Acute procedural efficacy and safety of a novel expandable diameter cryoballoon in atrial fibrillation ablation: Early results from a multicenter ablation registry
ConclusionsOur real-world early data demonstrated high acute efficacy and safety of the novel-sized adjustable CB. The biophysical parameters were similar between the 28  and 31 mm balloons. No marked decrease in the incidence of PNI was observed even with 31 mm balloons. (Source: Journal of Cardiovascular Electrophysiology)

Comparison of electrogram characteristics in persistent atrial fibrillation
ConclusionMultiple techniques can be employed for electrogram analysis in AF. In this analysis of eight  different electrogram characteristics, seven were poorly to moderately correlated and do not identify similar locations. Only some characteristics were predictive of ablation outcome. Further studies to consider electrogram properties, perhaps in combination, for categorizing and/or mapping AF are warranted. Cardiac stereotactic radiation therapy for refractory ventricular arrhythmias in patients with left ventricular assist devices
AbstractLeft ventricular assist device (LVAD) implantation is an established treatment for patients with advanced heart failure  refractory to medical therapy. However, the incidence of ventricular arrhythmias (VAs) is high in this population, both in the acute and delayed phases after implantation. About one-third of patients implanted with an LVAD will experience sustained VAs, predisposing these patients to worse outcom es and complicating patient management. The combination of pre-existing myocardial substrate and complex electrical remodeling after LVAD implantation account for the high incidence of VAs observed in ...

Managing ventricular arrhythmias and implantable cardiac defibrillator shocks after left ventricular assist device implantation
AbstractContinuous flow left ventricular assist devices (CF-LVADs) have been shown to reduce mortality and morbidity in patients with advanced heart failure with reduced ejection fraction. However, ventricular arrhythmias (VA) are common, are mostly secondary to underlying myocardial scar, and have a higher incidence in patients with pre-LVAD VA. Sustained VA is well tolerated in the LVAD patient but can result in implantable defibrillator (ICD) shocks, right ventricular failure, hospitalizations, and reduced quality of life. There is limited data regarding best practices for the medical management of VA as well as the rol...

Cardiac resynchronization using fusion pacing during exercise
ConclusionFusion pacing is maintained during exercise without impairing exercise capacity compared with fixed AVD. However, using whichever algorithm gives the narrowest QRSd at rest is associated with a narrower QRSd during exercise, higher peak stroke volume, and improved cardiac efficiency. Left atrial strain is a good predictor of atrio ‐ventricular synchrony in leadless pacemaker pacing
ConclusionPreimplant echocardiographic evaluation of the atrial contractility may be useful in predicting adequate A4 sensing and consequently a good atrio-ventricular synchrony pacing. Echocardiography LA strain study seems promising in Micra-AV patient selection. (Source: Journal of Cardiovascular Electrophysiology)

High ‐power short‐duration versus low‐power long‐duration ablation for pulmonary vein isolation: A substudy of the AWARE randomized controlled trial

Pulsed ‐field ablation does not induce esophageal and periesophageal injury—A new esophageal safety paradigm in catheter ablation of atrial fibrillation
ConclusionIn contrast to thermic ablation tools, PFA is not associated with relevant esophageal and periesophageal injury, and might, therefore, reduce or eliminate the risk of potentially lethal AEF in interventional treatment of AF. The etiology of ablation-induced periesophageal edema is unknown but has not been shown to be related to lesion progression. (Source: Journal of Cardiovascular Electrophysiology)

Catheter ablation of typical atrial flutter improves cardiac chamber size and function
ConclusionsPatients who underwent CTI dependent AFL ablation showed an improvement in cardiac size and function at follow-up evaluation. While long-term results are still unknown, these findings indicate that restoration of sinus rhythm in patients with typical AFL is associated with improvement in atrial size and left ventricular function.
