Question: A 67-year-old man with diabetes mellitus presented to our emergency department with hematemesis. He had undergone laparoscopic cholecystectomy 2 months previously. On arrival, his blood pressure was 97/50 mmHg and his heart rate was 101 bpm. Laboratory tests revealed a hemoglobin concentration of 9.3 g/dL (normal, 12.3–18.3), elevated urea (29 mg/dL; normal, 8.0–20.0), and normal creatinine (0.9 mg/dL). Liver function tests and prothrombin time were within normal limits. He denied taking aspirin or nonsteroid anti-inflammatory analgesics. (Source: Gastroenterology)

Question: An 87-year-old woman presented to her cardiologist with a 4-week history of dysphagia and a 20-pound weight loss. Three years previously, she developed a 4-cm Stanford type B descending thoracic aorta aneurysm. She also had hypertension and vertebral compression fractures. We read with great interest the article by Söderlund et al evaluating cancer occurrence and mortality in a population-based study from Sweden including 7,607 patients with ulcerative colitis (UC) or Crohn's disease (CD) who were diagnosed from 1954 to 1989. Data on colorectal cancer (CRC) were available through 2004. The authors demonstrated that over the past 35 years the risk of death from CRC declined markedly within the inflammatory bowel disease cohort. (Source: Gastroenterology)

We appreciate the interest that our concern has generated regarding the application of the Clavien system for surgical complications in the A2ALL study. We admire the philosophy and the achievements of the multicenter A2ALL study group, and will continue to follow their reports with keen interest. Morbidity in living donors continues to be the most serious concern in the field, and we consider discussions of this aspect valuable towards strengthening our global collaboration as medical professionals to improve patient care. One of the primary aims of the Adult-to-Adult Living Donor Liver Transplantation Cohort Study (A2ALL) is the analysis of short- and long-term outcomes in living liver donors. We have adopted the use of the severity grading system for complications described by Clavien in 1994 and still believe it is the best grading system available to classify donor complications. Question: A 34-year-old woman was referred for workup of severe cachexia (26 kg, 160 cm; body mass index, 10.2 kg/m2; highest weight, 40 kg), new-onset intractable vomiting and upper abdominal pain. Question: An 88-year-old woman ate dry, crispy Yorkshire pudding causing slight epigastric pain. Thirty minutes later she developed sudden-onset, severe, central chest pain radiating to the back and epigastrium, associated with shortness of breathe and bouts of hematemesis. Helicobacter pylori infection is a well-established risk factor for gastric carcinogenesis. In animal models, H pylori infection induces a sequence of atrophic gastritis, intestinal metaplasia, dysplasia, and cancer. In some animal models, early eradication of H pylori decreases the incidence of gastric cancer. Whether eradication of H pylori prevents future development of gastric cancer in man remains to be proven. (Source: Gastroenterology)

In 2003, the American Gastroenterological Association established a new group focused on Imaging and Advanced Technology (IAT), which developed new programs for Digestive Disease Week, organized national seminars, and more recently oversaw IAT editorial activities for our journal Gastroenterology.
