Smoking Cessation Among Women with and at Risk for HIV: Are They Quitting?

CONCLUSIONS HIV-infected and at-risk women in this cohort have lower smoking cessation rates than the general population. Given the high
prevalence of smoking, the high risk of adverse health events from smoking, and low rates of cessation, it is imperative that
we increase efforts and overcome barriers to help these women quit smoking.

We report the case of a 47-year-old
man with recurrent episodes of intractable nausea and vomiting for one year. His symptoms persisted for 4–7 days and then
resolved spontaneously, only to return after periods of time ranging from one week up to a month. After an extensive workup,
which failed to determine any causative explanation for his symptoms, he was diagnosed with cyclic vomiting syndrome. His
episodes of vomiting were successfully terminated with the use of intranasal sumatriptan. In this case, we highlight that
sumatriptan effectively aborted symptoms in an adult patient with cyclic vomiting syndrome....

Abstract An African American male to female transgender patient treated with estrogen detected a breast lump that was confirmed by

with spinal cord compression. We used ethnographic interviews and observations to elicit the patient’s conceptions of her
illness and actions. The patient identified herself as biologically male and socially female; she thought that the former
protected her against breast cancer; she had fears that excision would make a breast tumor spread; and she beli...

CONCLUSIONS Although the prevalence of smoking has declined among US adults, there are opportunities to further reduce smoking among US
veterans, particularly young veterans for whom the prevalence of smoking is similar to that of the US adult population during
the late 1960s/early 1970s. Continued work is necessary to target the high smoking prevalence among veterans with CHD, a group
for which smoking cessation is especially important.

Abstract Although the idea of preventing type 2 diabetes has been articulated since the discovery of insulin, only in the past decade
have clinical trials demonstrated that diabetes can be prevented or delayed. These trials found lifestyle intervention reduces
diabetes incidence by over 50% and is more efficacious than metformin. Evidence from prevention trials comes from persons
with “pre-diabetes” in which blood glucose levels are elevated but not yet in the diabetes range. In normoglycemic persons,
lifestyle or drug intervention has little impact on diabetes incidence. Prevention programs are often co...

Abstract The academic model of medical education in the United States is facing substantial challenges. Apprenticeship experiences
with clinical faculty are increasingly important in most medical schools and residency programs. This trend threatens to separate
clinical education from the scientific foundations of medical practice. Paradoxically, this devaluation of biomedical science
is occurring as the ability to use new discoveries to rationalize clinical decision making is rapidly expanding. Understanding
the scientific foundations of medical practice and the ability to apply them in the care of patients ...

CONCLUSIONS In its first year, Part D coverage appears to have moderated prescription spending and cost-related burden for those who previously
had meager benefits or none. Increased spending among those with employer-based coverage may reflect a narrowing of those
benefits over this period. Evidence of foregone care among low-income, chronically ill seniors who still lack prescription
coverage highlights the importance of targeted outreach to this group for Part D’s low-income subsidy program.

intrinsic characteristics.

Breast, Colorectal and Prostate Cancer Screening for Cancer Survivors and Non-Cancer Patients in Community Practices

CONCLUSIONS Cancer survivors were more likely to self-report receipt of cancer screening than noncancer patients. Medical record reports
of cancer screening were lower than self-reports for cancer survivors and noncancer patients. Identifying factors that affect
cancer screening among cancer survivors is important and has implications for intervention design.

General Internist Communication about Sexual Function with Cancer Survivors

CONCLUSIONS Cancer survivors receive their long-term care from internists. Our results suggest that sexual dysfunction is often not addressed
during their follow-up care. Additional training is needed to prepare physicians to negotiate this difficult issue.

Self-identity After Cancer: “Survivor”, “Victim”, “Patient”, and “Person with Cancer”

Conclusions Survivor identity appears most common and most associated with active involvement and better psychological well-being, but
other identifications are also common and simultaneously held. Adoption of specific cancer identities is likely to impact
interactions with health care providers, including those in general internal medicine, and health behavior changes.

Transitioning to Breast Cancer Survivorship: Perspectives of Patients, Cancer Specialists, and Primary Care Providers

CONCLUSIONS Breast cancer patients may experience difficulties transitioning to survivorship, including ongoing psychosocial issues. African-American
patients may face additional and unique barriers to successful survivorship. Oncology specialists may have concerns about
discharging cherished patients. These findings suggest a psychological component that may influence the use of written survivorship
care plans. PCPs may need additional training and greater access to specialists in order to care for survivors.

Cancer-Related Fatigue: The Approach and Treatment

Abstract As the volume of cancer survivors continues to increase, clinicians are being faced with a growing number of patients with
cancer-related fatigue (CRF). Survivors with a variety of malignancies may experience fatigue. Many potential barriers to
the identification of this symptom in a cancer survivor may exist, due in part to both the patient and the clinician. Assessment
of patients for fatigue is important because it can profoundly effect their daily lives. Many factors contribute to CRF. Hence,
the clinician may face a daunting challenge in attempting to alleviate CRF. Treatment modalities for CRF...

Cancer Survivorship Care: Exploring the Role of the General Internist

Abstract According to the National Cancer Institute (NCI), cancer survivorship encompasses the “physical, psychosocial, and economic
issues of cancer from diagnosis until the end of life.” Today, one in 30 Americans are cancer survivors. Almost two-thirds
have at least one chronic health condition. As the numbers of cancer survivors increase, cancer itself can be viewed as a
chronic medical condition. This paper illustrates some of the challenges faced by cancer survivors. We discuss the limitations
of current models of survivorship care, including shared care. In addition, we explore how the American Bo...

CONCLUSIONS In situations where patients experience significant negative side effects from AI therapy, discussions to discontinue treatment
(and switch to an alternative endocrine therapy) should involve the cancer specialist and take into consideration the patient’s
risk for breast cancer recurrence and the impact of therapy on their quality of life. In some cases, patients may choose to
never initiate AI treatment. In other cases, patients may choose to prematurely discontinue therapy even if therapy is well
tolerated. In both settings increased knowledge by the general internists will likely facilitate ...

Conclusion Research exploring the role of genetic susceptibility in the development of therapy-related adverse outcomes is still in its
infancy. There is a need for continued efforts to study these outcomes in the context of complex gene-gene and gene-environment
interactions unique to cancer survivors. A better understanding of the pathogenesis of these outcomes will help develop effective
targeted prevention strategies.

and use of cessation treatments among cancer survivors.

ABSTRACT Women who have survived cancer may need guidance in choosing a method of contraception. This paper reviews the evidence supporting
the safety and efficacy of available methods of contraception for cancer survivors and concludes that the Copper T380A intrauterine
device (IUD), a highly effective, reversible, long-acting, hormone-free method should be considered a first-line contraceptive
option for women with a history of a hormonally mediated cancer. However, the levonorgestrel-containing IUD may be preferable
for women being treated with tamoxifen and women who have survived non-hormonally mediated...
