Current management of surgical neonates: is it optimal or do we need to improve? A national survey of the Italian Society of Neonatology
ConclusionSome critical issues emerged from this survey: the neonatologist involvement in PM, the spread of BS, and the availability of specific care protocols need to be implemented to optimize the care of this fragile category of patients. (Source: Pediatric Surgery International)

A new classification for surgical NEC during exploratory laparotomy: introduction and reproducibility assessment
ConclusionThe introduced classification system for surgical NEC shows high reliability, deepening the understanding of NEC's intraoperative exploration aspects. It promises to indicate operative strategies, enhance prognosis prediction, and substantially facilitate scholarly communication in pediatric surgery. Importantly, it explores the potential for a standardized report and may represent a step forward in classifying surgical NEC, if pediatric surgeons are open to change. Impact of Colorectal Nurse Specialist supervised parental administration of rectal washouts on Hirschsprung ’s disease outcomes: a retrospective review
ConclusionOur stoma rates are lower compared to recent UK data. This could potentially be due to emphasis on parental ability to perform effective rectal washouts at home under CNS supervision. (Source: Pediatric Surgery International)

Clinical characteristics and influence of  postoperative Hirschsprung-associated enterocolitis: retrospective study at a tertiary children’s hospital
ConclusionClassification of post-HAEC based on the first episode time and frequency was necessary. The earlier or more frequent episodes of post-HAEC have detrimental influences on long-term outcomes. Furthermore, risk factors for early and recurrent HAEC were different. Sternal cleft and pectus excavatum: an overlooked congenital association?
ConclusionOur findings suggest an underestimated association between PE and SC in the existing literature. SC patients may exhibit a predisposition to PE from birth, which may become more apparent with growth after SC repair. Consequently, PE prevention or treatment should be considered during SC repair procedures. (Source: Pediatric Surgery International)

Voiding cystourethrography in patients undergoing endoscopic decompression of duplex system ureteroceles: to do or not to do?
ConclusionsSS rate was significantly higher in patients with preoperative VUR. Instead, it was not significantly different between patients without VUR and those who did not undergo a VCUG before ED, despite all the latter who underwent a VCUG during follow-up had evidence of VUR generally in the absence of symptoms. In our opinion, a VCUG could be limited to patients developing symptoms after ED. If a VCUG is performed before ED, a preemptive treatment of VUR should be taken into consideration. (F)utility of preoperative pulmonary function testing in pectus excavatum to assess severity
ConclusionsWe found that PFT completers were not significantly different from non-completers in most structural and functional measures of pectus deformity and cardiopulmonary function. Inability to complete PFT is not an indicator of pectus severity. (Source: Pediatric Surgery International)

Comparison of the effects of ring block and dorsal penile nerve block on parental satisfaction for circumcision operation in children: randomized controlled trial
ConclusionRB and DPNB were compared in circumcision, which is the most common surgical procedure for children. Parental satisfaction, anesthesia, and analgesic effects of both blocks were found to be similar.Clinical trialsACTRN12622001211752. (Source: Pediatric Surgery International)

Management of complex pediatric and adolescent liver trauma: adult vs pediatric level 1 trauma centers
ConclusionOur retrospective review of high-grade pediatric and adolescent liver trauma demonstrated higher rates of IR and operating room use at the ATC compared to the PTC in the setting of higher Injury Severity Score and age. While the PTC successfully managed  >  95% of Grade 4/5 liver injuries non-operatively, prospective data are needed to determine the optimal algorithm for management in the older adolescent population.Level of evidenceLevel IV.
