This study aimed to evaluate the importance of using the buccal myomucosal flap in cleft palate repair. This is a retrospective
comparative study between two centers in which almost the same technique of cleft palate repair is used. The main difference
in the repairs is that a buccal myomucosal flap is used as a part of the operation in one center and not in the other. The
patients were divided into two groups. Group A was composed of the cases operated at the Craniofacial Institute in Southfield,
MI, USA by the second author. In these, a buccal myomucosal flap was used as a step in the cleft palate repair. Group B con...

Abstract The chimeric anterolateral thigh (ALT) free flap with vastus lateralis (VL) has been our workhorse for soft tissue head and
neck reconstruction following cancer resection. Using the distal portion of the VL muscle, as part of the chimeric flap, which
is based on the descending branch of the lateral circumflex femoral artery and supplied separately from the skin paddle of
the flap, has proved to be a more flexible tool for coverage of these extensive and multidimensional defects. The ALT flap
has been a reliable soft tissue source and has shown superiority over other flaps for head and neck reconstru...

We present the case of a 54-year-old woman who underwent a two-stage breast reconstruction with a tissue expander after sustaining
a lumpectomy and local radiotherapy for breast cancer. During expansion, the woman developed an abnormal concave deformity
of the chest wall. Although respiratory or aesthetic consequences were expected, our patient reported only pain and was satisfied
with the end result. Osteoporosis or local recurrence was excluded as predisposing factors, and radiotherapy was considered
to be the causal factor in our patient. On the basis of this finding, we advise surgeons to take the risk of chest-wal...

Abstract Liposuction has been used as a surgical method of shaping the body for more than 30 years. Although of considerable interest,
there are very few studies dealing with preoperative and postoperative blood parameters. There are no papers reporting what
goes on in a patient's blood after laser-assisted lipectomy. Standard blood samples were evaluated in patients before and
after lipectomy. We compared values of hemoglobin, erythrocytes, hematocrit, and leukocytes. We performed the same evaluation
in patients undergoing laser-supported lipectomy. Moreover, we compared the decrease in postoperative v...

Abstract Microdialysis can be used to detect tissue ischemia. In some experimental and clinical setups, the microdialysis catheter
is subjected to external pressure, for example, during monitoring of free flaps or during pressure ulcer research. However,
it is not known how external pressure affects the properties of the microdialysis membrane. Therefore, the aim of this study

Abstract Lipofibromatous hamartoma (LFH) of nerve is a rare benign, tumour-like fibro-fatty malformation of the epi- and perineurium.
Major peripheral nerves of the upper extremity are mostly involved, most commonly the median nerve and its branches. Although
there is a relatively large number of publications on LFH, involvement of digital nerves is rarely reported or described.
A case of LFH involving the digital ulnar nerve of the index finger without macrodactyly is presented; the aetiology, pathogenesis,
differential diagnosis and surgical management are described.

In conclusion,
the increase in temperature is most likely to be caused by facilitated heat conduction from the body core caused by compression
of the tissues. An increase in temperature as a result of loading may make the tissues even more vulnerable to pressure-induced
ischemia due to an increase in metabolic demand.

reason, minimal traumas may result in fractures during an epileptic seizure. However, mandible fracture cases have rarely
been reported. To the authors’ knowledge, there is nothing published on subcondylar fracture of the mandible as a result of
an epileptic seizure, probably because of their very low incidence.

of emergency physicians and plastic surgeons in the management of snakebites. In general, there was a good level of previous
experience of managing snakebites abroad in the vast majority of cases. However, the survey findings indicated poor knowledge
of the consequences of snakebites and the administration of antivenom. The authors feel better awareness and training for
clinical staff who may be asked to deal with such problems would be of great benefit. Further, public awareness for holiday
makers should be improved in an eff...

Abstract Soft-tissue reconstruction for isolated heel defects represents a challenging surgical dilemma due to tight inelastic tissue
and inevitable acute angulations of the fibro-vascular pedicle of local flaps. A modified pedicled flap with a tongue-like
skin extension has been designed to eliminate tunnel tightness to avoid traction injury and kinking of the blood vessel. Between
2003 and 2005, eight patients with isolated heel defects were analysed (six men, two women). Their age ranged from 5 to 47 years
(median age = 31). Six were traumatic while two were neuropathic. Reversed sural (n = 4...

Abstract Male circumcision is a standard surgical procedure to remove part of the foreskin for cultural, religious, or medical reasons
such as phimosis. Hypertrophic scars and keloids after male circumcision are very rare postoperative complications. To our
knowledge, only a handful of cases have been published previously. In this case report, we present our experience using a
running-w-plasty after extralesional excision. Postoperative silicone-gel-sheets have been applied for a duration of 3 months.
Using these techniques, we achieved a very satisfactory functional and cosmetic result and our patient ...

Surveillance and early detection of altered biomechanical parameters help keeping reconstructed ulcer healed

Abstract Diabetic ulcers in neuropathic feet are difficult to manage. In earlier studies, appropriate footwear helps healing but recurrent
ulceration, recalcitrant and difficult ulcers need surgical intervention. Keeping surgically reconstructed ulcers healed is
a great challenge in diabetes mellitus patients when the primary cause of ulceration, reduced sensibility of skin persists.
We have developed a system of keeping these surgically treated ulcers under surveillance so that early danger signs are detected.
Easy color-coded printouts of pressures in the foot help us to explain to the patients the severit...

We present an audited method for otoplasty dressing accompanied by an intraoperative
photographic sequence. Proflavin-soaked wool is molded into the anatomical sulci anteriorly and a combination of Vaseline
gauze and dressing gauze are held in position posteriorly by tying the continuous skin suture over them. Gauze is then placed
over the pinna with a thick soft piece of cotton wool covering the whole area bilaterally. Finally, a length of stockinet
is used to secure the dressings in place for 10 days, ensuring support, yet without undue pressure. An audit of the cases
we have done so far demonstrated no complic...

We report our experience of treating three patients for scrotal reconstruction by using the anterolateral thigh island
flap. Three patients received scrotum reconstruction with the anterolateral thigh island flap from 2003 to 2007. Two of them
had Fournier’s gangrene and one had hidradenitis suppurativa. In two occasions a “true” perforator flap was used and in one
a small muscle cuff of the vastus lateralis muscle was also included. All three flaps were successful. One patient had a proximal
donor site wound dehiscence due to inadequate drainage. Simple conservative treatment resolved the problem. No patient had...

Abstract Anterolateral thigh flap (ALTF), since its first description, has challenged the role of the radial artery forearm free flap
(RFFF) in head and neck reconstruction. The aim of our study was to objectively compare the two flaps in head and neck reconstruction.
From March 2006 to January 2008, 61 head and neck reconstructions were done, 33 using RFFF and 28 using ALTF. Mean patient
age was 46.93 years. There were 54 tumor and six trauma cases. Flap elevation time, pedicle length and diameter, flap failure,
and re-exploration rates were not significantly different between the two flaps (p value ...

Abstract It is known that the desired shape and position of the nipple–areola complex may be difficult to achieve in vertical-scar
reduction mammaplasty. The marking of a mosque-shaped areolar pattern varies from one surgeon to another, and therefore, periareolar
trimming or resection may be inevitable with the use of such technique. We have developed a device to standardize the periareolar
marking, and reduce the irregularity of the periareolar region. This device mimics the elasticity of normal breast tissue,
and has the flexibility to be applicable to all breast types. We believe that this device improv...

Abstract A high-flow vascular malformation of the face is a rare entity, and it can cause severe disfigurement and can even be potentially
fatal as a result of massive hemorrhage. The authors describe a high-flow vascular malformation case that was successfully
treated by compartmentalization and sclerotherapy with polidocanol. Two weeks later, the necrotic tissues were debrided and
the resulting defect was covered with a skin graft. The resulting aesthetic outcome was more than anticipated by the patient.
With this technique, it was possible to treat what was considered an untreatable vascular malformation ...

Abstract Owing to the relatively low number of patients with cutaneous leishmaniasis seen in plastic surgery clinics, these lesions
may easily be confused with other skin diseases and cutaneous malignancies. Seven patients with cutaneous leishmaniasis, who
had been misdiagnosed as cutaneous malignancies, were referred to our plastic surgery department. The final diagnoses were
made using a Tzanck smear examination and histopathologic examination. Systemic or intralesional meglumine antimonite was
successfully used to treat the lesions. Plastic surgeons should be aware that some atypical skin lesions might be...

Abstract Our society is experiencing a pandemic of obesity, and bariatric surgery is therefore becoming more commonplace. Plastic surgery
procedures to salvage functional and cosmetic repercussions of massive weight loss are also experiencing exponential growth
such as resection of the panniculus morbidus following gastric bypass surgery. Suspending this mass above the patient has
the clear advantage of lifting the weight from the operative field and facilitates venous drainage from the tissues to be
resected. In this paper, we describe the use of an orthopaedic frame to assist with large panniculectomies. I...
