Pseudoaneurysm of the aortic arch is an unusual complication of arch vessel bypass and poses a complex surgical challenge. Operative management may involve open thoracotomy, cardiopulmonary bypass, and aortic arch repair, all of which carry significant morbidity and mortality. Conclusions: The results of the study, with clear demarcation of the ablation zones and transition zones in the normal breast tissue, support the potential of breast thermal ablation as a viable treatment for further study. Lactating tissue does not seem ideal for thermal ablation. The discrepancy of the extent of ablation and the length of the electrodes is an important finding in this study. Conclusions: Use of a single-incision technique for chest port implantation in adult and pediatric oncology patients is feasible. This may be the preferred method of subcutaneous port placement, as it has a very low complication rate and a high success rate. Endovascular retrieval of intracardiac or intrapulmonary inferior vena cava (IVC) filters provides a less-invasive alternative to surgery; however, only a few publications provide details describing this approach. The authors searched the medical literature and identified 42 endovascular attempts in 35 publications. I am writing a response to the Letter to the Editor from the March 2009 issue of JVIR () regarding fatal nontarget embolization during uterine artery embolization (UAE). I commend the authors for submitting this case. It illustrates the need for more uniformity in the approach used for UAE as well as more understanding of the type and amounts of embolic material used in the procedure. The standard treatment for rectal varices has not been established, as serious rectal varices causing massive hemorrhage are rare. Previously reported treatments for the varices include endoscopic sclerotherapy, transjuglar intravenous portosystemic shunt (TIPS) placement to decrease the portal venous pressure (), and embolization with a transhepatic () or TIPS approach (). Isolated aneurysms of the profunda femoris artery (PFA) are rare and account for only 0.5% of all peripheral aneurysms and 1%–2.6% of all femoral aneurysms. However, PFA aneuryms have a higher rate of rupture than other peripheral aneurysms and are associated with significant morbidity (). PFA aneurysms have an asymptomatic course and are not easily detected unless they are complicated with rupture or critical limb ischemia; indeed, approximately 58% of patients present with a complicated PFA aneurysm (). Endovascular Repair of an Arteriovenous Fistula after Intervertebral Disk Surgery: Case Report

Vascular injuries that complicate intervertebral disk surgery are rare. Despite being well-described, their clinical presentation is often overlooked. The authors report the delayed diagnosis of an arteriovenous fistula following disk surgery that led to advanced congestive failure and pulmonary hypertension mistakenly attributed to hepatic failure. Because endovascular repair offers complete resolution, accurate diagnosis is essential. Congenital vascular anomalies of pelvis and lower limb arteries are rare. During embryologic development, the sciatic artery represents the dominant supplier of blood to the lower limb. The external iliac and femoral arteries appear later in the process and take over as the sciatic artery regresses.
