CONCLUSION: From the available data hypernatremic alkalosis could not be defined as an independent metabolic disorder. In would seem more appropriate to use the term "strong ion alkalosis" in this context.

In February 2009 a major case of scientific misconduct was discovered. The American pain researcher Dr. S. Reuben had published 21 papers over a period of 15 years that were found to be fraudulent. Suddenly many advances in postoperative pain therapy which had been assumed to be correct seemed questionable. In this review article the lessons which can be learnt from this case are described. CONCLUSIONS: Infectious complications of catheter-based regional anaesthesia are common. Strict hygienic standards must therefore be complied with. More data are necessary to calculate risk factors. The registry provided can also be used as a benchmark to reduce these rates further.

CONCLUSIONS: This study demonstrated that 25 mg ketamine racemate given just before surgery significantly prolongs recovery times without reducing post-operative analgesic needs when applied to patients undergoing vaginal hysterectomy and receiving propofol-remifentanil anaesthesia. A case of extreme metabolic acidosis (pH 6.6) and severe hypothermia (27.3 degrees C) is reported. The patient's anamnesis and medical examination led to the assumption of ketoacidosis. Pronounced ketoacidosis is not unusual in juvenile diabetics type 1 and the incidence in Europe is reported to be between 15 and 70% for all type 1 diabetics. The case describes the targeted therapy of controlled re-warming to avoid neurological damage.

An 11-year-old boy with spasticity and apallic syndrome needed general anesthesia for exchange of a baclofen pump but 2 h later he became increasingly hypothermic, hypotonic with bradycardy and dyspnea. In the present study the efficacy of subcutaneous tunneling and a 10 min disinfection time with a 70% alcoholic solution to reduce the infection rate in continuous interscalene plexus anesthesia were examined. CONCLUSIONS: The SenseWear(R) armband is non-invasive, convenient and easy to handle, but has a significant measurement bias in the hypercaloric range. Although IC is still best suited to determining metabolic need in intubated patients, measurements with the SenseWear(R) armband provide significant advantages, e.g. in non-intubated patients, and give a fair estimation of daily energy expenditure when used alone.

CONCLUSION: The tested SGADs were comparable with regard to ease of insertion, insertion times and airway morbidity. Considering leak tightness and patient comfort the PLMA and the AuraOnce fared better with regard to tightness of seal and patient comfort.

The objective of this article is to review the monitoring devices that are currently used to assess haemodynamics and filling status in the perioperative setting. Methods and principles for measuring haemodynamic variables, the measured and calculated parameters as well as clinical benefits and shortcomings of each device are described. We report the case of a 25-year-old primigravida with a history of FA and dorsal stabilisation of the vertebral column, who was admitted to our hospital for elective caesarean section. Due to increased sensitivity to muscle relaxants, peridural anaesthesia with 8 ml 0.75% ropivacaine and 10 microg sufentanil was used in this case. The perioperative neurological consultation revealed no undue exacerbation of symptoms.

CONCLUSIONS: Ventilation windows of 0.25, 0.3, and 0.5 s were too short to provide adequate tidal volumes in a simulated non-intubated cardiac arrest patient. In a simulated intubated cardiac arrest patient, ventilation windows of at least 0.5 s were necessary to provide adequate tidal volumes.
