Isolated Inguinal Lymph Node Metastasis from Colon Cancer: A Case Report
We reported a rare case of isolated metachronous inguinal lymph node metastasis from colon cancer with a round ligament route of spread as the hypothesized mechanism. Surgical resection with inguinal LN dissection is the preferred treatment option for isolated inguinal lymph node metastasis from colon cancer followed by adjuvant chemotherapy, yet long term follow-up data is needed to support this strategy. Ectopic Follicular Variant of Papillary Thyroid Carcinoma in Anterior Neck Region with a Normal Thyroid Gland —A Rare Entity
We report a rare case of ectopic follicular variant of papillary carcinoma in a 73-year-old female with a normal orthotopic thyroid gland. Despite a few reports on ectopic papillary carcinoma thyroid (PTC) in submental, thyroglossal duct cysts, and bronchial cleft cysts with normal pathologic examinations of orthotopic thyroid, only one case of PTC in the anterior neck region without any neoplastic involvement of the orthotopic thyroid gland has been reported. Ectopic thyroid carcinoma should be taken into account in the differential diagnosis of a mass lesion in the front of the neck even in the presence of a normal thyro...

Utility of Microvascular Reconstruction in Gastrointestinal Cancer Surgery During Complex Resections and Emergency Salvage
AbstractMajor gastrointestinal surgical resections and subsequent reconstruction can occasionally need arterial or venous resection, can encounter variant anatomy, or may lead to injury to vessels. These can lead to arterial and/or venous insufficiency of viscera like the stomach, liver, colon, or spleen. Left unaddressed, these can lead to, partial or total, organ ischemia or necrosis. This can trigger a cascade of systemic clinical complications resulting in significant morbidity or even mortality. The aim of this case series is to highlight the utility of microvascular plastic surgical principles and practices in counte...

Heterotopic Pancreas: Unusual Cause of Usual Diarrhoea
AbstractHeterotopic pancreas (HP) also known as ectopic pancreas, pancreatic crest or accessory pancreas is the normal pancreatic tissue, found in a remote area other than its natural location, with no anatomic or vascular connection to main pancreatic tissue. It is a rare congenital anomaly and has been reported at many locations such as stomach (antrum) and small intestine. HP is usually an incidental finding and asymptomatic, however there are reports of pancreatitis, obstruction, perforation and malignant transformation as uncommon manifestations. Diagnosis of HP is primarily based on histological examination either by...

Treatment Outcomes of Incidental Gallbladder Cancer – Results from a High-Volume Tertiary Care Centre in North India
We present the institutional experience of a high-volume tertiary care center in northern India.  Retrospective analysis of a prospectively maintained database was performed and data of all IGBC patients between January 2014 to December 2021 was analyzed. There were 125 patients of IGBC among the 750 patients of GBC seen during the study period. Of these 125 patients, 72 (57.6%) patients were not eligible for surgery. Successful completion radical cholecystectomy (CRC) was possible in 37 (69.8%) of the 53 patients who underwent surgery. On univariate analysis, thickness of gallbladder wall 10 mm or more (p <  0.0...

Isolated Limb Perfusion for Extremity Soft Tissue Sarcoma and Malignant Melanoma
AbstractIsolated limb perfusion (ILP) is a regional chemotherapy technique used in the treatment of locally advanced or unresectable extremity soft tissue sarcoma (ESTS) or malignant melanoma (MM) of the limbs. It allows for high concentrations of chemotherapeutic agents to be perfused in the limb while minimising the risk of systemic side-effects. While the technique has been utilized for decades, the role of ILP has evolved as other treatment strategies have become available. Current indications for ILP in sarcoma include induction in unresectable ESTS to allow for future definitive limb preservation procedures  as well...

Osteosarcoma of Mandible — A Case Report
AbstractAfter plasma cell neoplasia, osteosarcoma is the most common primary bone tumor which accounts for 20% of all sarcomas. A malignant tumor is characterized by the formation of bone or osteoid by tumor cells. Long bones are the most common site of involvement. In the jaw bones, approximately 5% of involvement with an incidence of 1 in 1.5 million persons per year. It develops in a broad range of ages but is more common in the third and fourth decades, slightly more often in the mandible than in the maxilla. In this case report, a 24-year-old male patient reported a mass in the left mandible for the past 1  week whic...

Efficacy of Lapatinib Plus Capecitabine After TDM-1 in HER2-Positive Metastatic Breast Cancer Patients
This study evaluated the efficacy and tolerability of the lapatinib plus capecitabine combination after TDM-1 in HER2-positive metastatic breast cancer patients. We retrospectively evaluated the HER2-positive metastatic breast cancer patients ’ data. The patients who progressed after trastuzumab-based chemotherapy and TDM-1 were included in the study. We used the Kaplan–Meier for survival analysis. Eighteen patients were involved in the study. The average age was 48 (range 31–65). De novo metastatic patient’s number was 5 (27.8%) . The most common metastatic sites were liver (50%), lung (44.4%), and brain (44.4%), ...

Flow-Through Radial Artery Forearm Flap for Tongue Revascularization After Excision of Base of Tongue Malignancies
We report here two cases of base of tongue excision for which free flow-through radial forearm flap was used to reconstruct the tongue defect as well as salvage the remaining part of ischemic tongue. This technique can be effectively considered in selected cases of vascular compromise due to lingual vessel sacrifice to preserve the cancer-free anterior native tongue tissue with improved functional outcomes. Cartilaginous Transdifferentiation in Melanoma: A Diagnostic Challenge
AbstractMalignant melanoma  is a formidable tumor originating from melanocytes of neural crest origin, found in various anatomical locations, primarily in the skin, followed by the eyes and mucosal membranes. This tumor stands out due to its remarkable phenotypic diversity. Transdifferentiation, the process of differentiatio n into cell lineages other than the one from which the tumor originated, and phenotypic plasticity, characterized by changes in behavior, morphology, and physiology in response to different environmental conditions, can make melanoma a diagnostic conundrum for unwary pathologists. In this case repor t...

Long-Term Outcomes of Recurrent Laryngeal Nerve Repair/Reconstruction in Oncological Settings
This article aims to convey our experience with RLN resections/injuries and their subsequent effects. We assessed the RLN repairs that our institute had completed from April 2018 to September 2023(5  years and 5 months) including follow-up of minimum 1 year. The Functional Oral Intake Scale (FOSI) was applied to assess dysphagia, aspiration risk, and glottic gap by laryngoscopy, and GRBAS (Grade, Roughness, Breathiness, Asthenia, Strain) comprised the basis for the assessment of voice qualit y by speech specialists. Surgical technique included using 9–0 Ethilon either for primary repair or for repair with a nerve graf...

Unusual Presentation of Squamous Cell Carcinoma in a Young Male: Clinicopathological Insights and Considerations
We present a case of a 21-year-old male student who presented with a painless, gradually increasing, occasionally bleeding, nodulo-ulcerated lower lip lesion. The lesion was removed 3  months back with a carbon dioxide laser, and this was a recurrent lesion. We did lesion biopsy which confirmed squamous cell carcinoma. The patient started treatment at the T2N0M0 stage. Patient has responded well to the radiotherapy treatment. Our case had no known predisposing risk factors for c arcinoma. For etiopathogenesis in this case, role of prior lesion removal with carbon dioxide laser is debatable. As laser treatment does not spa...
