This descriptive, cross-sectional study explored the factors associated with frequency and intensity of complementary/alternative medicine (CAM) use in 301 HIV-infected men from southern California (n=75) and northern Florida/southern Georgia (n=226). Logistic regression analysis was conducted to identify which demographic, biomedical, psychosocial, and health behavior variables (risk and health-promoting behaviors) were predictors of CAM use and intensity of use. The majority (69%) of participants reported CAM use. The types of CAM most frequently cited were dietary supplements (71%) and spiritual therapies (66%). Odds of...

There is an increasing need for community-based interventions to help women living with HIV better manage their symptoms and self-care. The investigators conducted a small descriptive pilot study to assess whether women living with HIV needed and wanted a community-based symptom management workshop, such as the Positive Self-Management Program (PSMP). A total of 7 HIV-infected adult women participated in five, 2-hour semi-structured focus groups and completed a brief survey on demographic information, HIV medications, HIV symptoms, and self-efficacy. Quantitative and qualitative data are presented. This pilot study suggest...

This pilot study (a) examined the stages of change (SOC) for condom use with primary and casual partners among rural heterosexual African American stimulant users, (b) identified gender differences in variables associated with SOC, and (c) assessed the association of SOC with decisional balance and self-efficacy. A total of 72 participants completed the study. SOC with a primary partner were much lower than with a casual partner, indicating more consistent condom use with casual partners. Significant gender differences existed in decisional balance for condom use for both primary (p = .02) and casual partners (p =.03), wit...

I am a nurse practitioner who cares for HIV-infected adults at an urban hospital-based infectious diseases clinic. Both were young mothers with daughters under the age of 2, both were married, both lived in the same suburban area, both had received prenatal care throughout their entire pregnancies, both had delivered their babies at large tertiary care hospitals, both had limited English pr...

It is not necessary to change. Survival is not mandatory.—W. Edwards Deming Change is not a word that inspires a warm, fuzzy feeling. Personally, I have always approached it as something to be borne, bravely, but with a lot of complaining. Hearing the quote above in a presentation at the recent International Academy of Nursing Editors conference, however, made me rethink my position. Change may be anxiety producing, difficult, and time consuming, but it is also thought provoking, stimulating, and a path to the future. Change indicates that we are living. For many HIV health care providers and health professions students, there is often a fundamental knowledge deficit and misunderstanding of who transgender people are and what their health care needs may be. The word transgender refers to individuals with gender identity that differs from their biological (or anatomic) sex (). Biological sex is most commonly assigned at birth based upon visualization of the external genitalia. In contrast, gender is personally and culturally constructed based upon an individual's internal sense of being feminine or masculine (). (Source: Journal of the Association of Nurses in AIDS Care)

Preliminary evidence has suggested that some transgender men who have sex with nontransgender men (“trans MSM”) may be at risk for HIV and sexually transmitted infections and that their prevention needs are not being met. Quantitative (n =45) and qualitative (n =15) interviews explored risk behaviors, protective strategies, and perceptions of the impact of transgender identity on sexual decision making among trans MSM. A majority of the participants reported inconsistent condom use during receptive vaginal and anal sex with nontrans male partners; HIV prevalence was 2.2%. For people who are transgender, transsexual, or transitioned (trans), access to primary, emergency, and transition-related health care is often problematic. Results from Phase I of the Trans PULSE Project, a community-based research project in Ontario, Canada, are presented. Based on qualitative data from focus groups with 85 trans community members, a theoretical framework describing how erasure functions to impact experiences interacting with the health care system was developed. Two key sites of erasure were identified: informational erasure and institutional erasure. How these processes work in a mutually reinforcing m...

In HIV-infected patients undergoing antiretroviral therapy, the question of whether selenium supplementation has any therapeutic benefit is still open. With recent popular coverage of this issue, many patients have considered using selenium. Clinicians have a duty to ensure that the recommendations they make to their patients are evidence based. The literature search reported here showed that evidence to support standard selenium supplementation in patients with HIV is both limited and insufficient. To definitively answer this clinical question, the overall effect of selenium supplementation would need to be evaluated in ...

In this original edition of HIV/AIDS: Global Frontiers in Prevention/Intervention, Cynthia Pope, Renée T. White, and Robert Malow provide an impressive examination of HIV/AIDS from a historical, social, political, and biomedical perspective while transcending issues of culture, gender, and global impact.
