We describe complete emergency arterial coronary artery bypass grafting performed on the beating heart of a 73-year-old man
with situs inversus totalis and triple-vessel disease. The right internal mammary artery was anastomosed to the left anterior
descending artery in situ. The first and second obtuse marginal branches of the circumflex coronary and the posterior descending
branch of the right coronary artery were sequentially revascularized using the left internal mammary and radial arteries in
situ. The only abnormality was that the position of the heart mirrored that of a normal heart. Beating heart surgery appear...

Abstract Here we report a minimally invasive cardiac surgery procedure without sternotomy combining video-assisted bilateral pulmonary
vein isolation and minimally invasive direct coronary artery bypass as treatment for atrial fibrillation complicated by coronary
artery disease. The patient’s chest was accessed through a left anterolateral thoracotomy over the fourth intercostal space,
and the left internal thoracic artery (LITA) was harvested under direct vision. The left pulmonary veins were isolated electrically
by a bipolar radiofrequency clamp. Subsequently, the harvested LITA graft was anastomosed to...

Abstract The initiation and development of surgery for acquired valvular heart disease in Japan was reviewed. The first series of attempts
at closed valvular surgery were performed in 1951–1952 by collaboration between the brothers Tohru and Shigeru Sakakibara
in patients with pulmonary or mitral stenosis. During the popularization of closed valvular surgery, open heart surgery under
direct vision was successfully performed by Shigeru Sakakibara with cooling of the body in 1954 and by using cardiopulmonary
bypass (CPB) in 1956. With the development of CPB and artificial heart valves, closed valvular heart ...

had been reduced by the use of DES, but the Achilles’ heel of DES is represented by a higher rate of stent thrombosis. In
our case, the DES thrombosis occurred 5 years after its implantation, underlining the importance of prolonged dual antiplatelet
therapy. Even though rare, this complication may be life-threatening. We believe that CABG provides better event-free ...

Abstract The efficacy of lung cancer screening should not be evaluated by the survival rate of lung cancer patients but by lung cancer
mortality in a certain population because the survival rate can be greatly affected by several types of bias. Randomized controlled
trials that were conducted during the 1970s and 1980s in Europe and the United States failed to prove the efficacy of lung
cancer screening in decreasing the mortality rate; but recently the results of case-control studies in Japan have revealed
that undergoing currently available screening decreases the risk of lung cancer deaths by 30%–60%. A...

years or more, we observe patients carefully. If it does occur, an operation should be considered under limited indications.
We should also recognize that recurrence may occur within a short interval after metastatectomy as well.

augment the left coronary flow. Direct implantation of the left coronary artery and closure of th...

We report a case of surgical treatment
for a papillary fibroelastoma of the aortic valve. The patient was a 62-year-old woman who was asymptomatic for cardiac papillary
fibroelastoma. Echocardiography demonstrated a mobile, rounded mass on the left coronary cusp of the aortic valve. The patient
underwent surgical excision, had an uneventful recovery, and was discharged 10 days after the operation. Pathology examination
confirmed papillary fibroelastoma. Surgical excision of the tumor is recommended for patients with symptoms, but the treatment
of asymptomatic patients is controversial. We believe that surgical treatme...

Abstract Despite recent advances in surgery, irradiation, and chemotherapy, the prognosis of patients with lung cancer is still poor.
Therefore, the development and application of new therapeutic strategies are essential for improving the prognosis of this
disease. Significant progress in our understanding of tumor immunology and molecular biology has allowed us to identify the
tumor-associated antigens recognized by cytotoxic T lymphocytes. Immune responses and tumor-associated antigens against not
only malignant melanoma but also lung cancer have been elucidated at the molecular level. In a theoretical sen...

Abstract A 70-year-old woman was readmitted to our hospital with a fever of 39°C on the 30th day after replacement of a prosthetic
aortic valve. She required percutaneous cardiopulmonary support for her heart failure and was weaned after 7 days. Echocardiography
revealed an akinetic and aneurysmally dilated left ventricular apex and hyperdynamic basal segments. This case indicates that
fulminant apical ballooning syndrome might be added to the list of potential complications after cardiac surgery.

We report a case of apicoaortic bypass using an apical connector for severe aortic stenosis. A 74-year-old woman suffered
from severe aortic stenosis, with a small aortic annulus and a severely calcified aorta. A valved conduit with an apical connector
was placed between the left ventricular apex and the descending thoracic aorta because the risk of aortic valve replacement
was high. Use of an apical connector facilitated a secure connection between the conduit and the left ventricular apex. The
pressure gradient across the native aortic valve fell from 64 mm Hg before the operation to 19 mm Hg afterward. Apicoaortic

We report a 20-year-old woman who underwent complete resection of a granular cell tumor (GCT). On chest computed tomography
(CT) scan, a mass with a maximum diameter of 36 mm at the lower bronchus with atelectasis of the right lower lobe was noted.
Bronchoscopic examination revealed a whitish mass in the truncus intermedius, and the middle and lower bronchus were unable
to be seen. A cytopathological examination of the mass revealed GCT. A right middle and lower lobectomy was performed via
a posterolateral thoracotomy. Microscopically, the tumor was composed of polygonal cells with oxyphilic granular cytoplasm
and sma...

Abstract A 75-year-old woman presented with chronic cough and hemoptysis. Chest computed tomography (CT) and aortography revealed a
small, contrast-filled outpouching in the wall of the descending aorta, which was thought to be the source of bleeding. At
thoracotomy, there were firm adhesions between the descending aorta and the left S6. The aorta was clamped and the adhesions
were removed revealing a defect in the aortic wall with thrombus. The defect was sutured. A penetrating atherosclerotic ulcer
and intramural hematoma were diagnosed based on the radiological and operative findings.

We present a case of left subclavian artery aneurysm in a 48-year-old man with Marfan syndrome. Aneurysmectomy and interposition
with an artificial graft were successfully performed through an infraclavicular incision by dividing the clavicle at its midshaft.
The clavicle bone was reconstructed with a steel plate, and the postoperative course was uneventful. Because the arterial
wall is fragile in cases of connective tissue disorders such as Marfan syndrome, our surgical approach was considered to be
helpful for gentle maneuvering in an adequate operative field.

Abstract A 29-year-old man presented with a mass in the left lower lung field on a chest radiograph obtained during a medical checkup.
Computed tomography revealed a tumor adjacent to the diaphragm. A sessile tumor measuring 10.5 × 8.5 × 4.5 cm arising from
the parietal pleura was resected. The tumor was accompanied by several little tumors on the nearby diaphragm. Pathologically,
the major tumor consisted of typical spindle-shaped cells with myxoid degeneration. There was no increased cellularity, cellular
pleomorphism, or a high mitotic count. In immunohistochemical studies, the spindle cells showed posi...
