First phase class II early treatments are able to restore a correct growth. DAC (Distal Active Concept) is an early fixed treatment using class II elastics, coils and springs. The aim of this retrospective study was to evaluate short and long term dental and skeletal effects of head-gear applied on upper first molars associated to class II elastics and to compare our results to a sample treated with early DAC. Headgear group included 37 subjects. DAC group included 100 subjects. Cephalometric values came from Pancherz's and Tweed's analysis. Lateral head-films were analysed at t(1) (before early active treatment), at t...

As practicing professionals it is essential that orthodontists should think carefully about their status of care givers and their relationships with their patients. In doing this, they should analyze the dynamics of the impact of their therapeutic team on patients throughout the entire treatment procedure. It is equally important that they understand the decisive importance of the way patients are welcomed into the office ambiance and the way staff members communicate with them and take into consideration their needs and expectations. Today specialists in dento-facial orthopedics and their staff members are usually wel...

During human morphogenesis, intrauterine liquid movements, whose amniotic fluid is at the first place, play an essential part with some unexpected consequences on the vestibular apparatus development and function. Underlying the ubiquitous container-content relation, they give a new and effective approach of the morphogenetic process.

The goal of orthodontic treatment is to improve the patient's life by enhancing dental and jaw functions and dentofacial esthetics [ Graber TM, et al., Orthodontics current principles and techniques. 4(e) ed. St Louis: Elsevier, 2005.] . Harmonious occlusion is achieved following improvements of malocclusion via orthodontic treatment [Ehmer U and Broll P, Int J Adult Orthod Orthognath Surg 1992;7:153-159. Throckmorton GS, et al., J Prosthet Dent 1984;51:252-261.]. Perfect facial symmetry is extremely rare, and normal faces have a degree of asymmetry. Patients with dentofacial deformity more frequently have asymmetry of...

In conclusion, hyperactive children present more challenges during an orthodontic treatment compared to a control population. Nevertheless, the treatment is easier when children are diagnosed and treated multimodally. Further studies are required on a wider population to confirm the differences in therapeutic difficulties.

Mastery of the bonding stage of treatment is clearly a critical step in the clinical practice of orthodontics. There is such a wide variety of products available in orthodontics that a reasoned assessment of bonding systems is a practical necessity. Composite plastics, associated with hydrophobic or hydro-compatible adhesive systems, and the CVIMAR represent the two principal types of bonding agents used in dentistry. They are categorized according to their constituents into a wide range of products whose nuanced differences are sometimes difficult to discern. This paper first focuses on the development of the composit...

Conclusions: Both the acid-etched control and self-etching primer in combination with adhesive pre-coated brackets were successful for clinical bonding. Their combined failure rate was lower than that reported in similar trials. The literature on SEP's supports the findings of this study.

Enamel conditioning (elimination of dental plaque and creation of an irregular surface) is an essential step before bonding of orthodontic brackets. The most popular procedure in our practice is bonding with resin which requires enamel etching in order to get enough shear bond strength. Many studies have tried to evaluate the effects of enamel bonding using the acid-etching procedure as well as the changes caused by detachment of brackets. Thanks to the development of other adhesives such as glass ionomer cements which chemically bind to the enamel, new enamel conditioning methods appeared, in particular sandblasting w...

Bonding is a major component of our therapeutic arsenal. We routinely bond attachments to enamel as well as to other surfaces including dentin, ceramic, acrylic, steel, amalgam, and gold, especially for adult patients. This procedure poses certain technical problems for which adapted and specific technical protocols can provide solutions.

The author explains in this clinical note how the direct bonding method can be most efficiently utilized. He suggests that precision of placement and comfort for patients can be improved if the following two rules are respected: the attachments must be kept horizontal and the substrate must remain immobile.

The bonding procedures used in the Incognito((R)) system are now well known. Practitioners can work with either a protocol for Maximum-cure which utilizes a hydrophobic and chemo-polymerizable adhesive or an adhesive that is dual and hydrophilic, the association Nexus 3/Excite. Clinically the two protocols provide equivalent results in the quality of the bonding seal for attachments. They are simple to use, but in applying them practitioners must scrupulously follow the manufacturer's recommended stages of procedure.

The indirect bonding technique is pivotal for success in lingual orthodontics. There are different laboratory techniques available for indirect positioning and bonding of lingual brackets. Different approaches are presented and described to perform a clinical application.

The manufacturers of self-ligating brackets advertise a reduction of the friction engendered between the wire and the bracket, which is an essential parameter for treatment's speed and comfort. We have compared the friction obtained with four types of self-ligating brackets - In-Ovation R, Damon 3, Smart Clip and Quick - with that of a standard bracket Omniarch associated with an elastomeric ligature. All bracket were tested on a bench of traction with three types of wires: steel .019"x.025", TMA .019"x.025" and NEO sentalloy F300 .020"x.020". The results confirm a clear friction reduction for all tested wire. Les fabr...

The eight points cephalometric Treil model and the alveolodental parameters already allows an initial description of the maxillofacial framework and the teeth. Added six points allows taking into consideration an intermediate level: the maxillary and mandibular skeletal basis. So, the 14 points complete model allows a complete craniofacial anatomy description in three analysis levels: the framework or enveloppe, the osseous bases, the alveolodental arch. The correlation study between the 3D parameters at different levels defines the compensation notion. It allows understanding how skeletal disharmony may be distributed...

Vertical growth pattern is supposed to have adverse effects on sagittal growth and is considered as a contra-indication for early treatment. The aim of this retrospective cohort study was to analyse whether early treatment of children with Class II malocclusion was able to achieve as an important sagittal correction on subjects with growth vertical pattern as on subjects without vertical pattern without modification of vertical growth. The study was composed of 123 children (110 months old in average), with Class II division 1 malocclusion : 34 children with excessive vertical growth pattern and 74 children without exc...

With help of a completed 17 keys analysis system it is now really possible to position quite easily the teeth in one's face, to really build a harmonious, well balanced, and attractive smile: the so much coveted "star-smile"! And to get a really complete method, the ageing component of the face's soft tissues is also analyzed and used. But one has to know that unfortunately there are some limits. And all our patients will not reach this fantastic wish. There are some special situations where the teeth anatomy, the shape and lip thickness, the size of the mouth, and the ratios of all these elements do not match. Thus, i...

The aim of the study is to assess the dimensions of the masticatory muscles from CT scans using C2000 software and to analyse their variations depending on sagittal and vertical skeletal pattern. The maximal length, the area and the volume of the right and left masticatory muscles were measured from CT scans of 34 subjects selected upon their skeletal pattern. The analysis of the results shows that masticatory muscles dimensions vary depending on sagittal and vertical skeletal pattern. They are more developed in brachyfacial subjects and to a lesser degree in subjects without antero-posterior skeletal discrepancy. This...

The aim of this study is to analyze the differences between three common ways of maxillary expansion. For that purpose, 41 patients between 7 and 10 years old, have been studied in a randomized way with different expansion techniques. They were assigned into one of the three following groups: (1) expansion with a Quad helix((R)) appliance; (2) rapid palatal expansion with an acrylic bonded appliance; (3) rapid palatal expansion after uprighting the upper molars with a Quad helix appliance. The following changes were compared: skeletal expansion, dental expansion, axial inclination of the upper first molars and the infl...

A multifactorial basis is acknowledged for the extensively worn dentition and efforts to eliminate or minimize these underlying factors are important. The management of a case of severe incisal and occlusal wear can adopt different approaches. The author proposes the rationale and advantages for involving orthodontic preparation as part of the occlusal rehabilitation. Facing a dental crowding, the orthodontist has several therapeutic options. If maxillary expansion is often used, mandibular orthodontic expansion is proscribed because of its tendency of inefficiency and relapse. Mandibular symphyseal distraction osteogenesis allows to remove this proscription. This therapeutic protocol consists in a pre-surgery orthodontic preparation phase followed by surgery (symphyseal osteotomy and placement of the distraction device), a latency period (five to seven days), then an activation period (most of the time 1 mm/day in two steps) followed by a consolidation period (three months) during ...

The subtle mixture of a set of physical data, but also psychological and emotional, related to his (her) history, his (her) culture and his (her) environment will allow to lay out the elements of each one in order to achieve an individual specimen which cannot be reproduced.
