CONCLUSION: Radiofrequency ablation of breast carcinomas is feasible, well tolerated, safe, and efficacious in nearly 90% of invasive tumors. The efficacy of the technique should be confirmed through extended follow-up of patients without subsequent surgical intervention in carefully designed and monitored phase III trials.

We report our experience in the computed tomography (CT)-guided percutaneous radiofrequency ablation of more than 100 benign bone tumors in the last eight years. We affirm that this should be the technique of choice in the vast majority of osteoid osteomas; it can also be applied as a definitive treatment in many cases of osteoblastomas or chondroblastomas as well as in less common benign bone tumors. CT-guided percutaneous radiofrequency ablation has proven highly efficacious and has resulted in very few complications; thus, patients tend to recover very quickly.

We present the case of a 75-year-old woman who suddenly developed hemiplegia, aphasia, and reduced consciousness three hours after embolization of a cerebral aneurysm. No complications occurred during embolization and the findings at end-procedure angiography were normal. Cranial computed tomography (CT) after embolization showed hyperdensity of the cortex and sulci of the convexity; another CT examination performed four hours later showed these findings had disappeared. [Biliary obstruction due to adenocarcinoma of the ampulla of Vater ten years after orthotopic liver transplantation.]

We present the case of a 60-year-old man who underwent orthotopic liver transplantation 10 years earlier for terminal cirrhosis caused by hepatitis B. Imaging studies showed dilation of the intra- and extra-hepatic bile ducts with no evidence of lithiasis, anastomotic stenosis, or other biliary complications that are common in patients with liver transplants. The definitive diagnosis was reached by endoscopic retrograde cholangiopancreatography. Adenocarcinoma of the ampulla of Vater should be considered among the causes of biliary obstruction in patients with orthotopic liver transplants.

CONCLUSIONS: Percutaneous anterograde evacuation of bile duct stones by dilating the papilla with an angioplasty catheter and expelling the stones with an occlusion balloon is effective, nontraumatic, and safe; this procedure maintains the anatomic and functional integrity of the sphincter. When performed by an experienced interventional radiologist, it is a viable alternative to choledochotomy.

We present the case of an asymptomatic patient with an initial clinical and radiological diagnosis of lung metastases in whom histological study diagnosed anthracosilicosis. A review of the literature shows that this presentation of anthracosilicosis is exceptional; our patient had atypical radiological findings and a very long latency period (over 50 years) after a brief (nearly 6 years) exposure to coal dust.

We describe a case of intraventricular solitary fibrous tumor.The imaging characteristics of intraventricular solitary fibrous tumors are nonspecific; the differential diagnosis should include other tumors that can affect the ventricular system such as meningioma, high grade glioma, metastasis, subependymoma, choroid plexus papilloma, ependymoma, subependymal giant cell astrocytoma, and neurocytoma. At histological study, immunohistochemical techniques allow solitary fibrous tumor to be differentiated from fibrous meningioma and hemangiopericytoma.

CONCLUSIONS: Visualizing echogenic bands within the medulla should alert radiologists to the initial phase of renal vein thrombosis and enable the appropriate treatment to be administered. With the exception of cases discovered before birth, renal vein thrombosis has a poor prognosis, with atrophy and renal failure in the long term.

CONCLUSIONS: Contrast administration improves the performance of ultrasonography in the diagnosis of liver abscesses. There are three patterns of enhancement and these correlate well with the findings at CT and MRI. Contrast-enhanced ultrasonography is very useful for defining the internal architecture of the abscess, which is important for choosing the type of treatment. Contrast-enhanced ultrasonography also enables the differential diagnosis with other focal liver lesions.

We describe the neuroimaging findings (unenhanced cranial computed tomography [CT], CT angiography, CT perfusion studies, and cerebral angiography) and the intravascular treatment (mechanical and pharmacological intra-arterial fibrinolysis). Treatment yielded good morphological and clinical results; we also analyze the follow-up of the patient.

[MRI in flexed and extended positions for the diagnosis of cervical myelopathy in Hirayama's disease.]

We report the case of a young man who presented with atrophy of the musculature of the right forearm and hand. The clinician suspected cervical myelopathy in Hirayama's disease on the basis of the findings at electromyography and recommended an MRI examination in both flexed and neutral position. The typical imaging findings for cervical myelopathy in Hirayama's disease were observed and the diagnosis was confirmed. The key findings for Hirayama's disease consist of asymmetrical medullary atrophy with involvement of the anterior horns, dorsal detachment of the dura mater, and dilatation of the epidural venous plexus.

We present the case of a patient with a generalized and diffuse disorder of venous drainage that affected the right cerebral hemisphere who presented at the emergency department with ophthalmologic signs and symptoms after thrombosis of the superior ophthalmic vein and who had three sinus pericranii located in the frontal, parietal, and occipital areas.

We report a case of hypopharyngeal squamous cell carcinoma with multiple calcified metastatic cervical adenopathies, and we review the relevant literature. The finding of calcified metastatic adenopathies from squamous cell carcinoma in the head and neck is very uncommon; only three cases have been reported to date.
