Abstract  Primary rhabdomyosarcoma of the alimentary tract is extremely rare, only 11 cases have been reported in the literature. Long-term survival in adults may be possible if the tumor is detected at an early stage even though it is asymptomatic. This highly malignant tumor can be treated by surgical excision, chemotherapy, and irradiation. A 12th case—of stage 1, pararectal, rhabdomyosarcoma—is reported. Abstract  This paper analyzes 320 cases of colorectal adenocarcinoma at the University College Hospital, Ibadan, Nigeria over a period of 18 years. The sex ratio was 1.5∶1 in favor of males; the average age when first seen was 44 years. The symptom complex was weight loss, bloody mucoid stool, altered bowel habit, and abdominal mass. Eighty per cent of rectal cases were in the lower third of the rectum. Most cases were very advanced at the time they were first seen. This study was designed to evaluate any further risk factors to urinary stone formation in patients with Kock ileostomies. Nine Kock ileostomy patients were matched for age, sex, and body weight with nine conventional ileostomy patients and nine controls. Two 24-hour urine samples from each patient were analyzed for volume, pH, Na+/K+ ratio, oxalate, and uric acid concentration. Abstract  Of 182 polyps retrieved from 100 patients who underwent colonoscopic polypectomies, 160 were adenomas; of these, 16 contained epithelial inclusions within the stalk. The importance of recognizing this lesion lies in the fact that it mimics invasive cancer. Abstract  Records of 188 patients with documented portal hypertension were reviewed to determine the incidence of hemorrhoids as well as bleeding complications associated with this condition. The incidence of hemorrhoids among these patients was not increased compared to the normal population. Six of the patients with portal hypertension did, however, bleed massively from hemorrhoids. Elevated portal venous pressure is an important factor in those patients having severe hemorrhoidal bleeding. The presence of coagulation defects may also be of considerable importance. Abstract  A retrospective analysis of 153 patients having intraperitoneal closure of transverse loop colostomies was performed. The mortality was 1.4 per cent. The morbidity rate was 15 per cent, including 7 per cent wound infections and 5.2 per cent fecal fistulas. The incidence of wound infection was not significantly improved by the use of systemic or nonabsorbable intestinal antibiotics. Abstract  A computer analysis has been made of clinical experience of Crohn’s disease in Australian patients. Between 1950 and 1978, 50 patients were managed for Crohn’s disease by one of the authors (E.S.R.H.). This group represented 3.1. per cent of 1608 patients treated during the same period for primary inflammatory bowel disease. The mean follow-up period was 8.4 years. Thirty-nine of the 50 patients were female and 11 male. The small intestine was involved in seven patients, large intestine in 17 and both small and large intestine in 26. Abstract  Crohn’s disease of the colon and small bowel is associated with a greater-than-chance increased incidence of adenocarcinoma, which also differs from the norm in age and anatomic distribution. The cancer risk appears to rise with earlier age of onset and long duration of the granulomatous disease, and the prognosis is relatively poor, probably due to difficulty in diagnosis because of the overlapping Crohn’s disease. Abstract  Colitis cystica profunda is a benign disease of the colon. Its importance lies in differentiating it from mucus-producing adenocarcinoma. It has rarely been described in the surgical literature. A review of records of patients seen at the Mayo Clinic produced 66 clinical cases of localized colitis cystica profunda, and in 21 patients the diagnosis was confirmed histologically. Abstract  A retrospective review of 1353 cases of acute perforated sigmoid diverticulitis treated surgically demonstrates that those operations that resect or exteriorize the perforated segment at the first operation are associated with a lower operative mortality rate than procedures that fail to remove the perforated segment at the initial operation. Recently our attention has been focused on the colonic complications: fibrosis and stricture resulting in obstruction, severe obstipation and recurrent fecal impactions secondary to pseudo-obstruction, progressive colonic dilatation resulting in gangrene of the colon, and sigmoid volvulus and diverticulitis in the presence of both wide- and narrow-mouth pseudodiverticula. Abstract  Records of 266 patients who had undergone rubber ring ligation for hemorrhoidal complaints from 1969 through 1976 were reviewed to evaluate the long-term results. The minimum follow-up period was 36 months, with a mean of 60 months. Of the patients, 80 per cent were improved, with 69 per cent totally free of all symptoms. A subsequent hemorrhoidectomy was required in 7.5 per cent. Results were similar when either bleeding or prolapse was the primary indication for treatment. Abstract  A technical variant of the end-to-end colorectal anastomosis performed with the EEA stapler is described. It realizes a side-to-end anastomosis, performed via the oral-aboral route. In this manner, indications for stapled anastomoses can be extended to patients with discrepancy in bowel size, and where operative findings involve low anterior resection, with the patient not in the lithotomy—Trendelenburg position.
