side. Traditionally, ECG, echocardiography and stress testing have been used to evaluate these patients. Cardiac MRI, offering superior assessment of the right side and visualization of the myocardial fibrosis could become the preferred imaging modality for evaluating patients with BMD. Small bowel metastatic disease from primary breast cancer is rare. With this case report we illustrate a distinctly rare presentation of small bowel metastatic disease from breast cancer masquerading as an obstructed incisional hernia. This case is also an example of small bowel metastasis from an invasive ductal primary breast carcinoma as opposed to the more common pattern of small bowel metastasis from lobular carcinoma. We describe the experience of Yale's Center for Interdisciplinary Research on AIDS with AHI diagnosis in Connecticut, as a participating center in the National Institute of Mental Health Multisite AHI Study. We sought to identify AHI cases by clinical referrals and by screening for AHI at two substance abuse care facilities and an STD clinic. We identified one case by referral and one through screening of 590 persons. Screening for AHI is feasible and probably cost effective. Primary care providers should include AHI in the differential diagnosis when patients present with a nonspecific viral syndrome. PMID: 19637661 [...
