Treatment of idiopathic scoliosis exceeding 100 degrees - comparison of different surgical techniques.email this articleEmail this article to a colleague. save this article to My ClippingsSave this article to My Clippings. discuss this articleDiscuss or comment on this article.
The treatment of scoliosis exceeding 100 degrees remains a challenge. Anterior fusion only may lead to low correction and screw plowing, posterior fusion only may cause the cranckshaft phenomenon in skeletally immature patients. Two-stage surgery is advocated, comprising anterior release and posterior fusion. <br /> The aim of the paper is to compare treatment outcomes in patients with a >100% primary curve treated between 1984 - 2004 with one of the following techniques: halo-femoral traction with posterior fusion (Group I; n=124 patients), anterior release with halo-femoral traction and posterior fusion (Gro...

Does instrumentation removal cause curve progression in patients with adolescent idiopathic scoliosis?email this articleEmail this article to a colleague. save this article to My ClippingsSave this article to My Clippings. discuss this articleDiscuss or comment on this article.
Adolescent idiopathic scoliosis (AIS) is usually instrumented using a posterior approach. Hardware removal may be performed for specific clinical reasons. Little data is available on whether removal influences curve magnitude. The aim of the paper is to evaluate the impact of instrumentation removal on curve progression, and the safety and efficacy of the procedure. We analyzed 59 patients who underwent instrumentation removal. Curve types, reasons for removal, period between procedures, and Cobb angles: at baseline, immediately after correction, after removal and in follow-up were evaluated. Clinical symptoms were als...

Abstract Bilateral posterior fracture-dislocations of the shoulders are a rare injury that usually occurs secondary to a tonic-clonic seizure, electrocution or a high energy trauma. In this unique case, we present the first case of this injury resulting from a simple syncope in a 49 year old man with no history of epilepsy. He was fully investigated and no cause was found for his syncope. His shoulder fracture dislocations were reduced acutely and treated conservatively with full functional recovery. Osteoporosis is a systemic skeletal disease characterized by low bone mass and architectural derangements leading to an increased fracture risk. Evaluation of the quality of life of patients with osteoporosis concerns the physical, emotional, social and pain QoL domains. Quality of life can be measured to compare the effect of different treatments of osteoporosis. The as-sessment of quality of life is also useful in epidemiological studies to estimate the burden of disease and to evaluate the effectiveness of different treatments. The principal goals of health care for these patients are to maintain independence and pr...

Concomitant ipsilateral shaft and femoral neck or pertrochanteric fractures are uncommon. They result from high-energy traumas and the mechanism of injury is usually complex. The proximal fracture is often not displaced and may be hardly visible on x-rays. Overlooking the proximal fracture may result in therapeutic problems. The best method of treatment is by closed stabilization of both fractures with a reconstruction nail. In our opinion, concomitant neck and femoral shaft fractures demand particular alertness in x-ray analysis, precision in fracture stabilization and usually longer patient rehabilitation. PMID: ...

Strategies for neuroprotection following spinal cord injury.email this articleEmail this article to a colleague. save this article to My ClippingsSave this article to My Clippings. discuss this articleDiscuss or comment on this article.
Progress in neuropathology has made possible the description of local responses of neural tissue in early stages after traumatic spinal cord injury (SCI). The recent identification of multiple factors responsible for secondary spinal cord damage and for potential regenerative abilities has not resulted in the development of a standard for neuroprotective therapy in SCI patients. The paper reviews current knowledge concerning the sequence of biochemical events in the injured spinal cord and gives an overview of therapeutic possibilities for preventing the spread of secondary injury. The literature survey has led to the ...

Functional assessment of knee muscles 13 weeks after anterior cruciate ligament reconstruction - pilot study.email this articleEmail this article to a colleague. save this article to My ClippingsSave this article to My Clippings. discuss this articleDiscuss or comment on this article.
CONCLUSIONS: After ACL reconstruction functional deficiency relates first of all to the quadriceps femoris what is confirmed by the higher H/Q ratio in peak torque compare to the healthy limb. Although low number of subjects participating in the study results of tests demonstrates reliable assessment of knee muscles' function in patients after ACL reconstruction allows calculation of deficit levels of force - velocity parameters and ipso facto possibility to select optimal rehabilitation program for each patient. CONCLUSIONS: 1. The results of the examinations appear to suggest that evaluation of resting muscle tone can be used to provide an objective assessment of the analgesic efficacy of physiotherapy, including neuromobilization. 2. In the treated group, neuromobilization therapy was superior to standard physiotherapy as regards eliminating or decreasing elevated resting muscle tone in response to pain in patients with low back pain. 3. Neuromobilization produced a significant improvement in clinical test results and clinical symptoms in the treated group as opposed to the control group, which demonstrates the efficacy of this ...

Overlooked posterior shoulder dislocation: preoperative and postoperative CT studies (a case report).email this articleEmail this article to a colleague. save this article to My ClippingsSave this article to My Clippings. discuss this articleDiscuss or comment on this article.
Posterior shoulder dislocation is an extremely rare injury. A radiograph of the dislocated shoulder is generally accepted as normal owing to several traps. Therefore if the clinician is not suspicious, this injury can easily be overlooked. Most of the cases described in the literature were overlooked dislocations reported as case reports or series. We aimed to present a case of sustained posterior shoulder dislocation. The injury was initially overlooked, the patient was admitted 1 month later and treated by modified McLaughlin procedure. Leptin as a modulator of osteogenesis.email this articleEmail this article to a colleague. save this article to My ClippingsSave this article to My Clippings. discuss this articleDiscuss or comment on this article.
Leptin, a hormone secreted by adipose tissue, but also by several other tissues, has a dual effect on bone, acting by two independent mechanisms. As a signal molecule with growth factor characteristics, leptin is able to stimulate osteoblastic cells and to inhibit osteoclast formation and activity, thus promoting osteogenesis. However, as a molecule which stimulates sympathetic neurons in the hypothalamus, leptin indirectly inhibits bone formation. This inhibitory effect of leptin mediated by activation of sympathetic nervous system can be abrogated by application of blood pressure-reducing beta-blockers, which also in...
