[Psychological assistance for men with failures of procreation: Look out for the secret wound!]

When one thinks of the failures of ART one naturally thinks of the suffering that they generate; but behind this pain, there is the suffering of infertility and, behind this latter, other distress we showed at the time of a recent research that they could play a part in infertility itself. The psychotherapy will help assume the failures, so that the traumatisms do not come to solidify in an impossible mourning, covering non elaborate former grieves. A sublimation of the desire of reproduction can make it possible for the unfertile man to peacefully consider other forms of paternity. For it to be possible, it is necessa...

Preimplantation embryo development is one of the key features with implantation itself to achieve a pregnancy. Assisted Reproductive Technologies both in human and animal have improved our knowledge on these events, although it remains elusive to predict embryo potential to give a baby. Among various ways to define embryo viability, noninvasive approaches get a serious advantage linked to the final transfer of the embryo. Techniques devoted to characterize the embryo secretome using proteomic or metabolomic approaches may be non invasive. Based on a direct identification of products of the embryo metabolism or an asses...

A cervical stenosis can follow an infection, a conisation or a trachelectomy and lead to a cervical infertility. In in vitro fertilization (IVF), embryo transfers are difficult in case of a cervical stenosis and specific care is required. Treatments try to restore an adequate cervical patency. Repeated cervical dilatations can represent a solution. In case of inefficiency, a new cervicoisthmic opening has to be created with surgery. Perpetuity of the result is obtained thanks to post surgical dilatations or thanks to implementation of an intracervical catheter till complete cicatrization. In case of failure of canaliza...

Oocyte donation is a complex situation that requires the applicant couple to deal with the presence of the donor in the history of the child conception. Accepting the eggs is not the same thing than accepting the donor. Her place in the child's life depends on how his parents will accept her phantasmal reality beyond her real person. Paying attention to the story told by the donors on their motivations may help parents internalize this conception to three. We show from two clinical observations, that the generosity of donors is connected to personal issues that do not relate to unborn child or its parents. If there are...

From all the possibilities of assessment of the embryonic viability in vitro, morphology observed at microscope is the most elective criteria because of its specificity, sensibility and its armless side. The use of classifications at each stage of the development is essential for the selection of the right embryo to be replaced but do not allow dynamic evaluation. The development of computerized tools to capture and analyse images must emancipate us from oversimplification that doesn't cover frequent daily situations. The purpose is to examine the most frequent widespread classifications, the proposed alternatives and ...

Human Y chromosome evolution has progressively been directed towards a role in sex determination and reproduction. Cytogenetically visible structural abnormalities have determined long arm chromosomal regions which define the AZF factor that contains genes implicated in the spermatogenic process. By using molecular tools, the AZF factor has been subdivided into three loci, AZFa, b and c, the deletion of which leads to specific spermatogenesis impairments due to the loss of particular genes. Most AZF genes are specifically expressed in testis but their functions are far to be known precisely. Partial deletions of AZF re...

Sex chromosome behaviour fundamentally differs between male and female meiosis. In oocyte, X chromosomes synapse giving a XX bivalent which is not recognizable in their morphology and behaviour from autosomal bivalents. In human male, X and Y chromosomes differ from one another in their morphology and their genetic content, leading to a limited pairing and preventing genetic recombination, excepted in homologous region PAR1. During pachytene stage of the first meiotic prophase, X and Y chromosomes undergo a progressive condensation and form a transcriptionally silenced peripheral XY body. The condensation of the XY biv...

Ovarian low malignant potential tumor account for 10 to 20 percent of ovarian epithelial tumors. They differ from typical ovarian cancers in that they do not grow into the ovarian stroma. Likewise, if they spread outside the ovary, for example, into the abdominal cavity, they do not usually grow into the lining of the abdomen. These cancers tend to affect women at a younger age than the typical ovarian cancers and are less life-threatening than most ovarian cancers. Guidelines for surgical treatment of borderline ovarian tumors are similar to those for ovarian cancer and include hysterectomy with bilateral salpingo-oop...

Ovarian gonadotropic inhibition is today an efficacious tool in the treatment of endometriosis mainly when associated with surgery and sometimes by itself. However, to be useful, this inhibition must be stable - without any cyclical looseness - of long duration, sometimes during years and sufficiently powerful. Depending on the severity of symptoms and that of the disease, the choice will be among GnRH agonists, gestagens and combined OCs. The recent development of continuous oral contraception with protracted amenorrhoea makes treatment by continuous hormonal administration easier for the patients with endometriosis.

For men presenting azoospermia, the development of ICSI with epidydimal (MESA) or testicular (TESE) sperm, allowed them to father their own progeny. Little is known about the issue of these ICSI, in terms of efficiency and quality of the conceptus, and many controversies remain. Some studies emphasized that children born after Assisted Reproductive Technology (ART) with surgically sperm retrieved were at increased risk of birth defects. In this context, we proposed a retrospective analysis of pregnancy issues with non-ejaculated sperm.

The ovarian stimulation and the follicular puncture in ART present risks which must be planned in order to better prevent them. The presence of an endometrioma can generate an increase in the infectious risk.

Actually, the choice of surgical approach for pelvic organ prolapse repair depends more on surgeon experience rather than patients characteristics. Laparoscopic sacral colpopexy seems to offer complete repair, low morbidity and satisfactory anatomic and functional results. This technique requires specially trained and experienced surgeons and optimal operative conditions. To improve feasibility of laparoscopic sacral colpopexy, we propose a simplified operative strategy to optimise this complex procedure.

CONCLUSIONS: Perineal complications (severe lacerations) associated with spatulas are increased with regard to SVD, but comparable to that reported with forceps. The main disadvantage is the high frequency of episiotomy, which should not be systematic. Neonatal morbidity is reduced. Comparative studies (spatulas vs. other procedures) are needed to confirm these data, but spatulas remain a multipurpose instrument which should continue to be taught.

Early-stage cervical cancer (IA2 and IB1) treatment consists in surgery with a radical colpohysterectomy associated to a systematic pelvic lymphadenectomy more or less a paraaortic lymphadenectomy (Wertheim-Meigs surgery). Pelvic lymph node involvement is the main independent prognostic factor and justifies an adjuvant therapy. Sentinel lymph node (SLN) procedure allows a reliable evaluation of the lymph node status thanks to an accurate histologic analysis. This procedure is still not validated in practice, which means the bilateral pelvic lymphadenectomy is systematically done whatever the lymph node status is. Never...

Ovarian cryopreservation is presently indicated in patients who undergo a gonadotoxic treatment, most commonly for anticancer procedures. These procedures can strongly alter fertility by damaging the follicular ovarian reserve. Although six human live births have been described in the world after ovarian tissue cryopreservation and autografting, the techniques of cryopreservation techniques are not consensual. Vitrification is a physical process that allows cryopreservation without formation of ice crystals, by transformation of a highly concentrated solution in a glassy or amorphous state. Vitrification is at present ...

Atypical hyperplasia represents 4% of all benign breast diseases. There are two different types: atypical ductal hyperplasia and atypical lobular hyperplasia. Aside columnar cell lesion. They represent an early stage of some forms of low grade carcinoma in situ and invasive carcinomas. Atypical hyperplasia is a benign lesion with intermediate carcinologic risk and the existence of a concomitant aggressive lesion should be suspected. When atypical lesion is found on a biopsy specimen, surgical excision is recommended especially in case of atypical ductal hyperplasia. A regular supervision is recommended.

We report two cases of pregnancy-associated Plasmodium falciparum malaria discovered fortuitously. These two women were born in Africa and their last visit in an endemic area was more than one year before. It is well known that pregnancy is one of major risks of late onset of P. falciparum malaria. In the two cases reported in this study, clinical signs of malaria were not specific and we will describe the interest to detect more systematically pregnant African women, first arrival immigrants.

CONCLUSION: It seems important to establish a modern and low-cost teaching program to improve surgeon's theoretical and practical formation in hysteroscopy in order to increase success rates and reduce possible complications. Theoretical formation has to include a good knowledge of the material and its use: diameter and form of the operative channel, obliquity of the scope, liquid of distension and kind of current used.
