Abstract Primary rhabdomyosarcoma of the alimentary tract is extremely rare, only 11 cases have been reported in the literature. Long-term
survival in adults may be possible if the tumor is detected at an early stage even though it is asymptomatic. This highly
malignant tumor can be treated by surgical excision, chemotherapy, and irradiation. A 12th case—of stage 1, pararectal, rhabdomyosarcoma—is
reported.

Abstract This paper analyzes 320 cases of colorectal adenocarcinoma at the University College Hospital, Ibadan, Nigeria over a period
of 18 years. The sex ratio was 1.5∶1 in favor of males; the average age when first seen was 44 years. The symptom complex
was weight loss, bloody mucoid stool, altered bowel habit, and abdominal mass. Eighty per cent of rectal cases were in the
lower third of the rectum. Most cases were very advanced at the time they were first seen. The association of infective granuloma,
notably schistosomiasis and amebiasis, occurred in 11 cases. This probably had diagnostic and prognosti...

This study
was designed to evaluate any further risk factors to urinary stone formation in patients with Kock ileostomies. Nine Kock
ileostomy patients were matched for age, sex, and body weight with nine conventional ileostomy patients and nine controls.
Two 24-hour urine samples from each patient were analyzed for volume, pH, Na+/K+ ratio, oxalate, and uric acid concentration. Both ileostomy groups demonstrated reduced urinary volume and Na+/K+ ratio as compared to the control groups (P<0.05). The Kock ileostomy group had the lowest urinary volume. There was no significant
reduction in urinary pH or elevation in u...

Abstract Of 182 polyps retrieved from 100 patients who underwent colonoscopic polypectomies, 160 were adenomas; of these, 16 contained
epithelial inclusions within the stalk. The importance of recognizing this lesion lies in the fact that it mimics invasive
cancer. From histologic study, criteria are proposed for differentiating the pseudocarcinomatous lesion from invasive cancer.
The differentiation from colitis cystica profunda is also discussed.

Summary Cancers of the rectum can be divided into A B and C cases according to the extent of spread. A cases are those in which the
growth is limited to the wall of the rectum: B cases those in which there is extrarectal spread but no lymphatic metastases;
C cases those in which metastases are present in the regional lymph nodes. A striking difference is found in the operative
mortality rate and in the survival period after operation in these three groups. There is reason to believe that in A cases
the disease is completely eradicated by rectal excision and the excellent results of operative treatment confir...

Abstract Records of 188 patients with documented portal hypertension were reviewed to determine the incidence of hemorrhoids as well
as bleeding complications associated with this condition. The incidence of hemorrhoids among these patients was not increased
compared to the normal population. Six of the patients with portal hypertension did, however, bleed massively from hemorrhoids.
Elevated portal venous pressure is an important factor in those patients having severe hemorrhoidal bleeding. The presence
of coagulation defects may also be of considerable importance.

fistulas. The incidence of wound infection was not significantly improved by the use of systemic or nonabsorbable intestinal
antibiotics. Intraperitoneal drainage alone or in combination with subcutaneous drainage resulted in the highest rate of wound
infection. However, the use of intraperitoneal drains seems justified for the control of fecal fistula if i...

Abstract Crohn’s disease of the colon and small bowel is associated with a greater-than-chance increased incidence of adenocarcinoma,
which also differs from the norm in age and anatomic distribution. The cancer risk appears to rise with earlier age of onset
and long duration of the granulomatous disease, and the prognosis is relatively poor, probably due to difficulty in diagnosis
because of the overlapping Crohn’s disease. Two additional cases of adenocarcinoma arising in Crohn’s disease are described.

Abstract Colitis cystica profunda is a benign disease of the colon. Its importance lies in differentiating it from mucus-producing
adenocarcinoma. It has rarely been described in the surgical literature. A review of records of patients seen at the Mayo
Clinic produced 66 clinical cases of localized colitis cystica profunda, and in 21 patients the diagnosis was confirmed histologically.
Follow-up, which was available in all patients, ranged from 2 months to 29 years, with a mean followup of more than 8 years.
The data suggest that local excision is the preferred initial therapy.

Abstract A retrospective review of 1353 cases of acute perforated sigmoid diverticulitis treated surgically demonstrates that those

for both diffuse peritonitis and localized abscess.

80 per cent were improved, with 69 per cent totally free of all symptoms. A subsequent hemorrhoidectomy was required in 7.5
per cent. Results were similar when either bleeding or prolapse was the primary indication for treatment. Patients who had
a single band applied fared as well as those with two or more ligations.

using a rectal internal stent and gauze packing techniques for rectal mucosal stripping, with, in some patients, a...

Abstract A technical variant of the end-to-end colorectal anastomosis performed with the EEA stapler is described. It realizes a side-to-end
anastomosis, performed via the oral-aboral route. In this manner, indications for stapled anastomoses can be extended to patients
with discrepancy in bowel size, and where operative findings involve low anterior resection, with the patient not in the lithotomy—Trendelenburg
position. The technique has been used with safety in 17 patients with colorectal and ileorectal anastomoses.

Abstract A report of an innovative approach to the treatment of locally recurrent rectal cancer following abdominoperineal resection
is presented. Curative doses of external beam radiation therapy could not be delivered to an area of locally recurrent cancer
because of contiguous small-bowel loops. The patient received 5040 rads external beam radiation therapy, and then underwent
intraoperative electron beam boost to the main tumor-bearing area. An additional 1500 rads was delivered with 23 MeV electrons.
This approach can be safely applied to selected patients with residual, primarily unresectable, or local...

Abstract From a review of the literature and detailed study of 16 cases, the authors believe that ureteral obstruction in diverticulitis
is more common than is apparent from the literature. They suggest that urograms be performed preoperatively in patients destined
to undergo intestinal surgery. Management of the intestinal disease need not be altered from that initially planned for treatment
of diverticulitis except with the proviso that the retroperitoneum be examined for the presence of a fibrotic inflammatory
reaction. If this is found, they suggest that ureterolysis of the involved ureter be performed t...

Abstract A clinical study to evaluate the Makari Intradermal Test (MIT) involved 180 patients seen with sympotoms suggestive of malignant
disease, 85 of whom were subsequently shown to have carcinoma of the large bowel, and 66 asymptomatic volunteers. The prognostic
value of initial and serial studies relative to patient-survival rate and the efficacy of serial studies in detecting disease
in long-term follow-up of patients with resected malignant lesions were evaluated. On the basis of this study, the MIT appears
to merit further investigation, not as a definitive diagnostic procedure, but as a survey for i...
