A lack of human resources is a significant barrier to improving obstetric care, especially in rural areas. This is true in many low-income and some high-income countries. Emigration of doctors and nurses from many low-income countries contributes to this, as does a widespread unwillingness—for many reasons—to work in rural areas . There are several ways to address this shortage, one of which is task shifting, and this issue of the Keystone Section is devoted to this topic. Conclusions: This innovative program shows that, at least in some areas of India, it is possible to develop a large scale partnership with the private sector to provide skilled birth attendance and emergency obstetric care to poor women at a relatively low cost. This is one way of addressing the human resource deficit in the public sector in rural areas of low-income countries to achieve Millennium Development Goals 4 and 5. A 33-year-old primigravida was admitted following detection of uterine abnormalities on ultrasound at 24weeks of pregnancy. Use of traditional Chinese medicine among pregnant women in Taiwan

Abstract: Objective: To investigate the patterns of traditional Chinese medicine (TCM) use among pregnant women in the National Health Insurance program in Taiwan.Methods: Women who gave birth in Taiwan in 2006 were identified from the National Health Insurance Research Database. Claims for reimbursement following TCM ambulatory visits by these women were analyzed.Results: In total, 20.9% of women in the study who gave birth in 2006 used TCM during pregnancy, with older women more likely to utilize this form of care (23.9% of women >35 years of age vs 16.2% of women (Source: International Journal of Gynaecology and Obstetrics)

Lymphangioma circumscriptum is a benign malformation of inner lymphatic channels that can extend to the dermis, the subcutaneous layers, and rarely the muscle. Abstract: Objective: To compare the accuracy of transvaginal (TVS) versus transabdominal (TAS) ultrasound to assess the thickness of the lower uterine segment (LUS).Methods: Eighty-three pregnant women admitted for an elective cesarean delivery were enrolled. LUS thickness was measured using both TVS and TAS prior to the cesarean.
