Comparison of outcomes in emergency department revisiting patients before and after coronavirus disease 2019 epidemic
Background and importance
The outbreak of COVID-19 challenged the global health system and specifically impacted the emergency departments (EDs). Studying the quality indicators of ED care under COVID-19 has been a necessary task, and ED revisits have been used as an indicator to monitor ED performance.
Objectives
The study investigated whether discrepancies existed among ED revisiting cases before and after COVID-19 and whether the COVID-19 epidemic was a predictor of poor outcomes of ED revisits.
Design
Retrospective study.
Settings and participants
We used electronic health records data from a t...

Age- and sex-related differences in community-acquired pneumonia at presentation to the emergency department: a retrospective cohort study
Background and importance
Because of its associated high morbidity and mortality, early identification and treatment of community-acquired pneumonia (CAP) are essential.
Objectives
To investigate age- and sex-related differences in clinical symptoms, radiologic findings and outcomes in patients presenting to the emergency department (ED) with CAP.
Design
Retrospective cohort study.
Setting and participants
Patients admitted to one Swiss ED with radiologically confirmed CAP between 1 January 2017 and 31 December 2018.
Outcome measures and analysis
Primary aim was to evaluate differences in cl...

Prospective validation of a transcriptomic severity classifier among patients with suspected acute infection and sepsis in the emergency department
Conclusion
The IMX-SEV-2 classifier showed a superior prediction of in-hospital mortality compared to biomarkers and clinical scores among ED patients with suspected infections. No improvement for predicting multiorgan failure was found compared to established scores or biomarkers. Identifying patients with a high risk of mortality or multiorgan failure may improve patient outcomes, resource utilization and guide therapy decision-making. Prognostic value of serial score measurements of the national early warning score, the quick sequential organ failure assessment and the systemic inflammatory response syndrome to predict clinical outcome in early sepsis
This study was performed in the emergency department (ED) of a tertiary-care teaching hospital. Adult medical patients with (potential) sepsis were included.
Outcome measures and analysis
The primary outcome was clinical deterioration within 72 h after admission, defined as organ failure development, the composite outcome of ICU-admission and death. Secondary outcomes were the composite of ICU-admission/death and a rise in SOFA at least 2. Scores were calculated at the ED with 30-min intervals. ROC analyses were constructed to compare the prognostic accuracy of the scores.
Results
In total, 1750 patients were...

Comparison of the safety and efficacy of YEARS, PEGeD, 4PEPS or the sole item “PE is the most likely diagnosis” strategies for the diagnosis of pulmonary embolism in the emergency department: post-hoc analysis of two European cohort studies
Conclusion:
In this post-hoc analysis of patients with suspicion of PE, YEARS and PEGeD combined with PERC, and 4PEPS were safe to exclude PE. The safety of the modified simplified MODS strategy was also confirmed. There was no significant difference of the failure rate between strategies. (Source: European Journal of Emergency Medicine)

Organisational models at the emergency department to reduce hospital admissions among paediatric patients: a systematic literature review
Paediatric attendances at the emergency department (ED) are often admitted to the hospital less than 24 h to allow time for more extended evaluation. Innovative organisational models could prevent these hospital admissions without compromising safety or quality of delivered care. Therefore, this systematic review identifies evidence on organisational models at the ED with the primary aim to reduce hospital admissions among paediatric patients. Following the PRISMA guidelines, three bibliographic databases (Ovid Medline, Embase, and Cochrane Library) were searched. Studies on organisational models in Western countries, pu...
