In the last years understanding of mechanisms in COPD has grown. At the same time therapeutic options have increased. Therapeutic nihilism is not anymore indicated when facing a patient with newly diagnosed COPD. Different strategies are important: Nicotine cessation, long-acting bronchodilators acting against bronchial obstruction and exertional dynamic hyperinflation, and an intensive physical training in the setting of a pulmonary rehabilitation program. A proper patient and allergen extract selection, an ideally adapted application regimen and good patient-guidance are decisive for the success of the SIT. There is a long list of differential diagnoses for chronic cough lasting longer than 4 weeks in children. Diagnostic work up starts with a detailed history taking and a clinical investigation followed by a chest X-ray (in one plane) and a spirometry. For the latter reliable results can be achieved by children older then 5 years. If the diagnostic work up is still inconclusive and if the child is in good clinical condition, a 4 weeks' course of inhalation therapy with steroids and betamimetics together with a 2 weeks' course of antibiotics with a macrolide is warranted. The principle of exercise stress test and myocardial perfusions scintigraphy (MPS) is based on the detection of exercise-induced myocardial ischaemia by ECG and non-invasive assessment of myocardial perfusion respectively, MPS being the more sensitive method. The diagnostic principle of stress echocardiography and cardiac magnetic resonance imaging (CMR) for the diagnosis of coronary artery disease is based on the visualisation of ischaemia-induced wall motion abnormalities.
