Authors: One underutilized method for reducing health disparities and training culturally competent health care workers is the engagement of undergraduate student health organizations in conducting health screenings, promotion, and health education outreach activities in in underserved racial/ethnic communities. We conducted a needs assessment of 14 predominantly racial/ethnic minority undergraduate student-run health organizations. The 14 organizations annually served approximately 12,425 people (67% Hispanic, 25% African American, 6.33% Asian Pacific Islander), predominantly at health fairs within Los Angeles County...

This study measured implicit and explicit attitudes about race using the Race Attitude Implicit Association Test (IAT) for a large sample of test takers (N=404,277), including a sub-sample of medical doctors (MDs) (n=2,535). Medical doctors, like the entire sample, showed an implicit preference for White Americans relative to Black Americans. We examined these effects among White, African American, Hispanic, and Asian MDs and by physician gender. Strength of implicit bias exceeded self-report among all test takers except African American MDs. African American MDs, on average, did not show an implicit preference for either ...

Authors: Comparative quality information (CQInfo) might help ameliorate racial disparities in health care. However, barriers such as low literacy and non-representative materials may make African Americans less likely than Whites to use CQInfo, thus diminishing its effect. Therefore, this analysis sought to evaluate racial differences in awareness and use of CQInfo using a national, random digit dial telephone survey. Results show that African Americans were less likely than Whites to have seen CQInfo concerning hospitals and doctors (not health plans), but were not less likely to have used it. Conditional on awarenes...

Authors: Over the past two decades, Canada has witnessed a proliferation of community-based initiatives providing charitable meals to homeless and under-housed individuals. The existing research has raised concerns about the ability of such initiatives to meet users' nutrient needs. As part of a study of Toronto meal programs, open-ended interviews with program coordinators and observations of 16 meal programs were conducted to provide insight into the nutritional vulnerability of program users. Analysis using ethnographic methods revealed that, although charitable meal programs began in response to concerns about unm...

Authors: This paper examines the importance of collecting and reporting data on race and ethnicity in public health and biomedical research in Canada. Literature and available statistics related to social determinants of health were reviewed and analyzed to illustrate that minority populations in Canada, especially Blacks, are likely to experience poorer health outcomes. Statistics Canada in its commitment to multiculturalism uses broad categories such as visible minorities and racialised groups as surrogates for race and ethnicity. These categories, when used in health literature may conceal underlying inequities in ...

Authors: The under-identification of depressive symptoms among low-income, minority women contributes to disparities in mental health outcomes. Pediatric visits offer a new venue for the identification of such symptoms. We explored women's views related to depression screening during pediatric well-child visits in interviews conducted with 42 mothers of diverse ethnicities. Women considered their child's pediatric provider to be an appropriate person with whom to discuss their emotional health and were aware of the inter-relationship between their mood and their child's well-being. We describe the annual cost and reimbursement associated with all services delivered to 483 program enrollees at one health care system during 2001-03. The average per-enrollee cost for all clinical services provided was $1,056. However, average revenue was $533 per enrollee ($138 from NBCCEDP and $395 from other sources); the cost of uncompensated care thus averaged $523 per year per enrollee (approximately 50% of total cost). The net cost to the health system was nearly four dollars for each dollar of NBCCEDP revenue. The public health benefit of the NBCCEDP may extend beyond breast and cervical cancer but at a cost of i...

This paper examines the hospital characteristics that are associated with patients choosing rural hospitals, and sheds light on the types of patients who depend on rural hospitals for care and, hence, may be the most harmed by the closure of rural hospitals. Using data from California hospitals, the paper shows that patients were more likely to choose nearby hospitals, larger hospital...

Why do some health centers provide more enabling services than others?email this articleEmail this article to a colleague. save this article to My ClippingsSave this article to My Clippings. discuss this articleDiscuss or comment on this article.
Enabling services (such as outreach, transportation, case management, and discharge planning) play a critical role in improving care for vulnerable populations. However, these services are generally not covered by third party payers, making them a challenge for safety net providers that are themselves often financially strained. The study reported here identified organizational and patient population characteristics associated with enabling services provided by community health centers funded by the Health Resources and Services Administration (HRSA). Lagged regressions on 2003-2004 data from HRSA's Uniform Data System...

Point-of-sale marketing of tobacco products: taking advantage of the socially disadvantaged?email this articleEmail this article to a colleague. save this article to My ClippingsSave this article to My Clippings. discuss this articleDiscuss or comment on this article.
With increasing regulation of tobacco industry marketing practices, point-of-sale advertising has become an important channel for promoting tobacco products. One hundred and ten convenience stores in Oklahoma County were surveyed for tobacco-related advertising. There were significantly more point-of-sale tobacco advertisements in low-income and minority neighborhoods than in better educated, higher-income, predominantly White neighborhoods. Storeowners or managers were also interviewed to determine who has decision-making power regarding store signage and placement, and to elicit perceptions of industry tactics. Contr...

Are there patient disparities when electronic health records are adopted?email this articleEmail this article to a colleague. save this article to My ClippingsSave this article to My Clippings. discuss this articleDiscuss or comment on this article.
Using nationally representative samples of visits from the 2005-2006 National Ambulatory Medical Care Surveys and the National Hospital Ambulatory Medical Care Surveys (N=39,343), this study examines whether electronic health record (EHR) systems have been adopted by primary care physicians or providers (PCPs) for poor minority patients at the same rate as by the PCPs for wealthier non-minority patients. Although we found that electronic health record adoption rates varied primarily by type of practice of the PCP, we also found that uninsured Black and Hispanic or Latino patients, as well as Hispanic or Latino Medicaid...

Preventive dental service utilization for Medicaid-enrolled children in New Hampshire: a comparison of care provided by pediatric dentists and general dentists.email this articleEmail this article to a colleague. save this article to My ClippingsSave this article to My Clippings. discuss this articleDiscuss or comment on this article.
In this study, we compared preventive dental utilization through visits to a pediatric dentist (PD) vs. visits to a general dentist (GD) among Medicaid-enrolled children in New Hampshire (n = 12,964). Dental claims were analyzed using conditional logistic regression models. After adjusting for covariates, children seen by a PD were 51% more likely to have received fluoride treatment, 26% more likely to have had at least two dental examinations, and 19% more likely to have received a sealant than children seen by a GD. Overall, our results suggest that children seen by a PD were more likely to have received preventive servi...

Inpatient hospital service utilization of the uninsured in Nebraska: policy implications for addressing uncompensated care.email this articleEmail this article to a colleague. save this article to My ClippingsSave this article to My Clippings. discuss this articleDiscuss or comment on this article.
In order better to inform policymakers about financing uncompensated hospital care through appropriate allocation of resources among Nebraska communities, this study used seven years (1996-2002) of county-level data from multiple sources to examine the relationship between population economic factors and the hospital inpatient care use by uninsured patients. The generalized estimating equation (GEE) regression analysis showed that, at the county level, the population uninsurance rate and other economic factors (e.g., per capita income, the percentage of population receiving welfare) are statistically significant predic...
