Pregnancy outcome after treatment for cervical intraepithelial neoplasia.email this articleEmail this article to a colleague. save this article to My ClippingsSave this article to My Clippings. discuss this articleDiscuss or comment on this article.
CONCLUSION:: Women with CIN3 have higher rates of spontaneous preterm delivery and preterm PROM than do those in the general population. Loop electrosurgical excision procedure did not alter these pregnancy complication rates. Women should be counseled adequately before treatment but should be reassured regarding the treatment of CIN on the risk of preterm delivery. LEVEL OF EVIDENCE:: II. Are uterine risk factors more important than nodal status in predicting survival in endometrial cancer?email this articleEmail this article to a colleague. save this article to My ClippingsSave this article to My Clippings. discuss this articleDiscuss or comment on this article.
CONCLUSION:: High-risk uterine factors including grade 3 tumor, deep myometrial invasion, and cervical stromal involvement are more significant determinants of survival in endometrial cancer than pelvic-node status. Uterine risk factors should be considered, regardless of nodal status, when offering systemic therapy to maximize survival outcomes. LEVEL OF EVIDENCE:: II. Effect of increased body mass index on first-trimester ultrasound examination for aneuploidy risk assessment.email this articleEmail this article to a colleague. save this article to My ClippingsSave this article to My Clippings. discuss this articleDiscuss or comment on this article.
CONCLUSION:: In singleton pregnancies, increased BMI is not associated with suboptimal visualization of nuchal translucency, but it is associated with a longer time to perform the first-trimester ultrasound examination for aneuploidy risk assessment, increased need for transvaginal ultrasound examination for nuchal-translucency visualization, and a lower likelihood of obtaining an adequate nasal-bone image. Previous abdominal surgery did not affect the ability to visualize the nasal bone. LEVEL OF EVIDENCE:: II. PMID: 19888045 [PubMed - in process] (Source: Obstetrics and Gynecology)

Recommendations for activity after obstetric and gynecologic procedures remain based on tradition and anecdote. After reviewing the current evidence base, guidelines, and practice for postdischarge instructions related to physical activity after the most common obstetric and gynecologic surgical procedures, we conclude that the available data do not support many of the recommendations currently provided. Restrictions on lifting and climbing stairs should likely be abandoned. Guidance on driving should focus on the concern regarding cognitive function and analgesics rather than concerns of wound separation/dehiscence. G...

Stillbirth is a major obstetric complication, with 3.2 million stillbirths worldwide and 26,000 stillbirths in the United States every year. The Eunice Kennedy Shriver National Institute of Child Health and Human Development held a workshop from October 22-24, 2007, to review the pathophysiology of conditions underlying stillbirth to define causes of death. The optimal classification system would identify the pathophysiologic entity initiating the chain of events that irreversibly led to death. Because the integrity of the classification is based on available pathologic, clinical, and diagnostic data, experts emphasize...

Community-acquired pneumonia is a common disease worldwide, with considerable morbidity and mortality. In the United States, pneumonia complicates 0.5 to 1.5 per 1,000 pregnancies. Physiologic adaptations in the respiratory and immunologic systems may increase susceptibility to pulmonary infections as well as alter their clinical course. Bacterial, viral and fungal pathogens all cause pneumonia in pregnancy, although the causative agent is identified in only 40-60% of cases. The most common single pathogen is Streptococcus pneumoniae, which is identified in 15-20% of community-acquired pneumonia cases in pregnancy. Rec...
