A 65-year-old female with glioblastoma multiforme (GBM) who developed aplastic anemia from temozolomide is reported. The patient presented with rapidly developing slurred speech, apathy, memory difficulties, an ataxic gait, and right-sided weakness. She was found to have GBM. She received standard external beam radiotherapy concomitant with temozolomide, followed by adjuvant temozolomide. On day 14 of adjuvant treatment, she presented with profound fatigue and spontaneous bruising and was noted to be severely pancytopenic. We describe an extremely rare case of emphysematous cystitis in a nondiabetic, nonimmunocompromised elderly patient and we summarize the pertinent literature. The only risk factors for emphysematous cystitis in our patient were old age and female sex. Emphysematous cystitis can present asymptomatically. In such instances, the diagnosis should be suspected based on imaging findings. In this report, we discuss some of the basic principles of diffusion-weighted imaging (DWI), a sensitive magnetic resonance imaging (MRI) technique that detects the mobility of water molecules within the brain parenchymal tissue. Although the most common cause of what is known as hyperintense lesions on DWI is ischemic stroke, we will present multiple cases that may also have hyperintense lesions but have a diagnosis other than ischemic stroke. Atrial flutter is a relatively common supraventricular tachycardia. Little is known about the etiology, microbiology and outcome of hospital-acquired thoracic empyema (HATE) in adults. In our study, we classified HATE into two categories: hospital-acquired pneumonia (HAP) related and non-HAP related. We found that HAP-related empyema had a significantly higher incidence of aerobic Gram-negative organisms (76% vs. 38.7%, P = 0.005), polymicrobial pathogens (40% vs. 9.7%, P = 0.008), and anaerobic pathogens (20% vs. 0%, P = 0.009) than non-HAP related empyema. Direct current cardioversion (DCCV) is an effective therapy to restore sinus rhythm (SR) in patients with atrial fibrillation (AF) and atrial flutter (AFL). Despite a high initial success rate, many patients revert back to AF within one month post DCCV. This retrospective study was designed to investigate whether surface electrocardiogram (ECG) analysis can be used to predict DCCV outcome immediately and at one month after cardioversion. Hypokalemic rhabdomyolysis caused by vasoactive intestinal peptide (VIP)-producing tumors is extremely rare and the literature is limited to one case report. The second case of an adult who presented with rhabdomyolysis due to severe hypokalemia is reported.
