Novel Operation of Snapping Biceps Femoris Tendon
This study reports a case of nontraumatic painful snapping of the BFT due to a prominent fibular head that was persistent intraoperatively after fibular head resection. We describe a novel surgical technique of release of the anterior arm of the biceps tendon followed by soft tissue tenodesis to the popliteofibular ligament, as opposed to the remaining fibular head, to prevent iatrogenic fracture. “Baker's Cyst”–Induced Above-Knee Amputation
We report a 65-year-old man who presented with a necrotic fifth toe, incipient phlegmon and hypesthesia of the right foot, a swollen lower leg, and a palpable popliteal mass. An occlusion of the popliteal artery secondary to a Baker's cyst was found to have caused protracted ischemia and the abovementioned symptoms. Despite several endovascular and open-surgery procedures to restore perfusion of the limb, the patient eventually had to undergo above-knee amputation. It might be speculated whether earlier surgery would have preserved the patient's limb. Whereas the traditional procedure of open resection of the Baker's cyst h...

Periosteal Sleeve Avulsion Fracture of the Quadriceps Tendon
We present the case of a 15-year-old girl who sustained a patellar sleeve avulsion of the quadriceps tendon 1 month following patellar dislocation without surgical treatment. Open repair of quadriceps tendon was performed with subsequent union. Arthrofibrosis of the knee was noted; two subsequent manipulations and adhesiolysis were performed. At 18-month postinjury, normal motion and strength were present. Special attention to postoperative recovery of range of motion is imperative for good clinical outcome after periosteal sleeve avulsion fractures of the patella. Medial Patellofemoral Ligament Reconstruction Following Total Knee Arthroplasty: A Case Report
J Knee Surg Rep DOI: 10.1055/s-0036-1587597Patella instability is a rare but serious complication seen following total knee arthroplasty. The typical cause of patella instability in this setting is malpositioned components. While the mainstay of treatment is recognizing and correcting any malpositioning of the components, continued instability can remain and be a difficult problem to treat. Medial patellofemoral ligament (MPFL) reconstruction can be a helpful adjunct for the surgeon when faced with cases of continued instability. In this case report, we describe the successful use of MPFL reconstruction in conjunction with...

Polyethylene Tibial Post Fracture in a Posterior Stabilized (Genesis II) Total Knee Arthroplasty
This study reports the case of a fractured polyethylene tibial post in a 49-year-old white male patient at 4 years status post a left posterior stabilized total knee arthroplasty. The patient was doing extremely well postoperatively until he felt pain in the left knee while completing personal resistance squats. He was found to have anterior/posterior instability in flexion as well as midflexion instability. The patient underwent removal of the fractured polyethylene insert and replacement with a thicker insert. The patient had immediate resolution of his preoperative pain and instability. To our knowledge, this is the fir...

A 17-Year Follow-Up after Total Tibial Replacement in the Course of an Osteosarcoma followed by Total Leg Replacement
We report a case of an initially 16-year-old female patient, diagnosed with an osteosarcoma of her left proximal tibia. Treatment consisted of tumor resection in combination with the implantation of a customized tumor prosthesis and chemotherapy. The patient developed a deep infection and several additional surgeries were inevitable. Because of increasing bone loss the distal tibia anchoring was insufficient and a custom-made total tibial prosthesis was designed and implanted to brace against the talus. After 17 years the femoral component became loose and the upper part of the prosthesis had to be exchanged with a total f...

Anterior Closing Wedge Tibial Osteotomy for Failed Anterior Cruciate Ligament Reconstruction
We present a case report outlining the surgical technique for correction of sagittal plane imbalance by a slope-decreasing anterior closing wedge tibial osteotomy combined with a second-stage revision ACL reconstruction. At 2 years following surgery, our patient had a stable knee and returned to low-impact athletic activity without symptoms. Further investigation is needed to refine the indications for this procedure in the setting of failed ACL reconstruction.[...]Thieme Medical Publishers 333 Seventh Avenue, New York, NY 10001, USA.Article in Thieme eJournals:Table of contents  |  Abstract  |  open access Full t...

Medial Collateral Ligament Release Facilitates Exposure in Revision Total Knee Arthroplasty
J Knee Surg RepDOI: 10.1055/s-0035-1551546Incidence of revision total knee arthroplasty is growing. Revision surgery aims at preserving the collateral ligaments to minimize implant constraint and stress on the implant-cement-bone interface. Exposure in the stiff knee can be challenging as a result of loss of soft tissue elasticity and capsular scarring brought on by multiple surgeries. If a quadriceps snip does not provide adequate visualization a quadriceps turndown or a tibial tubercle osteotomy is recommended to improve exposure. Such extensile approaches have various limitations and might risk compromising the extensor...

Sonographic Diagnosis of an Acute Lateral Meniscus Tear in a Division I Collegiate American Football Player
We present a case of an acute tear of the lateral meniscus in a collegiate athlete that was diagnosed in the training room immediately after injury using ultrasonography. The diagnosis was subsequently verified by MRI and arthroscopy. The patient was treated with partial meniscectomy and experienced an uneventful recovery with return to sport 3 weeks after injury. This case report supports the potential capabilities of ultrasonography as an effective and efficient point-of-injury diagnostic tool for athletes with suspected meniscal pathology.[...]Thieme Medical Publishers 333 Seventh Avenue, New York, NY 10001, USA.Article...

Anterior Cruciate Ligament Regeneration in an 8-Year-Old Patient
We report the case of an 8-year-old, skeletally immature patient with a midsubstance-ruptured ACL at physical exam and magnetic resonance imaging (MRI). Conservative treatment with a brace led to a stable knee with regeneration of the ACL on both the physical examination and MRI after 5 years. Nevertheless, further research will be necessary to gain better understanding about the biology and healing potential of the ACL.[...]Thieme Medical Publishers 333 Seventh Avenue, New York, NY 10001, USA.Article in Thieme eJournals:Table of contents  |  Abstract  |  open access Full text (Source: Journal of Knee Surgery Reports)

Acute Anterior Cruciate Ligament Injury with Medial and Lateral Bucket-Handle Meniscus Tears
We report the case of a healthy 31-year-old female professional billiard player presented with a 5-day history of severe left knee pain after a fall. A magnetic resonance imaging of the left knee showed that she had suffered an anterior cruciate ligament (ACL) rupture along with buckle-handle tears of both the medial and lateral meniscus. Both of these menisci had flipped anterior and centrally to the femoral condyles and were lodged in the notch. The patient had also suffered a mild injury to the medial collateral ligament. Repair of both menisci was performed using an inside-out technique. Following this, an ACL reconstr...
