Changes in the Vaginal Microbiota of Women With Secondary Localized Provoked Vulvodynia
Conclusions Localized provoked vulvodynia and LOD were associated with shifts in the vaginal microbiota. However, the impact of these changes on the development of LPV requires additional studies with a larger cohort. (Source: Journal of Lower Genital Tract Disease)

A Scoping Review of Treatment Outcome Measures for Vulvar Intraepithelial Neoplasia
Conclusions There is a large variation in outcome measures, instruments, and scales used for any clinician-reported treatment outcome such as “clinical response.” Most studies do not include patient-reported outcome measures assessing quality of life domains. A Core Outcome Set for the treatment of VIN is needed to improve the quality of VIN research. (Source: Journal of Lower Genital Tract Disease)

Prevaccine Human Papillomavirus Status in Invasive and Intraepithelial Lesions of the Vulva in New Zealand Women
Conclusions Overall, the HPV vaccine is likely to substantially alter the profile of SCCV in our region. The results provide a baseline assessment of the HPV status of vulvar neoplasia before the introduction of the HPV vaccine. (Source: Journal of Lower Genital Tract Disease)

Comorbid Vulvar Lichen Sclerosus and High-Grade Squamous Intraepithelial Lesions: A Management Conundrum
Conclusions High-potency TCS use for the treatment of LS did not seem to increase the risk of HSIL recurrence in patients with comorbid vulvar LS and HSIL. This suggests that high-potency TCS can be appropriately used for the treatment of LS even when HPV-associated disease is present. (Source: Journal of Lower Genital Tract Disease)

Recurrent Dysplasia After a Loop Electrosurgical Excision Procedure: Local Versus General Anesthesia
Conclusions Anesthesia mode does not seem to affect the rate of positive LEEP margins and the need for repeat conization. Our study findings suggest that LA should be preferred instead of GA in LEEP. (Source: Journal of Lower Genital Tract Disease)

Endocervical Margins Status in Excision for Preventing Cervical Cancer According to the Transformation Zone Type
Conclusions The endocervical margin was negative in 3 in every 4 women who underwent loop electrocautery excision procedure, regardless of excision type. Age, menopausal status, smoking, and hormonal contraception did not predict margin status. (Source: Journal of Lower Genital Tract Disease)

Examining 5-Year Cervical Cytology Progression Among Minority Women Living With HIV and Baseline Negative Cytology
This study aims to determine the rate of cervical cytologic progression and related factors in minority WLWH across 5 years. Materials and Methods We used our HIV clinic database, complemented with a retrospective chart review to identify WLWH with a baseline negative cervical cytology between 2009 and 2012 and 5-year follow-up. Data included race/ethnicity, age, years living with HIV, AIDS status, viral load, history of smoking, drug use, and HPV status. Multivariate logistic regression tested progression of negative cytology to low-grade/high-grade squamous intraepithelial lesions (LGSIL/HGSIL). Results Amo...

Diagnostic Utility of Endocervical Curettage During Colposcopy Among Patients of Varying Risk Factors
Objective Endocervical curettage (ECC) during colposcopy is recommended in certain circumstances; however, diagnostic use remains unclear. We evaluate the utility of ECC among patients with non-fully visualized squamocolumnar junction (SCJ) and certain patient socioeconomic factors. Methods Retrospective chart analysis was completed for patients aged older than 21 years who underwent a colposcopy at 2 study sites between 2012 and 2021. Demographics and histopathologic results were analyzed. Results A total of 1,516 colposcopies were reviewed; 73.8% (n = 1,119) had an ECC with colposcopy. Of those, 92.1% (...

Conservative Treatment for Cervical Adenocarcinoma In Situ: Long-Term Results
Conclusions Women with cervical AIS can be managed conservatively by an excisional procedure, provided that the margins are free and a close and long-term follow-up is guaranteed. (Source: Journal of Lower Genital Tract Disease)

HPV Test as Test of Cure After Conization for CIN2+: A Nationwide Register-Based Cohort Study
Conclusions Using the HPV test at the first post-treatment control as TOC for cervical intraepithelial neoplasia grade 2 or worse after stratifying for resection margins in cone resections yields an equally high sensitivity and negative predictive value as cotesting with cytology. We suggest that women with free resection margins return to the routine screening program after negative HPV test result as TOC at 6 months. (Source: Journal of Lower Genital Tract Disease)

Differentiated Exophytic Vulvar Intraepithelial Lesions: Case Reports and Review of Literature
Conclusions Differentiated vulvar intraepithelial lesions and invasive squamous cell carcinoma should be considered in the differential diagnosis of vulvar itch associated with hypertrophic plaques in postmenopausal women. Excision of suspicious plaques is recommended for definitive diagnosis. (Source: Journal of Lower Genital Tract Disease)

Presenting Symptoms and Diagnosis of Vulvar Lichen Sclerosus in Premenopausal Women: A Cross-Sectional Study
Conclusions Vulvar LS affects premenopausal women, commonly presenting with dyspareunia and tearing with intercourse. This condition should be considered and evaluated in premenopausal women presenting with vulvar symptoms and sexual pain. (Source: Journal of Lower Genital Tract Disease)

The Clinicopathologic Challenge of Nonneoplastic Vulvar Acanthosis
Conclusions There is a group of acanthotic tissue reactions that cannot be classified with standard histopathologic assessment. Further clinicopathologic research into unilateral acanthotic lesions may provide insight into separation of psoriasis and mycosis when organisms are absent. Once nuclear atypia is excluded, immunohistochemistry for p16 and p53 and HPV molecular testing do not assist in diagnostic identification. (Source: Journal of Lower Genital Tract Disease)
