Is ultrasound screening for DDH in babies born breech sufficient?

Conclusions Retrospectively, we found that, at approximately 6 weeks of age, ultrasound screening of breech patients with clinically stable
hips produces an incidence of DDH of 27%. In those patients with a normal ultrasound, 29%, at 4–6 months radiographic follow-up,
were found to have dysplasia requiring treatment. This data supports breech as the most important risk factor for hip dysplasia
and we, therefore, recommend careful and longitudinal evaluation of these patients with: a careful newborn physical examination,
an ultrasound at age 6 weeks, and an anteroposterior (AP) pelvis and fro...

Conclusions In the OCA technique, meticulous primary detersion and exact anatomic positioning of the damaged nail bed is mandatory before
applying the cyanoacrylate adhesive. In our experience, we found that the OCA technique provided a fast and safe result in
nail bed lesions. The final cosmetic appearance, function and patient satisfaction were good in our series. In these lesions,
discomfort and inconvenience associated with suture removal could be avoided by the use of OCA.

Conclusions The presented fracture pattern is generally undervalued when evaluating management in children; the reduction of this type
of injury should be surgical in order to achieve an adequate alignment. Parents should be informed about the treatment, its
evolution, and possible subsequent surgeries to reestablish the joint axes and the adequate length of the wrist bones.

Conclusions Children whose transcervical fractures of the femoral neck were surgically treated after a delay of 1 week or longer had a
high avascular necrosis rate and a relatively poor outcome. Reduction and internal fixation as the initial treatment should
be implemented shortly after injury in order to enhance treatment outcome.

Conclusion The MAC fixator seems to be a good alternative modality of stabilization and correction for severe congenital radial club
hand deformities. Its usage is fairly simple and it provides the ease of application of a mono-lateral fixator with far superior
three-dimensional control, like the circular external fixator. We recommend that clinicians should add this modality to their
armamentarium for the deformity correction of severe radial club hand and others in general.

Conclusions We demonstrate a simple, fast and reproducible surgical technique for percutaneous epiphysiodesis with low morbidity, rapid
rehabilitation and a rapid return to school and sports activities. We experienced no complications, such as overcorrection,
undercorrection, postoperative hematoma or infection. We conclude that percutaneous screw epiphysiodesis is an excellent option
for the treatment of genu valgum in adolescents.

fracture treatment using either plaster or fiberglass, but wish to emphasize the importance of not only the cast index, but

Conclusions We described five patients with a claw toe deformity following a tibia fracture associated with adhesions of the flexor hallucis
longus (FHL) and flexor digitorum longus (FDL) muscles to the surrounding structures under or just proximal to the flexor
retinaculum. It is the authors’ opinion that this condition may be related to a subclinical compartment syndrome localized
in the distal part of the deep posterior compartment. Soft-tissue release without tendon lengthening allowed recovery in all
patients.

Conclusions Operative treatment affords a significantly higher union rate over the non-operative management of medial epicondyle fractures.
There was no difference in pain at final follow-up between operative and non-operative treatments. As surgical indications
evolve, and the functional demands of pediatric patients increase, surgical fixation should be strongly considered to achieve
stable fixation and bony union.

Conclusion Our study results suggest that femur fractures in children with cerebral palsy can be treated non-operatively; however, because
of the high risk of malunion in this patient population, fracture alignment needs to be followed closely during healing. Careful
attention during casting is necessary to prevent pressure sores. Strong consideration should be given to initial operative
treatment in ambulatory patients in order to preserve function.

Conclusion Predicting blood loss for femoral rodding in patients with OI is difficult, with no differences between revision and primary
procedures or elective versus trauma cases. The blood loss in our patients undergoing femoral rodding was manageable, and
the transfusion rate was reasonably low. Although massive blood loss has been described in patients with OI in the literature,
we found that femoral rodding did not pose excessive risk of transfusion in our OI population.

Conclusion The preoperative c–b index of the open osteotomy group was statistically greater than the c–b index of the closed osteotomy
group and the preoperative h–b index of the open Salter group was statistically smaller than the h–b index of the closed Salter

Conclusion Mirror foot is a very rare congenital deformity of the foot. We successfully treated three novel cases of mirror feet. A classification

Conclusions The diagnosis of pediatric skeletal tuberculosis can be made with good correlation of clinical, radiographic, and laboratory
findings. Biopsy and culture are the gold standards in diagnosis. Prognosis is good with chemotherapy and non-operative management.

Grice arthrodesis in the treatment of valgus feet in children with myelomeningocele: a 12.8-year follow-up study

Conclusions Based on the radiological findings and patient satisfaction, the patients participating in this study benefited from having
had Grice arthrodeses performed on their valgus unstable feet. The results indicate good long-term correctional effect on
valgus deformity after Grice arthrodesis, as the talo-calcaneal and talo-first-metatarsal angles improved significantly. A
great majority of the patients were content with the surgery, and none claimed that any residual deformity was the cause for
any reduced ability to ambulate.

Conclusions In contrast to its success in upper limb reconstruction, in this series of three patients with hip articular destruction,
articular reconstruction using a vascularized proximal fibula graft was disappointing and led to unsatisfactory results in
terms of hip reconstruction. Such a procedure is complex and highly demanding, necessitating extremely intensive post-operative
care. An early slipped capital epiphysis can lead to full graft resorption. Consequently, despite important adaptation and
remodeling of the graft, the authors do not recommend this procedure at this location.

Discussion Our results are in accordance with the literature. The femoral overgrowth was proportional to the initial shortening. Masculine
gender, an oblique fracture and injury of the lower third of the femur were associated with the greatest femoral overgrowth.

Conclusions Our study shows that leg muscular atrophy is a primitive pathological component of CCF which is already present in the early
stages of fetal CCF development and in newborns before starting treatment. Muscular atrophy increases with the patient’s age,
suggesting a mechanism of muscle growth impairment as a possible pathogenic factor of CCF.

Conclusions Whatever the treatment, the outcome is poorer with increasing age. No treatment guarantees a good result for significant head
involvement or initial deformity but acetabular augmentation improves C/B ratio and center–edge angle and prevents progressive
femoral head deformity compared with no treatment, varus osteotomy, and plaster treatment.

Conclusion While not indicated in all cases of femoral head dislocation, MRI is useful to confirm concentric reduction of the femoral
head in a dysplastic acetabulum when examination under anaesthesia and radiographic screening have been uncertain. In our

Conclusions Correction of Blount’s disease with osteotomy of the tibia and fibula as well as dynamic fixation with the MAC system achieved
deformity correction as measured by radiographic indices with minimal intra- and post-operative complications. The ease of
application and adjustment of the MAC system makes it an attractive option for Blount’s deformity correction.

Conclusions With the exception of type II fractures, the philosophy of treatment of open fractures of the tibia has not significantly
changed over the past three decades. Closed treatment or internal fixation are both viable options for type II fractures based
on their relatively low incidence of infection. This study also demonstrates a strong relationship between Gustillo sub-types
and odds of infection in this population. Not surprisingly, union rates are also delayed with increasing injury severity.

Conclusion Talonavicular joint arthrodesis is a reliable technique that provides both functionally and cosmetically good results with
a low rate of complications in the treatment of severe pes planus valgus in older children and adolescents with CP. Careful
examination should rule out concomitant ankle valgus deformities. A stable fixation of the arthrodesis is recommended.
