Conclusion Outreach pharmacists are in a good position to collect unnecessary medications, and to educate patients on the prudent use
of government subsidy of medications. The economic ramification of medication disposal is significant, in terms of the cost
of medications that end up not being used. Cardiovascular medications are the main group that end up being wasted, and is
among the most expensive to subsidize. Further studies are needed to examine ways of minimising medication waste and reducing
the cost of such wastage.

Conclusions People from ‘hard-to-reach’ sectors of the population, men and people from less advantaged communities, accessed the HHA
service and were more likely to return moderate-to-high CHD risk. Pharmacists prioritised provision of lifestyle information
above the sale of a product. Our study supports the notion that pharmacies can serve as suitable environments for the delivery
of similar screening services.

Improvement of diabetes indices of care by a short pharmaceutical care program

Conclusion Our short-course pharmaceutical care program yielded measurable improvements in clinical indicators of diabetes and comorbidity
management. The results suggest that the pharmacist is a beneficial key component of integrated care for patients with type
2 diabetes. We think that the positive results observed in this first reported pharmaceutical care program on diabetes in
Turkey can be motivating and encouraging for all community pharmacists.

Conclusion The findings of this study indicated that community pharmacists perceive that there is room for improvement in the communication
between themselves and prescribers in the primary and secondary care settings, concerning the care of the stroke patient.
This highlights the need for the development of formal communication channels between community pharmacists and other members
of the healthcare team involved in the care of the stroke patient. However, the challenges of communicating patient information
across healthcare sectors are recognized.

Conclusion Unintentional non-adherence to antidepressant medication exceeded 50% in this population. Electronic reminders, in this case
text messages, were acceptable to approximately 60% of patients who reported being unintentionally non-adherent. Reasons for
refusal of electronic reminders should be investigated as this will affect the applicability of this intervention.

Conclusion We speculate that an interaction between ritonavir/lopinavir and vincristine was responsible for this severe toxicity. Vincristine
is transported by P-gp and is metabolized via CYP3A5. Ritonavir is a potent CYP3A5 isoenzyme and P-gp inhibitor. Lopinavir
is also a P-gp inhibitor. Ritonavir and lopinavir might have delayed vincristine elimination. Clinicians should be aware of
this possible interaction.

Conclusions Structured pharmaceutical care according to the IBOM-1 protocol led to more changes in drug therapy. Home visits resulted
in the clearing of redundant home drug supplies. In addition, patients were highly satisfied with the counselling at discharge
from hospital by their community pharmacist. Patient counselling at discharge from hospital by pharmacists, therefore, appears
to be a meaningful pharmaceutical care activity.

Conclusion This is the first tool developed specifically to detect potential DRPs in patients with type 2 diabetes. It was used to identify
DRPs in a sample of type 2 diabetes patients and demonstrated the high prevalence of DRPs per patient. The checklist may assist

Conclusion This review found that patient satisfaction has been measured within the community pharmacy context to a certain degree.
Further research is needed to develop and test instruments based on theoretical frameworks, to test satisfaction pre and post
hoc and in well designed randomized controlled trials and to measure changes in satisfaction over time. Novel approaches involving
an understanding of expectations and preferences of patients and matching these to the services provided also need to be explored.

Conclusion Thai out-patients were willing and able to complete questionnaires regarding potential ADRs. The questionnaire could form
part of routine out-patient monitoring, aiding identification of ADRs, and may help to increase ADR reporting in Thailand.

Conclusion Although pharmacotherapy is increasing common in the treatment of ADHD in both children and adults, there are still a lot
of questions about side effects and how best to counter them. This suggests an important role for close monitoring of children
and adults treated with stimulants or atomoxetine.

Conclusion The healthcare services in community pharmacies, currently insignificant, must undergo reforms to meet the changing needs
of modern medicines users. The pharmacist’s role in patient care is expected to grow as professional and educational standards
develop. Although pharmacists’ contributions to health care are not yet recognized, there is every reason to be optimistic
toward making patient care in community pharmacy setting a success. For this, the educational system for pharmacists has to
be adapted.

The development of a sleep disorder screening program in Australian community pharmacies

Conclusion The development and pilot of this screening tool was successful. The prevalence of sleep disorders in the sampled population
was high but generally consistent with previous studies on the general population. Furthermore, associations found may form
a foundation for a clinical algorithm to identify those at a higher risk of having or developing a sleep disorder. Further
work is required to validate this screening tool in the community pharmacy context.

Conclusion The prevalence of type 2 diabetes receiving metformin despite having a contraindication was over 80%. However, metformin
use in such conditions did not appear to increase the risks of hospitalization and death. No lactic acidosis was reported.
Other patient factors than metformin affect the risks of hospitalization and death.

Conclusions Flemish patients were satisfied with pharmacy distribution and services related to OTC medicines. They see an important role
for pharmacists and physicians to accompany them in their OTC medicine use. Our results highlighted the need to strengthen
communication between patients, pharmacists and physicians. It is also recommended that pharmacists keep an individual record
detailing patient use of OTC and prescription medicines.

Abstract This case describes a series of errors which resulted in an avoidable death of the patient. Upon being presented with the
83-year-old patient and her complaints, the physician in charge attempted to prescribe Vasolastine® (a complex preparation used, for example, in treatment of angiopathy, which is administered intramuscularly once a day).

Conclusion The results showed that a wide range of errors occurred during the dispensing process. A check performed after the initial
medication selection is also necessary to detect and correct dispensing errors. In order to decrease the occurrence of dispensing
errors, some practical measures have been implemented in the central pharmacy. But because some dispensing errors may remain
undetected, there is a requirement to develop other strategies that reduce or eliminate these errors. The pharmacy staff is
widely involved in this duty.

Conclusion The overall availability of essential medicines in the public health centers and private pharmacies of Sudan was acceptable.
Although affordability of medicines for some common diseases such as malaria and child pneumonia could be considered acceptable,
it was much higher for some other conditions including adult pneumonia. Prescription of antibiotics was high and adherence
to standard treatment guidelines for managing common and widely spread diseases such as diarrhea and malaria was low.
