The Hawai'i Breast and Cervical Cancer Control Program (BCCCP) offers free mammograms and Pap smears to women who are uninsured or underinsured through a statewide provider network. This report describes two bilingual patients who presented with early symptoms of dementia after regression to their primary language. The results of this study may help clinicians identify aging bilingual patients who are beginning to develop cognitive impairment or dementia and suggest that further studies on the long term cognitive effects of bilingualism and interactions with the aging process are indicated.

Public health concerns have been raised over the risk of parasitic helminth (roundworm, tapeworm and fluke) infections from eating raw fish, an increasing US consumer trend. Hawai'i consumers eat seafood at nearly 3 times the US national average rate, with a long tradition and high level of raw fish consumption. CONCLUSIONS: Diabetes prevalence in Native Hawaiians is high. The high proportion with IFG and the increase in CVD risk factors with diabetes suggest that community-based programs are needed to focus on diabetes and diabetes-related CVD.

A survey of injuries and medical conditions affecting competitive adult outrigger canoe paddlers on O'ahu.

CONCLUSIONS: Outrigger canoe paddlers report a high prevalence of musculoskeletal injuries and illnesses related to their sport. The shoulder and back were the most common sites of injury. The rib was the most commonly fractured bone. Paddling may also predispose to such environmentally related conditions as heat illness, injury from sea creatures, and perhaps skin cancers. Carcinoid tumor presenting as a primary mesenteric mass: a case report and review of the literature.

We report a case of a primary carcinoid tumor arising in the mesentery of the small bowel of a 52-year-old man. The mesenteric mass in this patient was first identified as an incidental finding on CT imaging for bilateral nephrolithiasis. As a result of the abnormal findings, follow-up evaluation identified a partially calcified lobulated mesenteric lymph node. Diagnostic laparoscopy with lymph node sampling showed no evidence of hepatic or other metastatic disease. We reviewed our ethnically/culturally diverse program to identify factors that contribute to donors' decision to participate. CONCLUSION: Although the weight loss in the study group was moderate, it was significant over the control group whose members actually had an increase in average weight over the study period. A case report and literature review of group C streptococcal pneumonia.

Group C streptococci rarely cause severe disease in humans. The authors report a 15-year-old girl with group C streptococcal pneumonia. No immune deficiencies were detected in our patient and investigation of virulence factors including emmL, sag A, spegg and smeZ genes failed to reveal insight into why the isolated group C streptococcal strain was so virulent. This article describes a study to understand variations in influenza vaccination rates for a population ages 50 and older using data from a large insurer in Hawai'i. Vaccination rates were lower in adults without than with a chronic disease, and especially low in those without chronic disease of ages 50-64. Immune globulin-responsive thrombocytopenia in acute post-streptococcal glomerulonephritis: report of a case in Hawai'i.

A 9-year-old boy in Hawai'i presented with bleeding and thrombocytopenia and was subsequently found to have post-streptococcal glomerulonephritis. He recovered completely with intravenous immune globulin, antibiotics, short-term antihypertensive therapy, and supportive management. This case was similar to the 5 cases previously reported in the literature, with the exception that steroids were not used as the primary immune-modulating therapy.

CONCLUSION: CDH, especially L-pCDH occurs with increased frequency in the USAPIs. The Web-based-store-and-forward telemedicine PIHCP allows for remote diagnosis. Patients so identified are transported to TAMC for definitive treatment. These patients provide excellent GME to our residents-in-training. Following surgical correction these patients are restored to health to return to their homes to mature and become contributing members of society.

This cross-sectional study focused on pain assessment in a group of older Asian Americans with cancer. Thirty-six participants and their primary nurses were interviewed, and pain intensity was measured using 3 different scales: the Numeric pain intensity 0-10 scale, the Faces expression scale, and Visual analog scale (VAS). Pacific Islanders currently comprise 35% of all end-stage renal disease patients in Hawai'i but a much smaller proportion of those who undergo renal transplant. CONCLUSION: In a community in which newborn circumcision rates are high, the frequency of circumcision is declining, while the number of circumcisions performed in children aged 1-18 is increasing. The number of circumcisions performed in non-US born children is disproportionately higher than what would be expected suggesting that the post-infant circumcision rates for this age group is higher in immigrants.

Cultural competence education is relatively new in the United States, particularly in the area of graduate and post-graduate medical education. There is, however, wide acceptance that an understanding of the role culture plays in the treatment and care of patients is critical. Numerous studies and a variety of commentaries document this importance, but valid, uniform evaluation methods for assessing the efficacy of these efforts is lacking. This review discusses existing evaluation efforts and makes suggestions regarding future development of such tools.

Internal medicine residents at the University of Hawai'i were surveyed in 2005 to identify their attitudes regarding ethics consultation based on exposure to ethics education as part of residency training. The purpose of this article is to describe four performance indicators to use as a scorecard to monitor progress in meeting the shortage of registered nurses in Hawai'i. In addition, the RN shortage in the state is described and progress in addressing the situation to date is reviewed. In 2005, a survey was mailed to a stratified, random sample of registered nurses (RNs) residing in the state. The mean age of RNs was 49.3 years, and 69.9% plan to retire by 2020.
