EndoscopyDOI: 10.1055/s-0029-1215321Development of an innovative method of endoscopic laser-supported diaphanoscopy, for precise demonstration of the location of gastrointestinal stromal tumors (GISTs) at laparoscopy is described. The equipment consists of a light transmission cable with an anchoring system for the gastric mucosa, a connecting system for the light source, and the laser light source itself. During surgery, transillumination by laser is used to show the shape of the tumor. The resection margins are then marked by electric coagulation. Ten patients have been successfully treated using this technique in laparo...

This study was a retrospective review of the management of symptomatic stent-induced pancreatic duct injury following stent placement for prevention of post-ERCP pancreatitis in eight patients with previously normal pancreatic ducts. Subsequent treatment included pancreatic sphincterotomy, balloon dilation of stricture, and placement of multiple 3 – 5-Fr soft polymer pancreatic stents. All patients showed improvement or resolution of pancreatic strictures. Five patients had resolution or substantial improvement of pain, one patient showed a fair response with repeated ERCPs, and two patients failed to res...

EndoscopyDOI: 10.1055/s-0029-1215269AbstractThe worldwide epidemic of obesity is forecast to worsen with a concomitant increase in the burden of co-morbid conditions. Bariatric surgery has some disadvantages, and intragastric balloons (IGBs) represent a generally safe, reversible and less invasive approach to weight reduction, based on occupying gastric space to enhance the sensation of satiety. A literature review found that the majority of the published data on IGBs relates to the Bioenterics intragastric balloon (BIB). The evidence for the efficacy of the BIB as a primary means of weight loss is still unconvincing, but ...

EndoscopyDOI: 10.1055/s-0029-1215231 Secure transluminal closure remains a fundamental barrier to clinical introduction of natural orifice transluminal endoscopic surgery (NOTES). Current NOTES closure modalities either do not provide secure closure or are too challenging to apply in vivo. The aims of this study were to evaluate gastric closure using the over-the-scope clip (OTSC) system in a previously described experimental setup, comparing the acute strength with a gold standard (hand surgical suturing).[...]© Georg Thieme Verlag KG Stuttgart · New YorkGet connected:Table of contents | Abstract | Full text (Source: Endoscopy)

In conclusion, capsule endoscopy
provided information additional to conventional endoscopy in patients with
juvenile polyposis syndrome and was well tolerated. However, no lesions
requiring clinical intervention were identified and polyp numbers were small.
Capsule endoscopy may appropriately be used as a baseline investigation for the
identification of patients with large or dense small-bowel polyps for whom
ongoing small-bowel investigation would be recommended. Patients in whom polyps
are confined to the colon are unlikely to require ongoing small-bowel
review.[...]© Georg Thieme Verlag ...

An Olympus prototype blind probe, with an external diameter of 6 mm and a 2.8-mm working channel, was used as a second endoscopic arm. Its purpose was to lift the lesion during dissection. The main endoscope served both to perform the dissection and to visualize the second endoscopic arm in the monitor. EndoscopyDOI: 10.1055/s-0029-1215062A patient with an early gastric cancer was treated initially by endoscopic submucosal dissection (ESD) at our hospital. The resected specimen showed a depth of submucosal infiltration of more than 500 µm. Therefore, in accordance with the extended criteria with regard to EMR, the patient underwent gastrectomy. The TNM stage was pT1 (sm3), pN0 (0 / 58), cM0, L0, V0, G2 after surgery (UICC stage Ia). At 3 months later ultrasound revealed a new mass in the liver, and biopsy showed a rapidly growing metastasis of the gastric adenocarcinoma. Finally, immunohistochemical ana...

EndoscopyDOI: 10.1055/s-0029-1215087There is little guidance on the performance of endoscopic retrograde cholangiopancreatography (ERCP) in patients with previous pancreatoduodenectomy. We reviewed techniques for ERCP with a conventional endoscope and assessed its value in 10 patients with previous pancreatoduodenectomy (15 ERCPs). After exploration of the surgical reconstruction, we used a front-viewing endoscope, and we used a small firm pillow under the abdomen and hand compression for preventing loop formation. Successful insertion to the ductal anastomoses and biliary cannulation were achieved in 13 /̴...

EndoscopyDOI: 10.1055/s-0029-1215086Endoscopes, including duodenoscopes, are medical devices that are frequently associated with outbreaks of nosocomial infections. We investigated an outbreak of multidrug-resistant sepsis affecting three patients after endoscopic retrograde cholangiopancreaticography (ERCP). Epidemiologic investigation supplemented by molecular typing revealed that one ERCP scope was the source of infection with No contamination with this microorganism was found after screening of washer-disinfectors, connecting tubes, and environmental surfaces in the endoscopy center. isolates from blood and endoscope c...
