Sphincters to guarantee continence are in pricipal the simplest muscles, because only two states (closed and open) seem to be important. The healthy urinary sphincter, however, provides dynamic components. During the filling phase the increase in tonus prevents urinary loss. The sphincter rapidly responds to pressure pulses caused, for example, by coughing. Contemporary artificial sphincters, however, merely generate two states and often induce atrophy and erosion. Hence the success of commercially available, continually improved implants is still limited. This communication reviews two physical principles, shape memor...

This technology has been proven to be technically reliable and helpful in certain conditions. It is of particular benefit in monitoring devices that are under a safety alert since it allows early identification of device malfunction and minimises the risk of under-report...

The in vitro engineering of functionally developed biological cartilage substitutes, based on cells and appropriate structural and soluble factors, is an attractive concept for the clinical treatment of cartilage injuries and degeneration. The field of cartilage tissue engineering has developed strongly in the last few years, bringing together the scientific, clinical and commercial interests of highly interdisciplinary communities. However, engineered grafts are still far from being the standard of care for cartilage repair. In this review we present some of the issues challenging the reproducible engineering of funct...

Chronic obstructive pulmonary disease (COPD) is one of the leading causes of death in the developed world and associated with a high individual and socioeconomic burden. Despite emerging preventive efforts and ongoing clinical trials, the frequency and mortality of COPD are expected to continue to rise over the next decades. COPD is defined as an irreversible expiratory airflow limitation, which is caused by various degrees of the following two main features: First, small airway disease (SAD), which includes airway inflammation and remodelling, and second, emphysema, which is characterised by airspace enlargement. Combination antiretroviral therapy (cART) has dramatically improved the prognosis of HIV-infected persons to a level close to a normal life expectancy in a significant proportion of treated individuals. On starting cART HIV-induced immune deficiency can be prevented or, if already present, reconstituted. Remaining morbidity and mortality is partly due to late presentation of patients, when CD4-T-cells have already fallen below 200 cells/muL often accompanied by symptomatic disease. However, at present morbidity and mortality are mainly related to comorbidities such as hepatitis and tumours at least partly associated wi...

During the last few years significant advances have been achieved in the understanding of the pathogenesis of inflammatory bowel disease (IBD). A genetic susceptibility to Crohn's disease has been proven by identification of variations as risk factor NOD2/CARD15. Functional data on NOD2/CARD15 and NF-kappaB activation indicate that an inflammatory reaction of the intestinal mucosa, as an immediate response of the innate immune system, may be necessary for the maintenance of gut homeostasis. Crohn's disease is now also discussed as an impaired and inadequate immune reaction and no longer only as a hyper-responsiveness o...

INTRODUCTION: Hyperglycaemia has been shown to be detrimental in severely ill patients. Prospective randomized controlled trials in ICU patients demonstrated the benefit of near-normoglycaemia in reducing morbidity and mortality. Recommendations of professional societies (e.g., the American Diabetes Association) on glycaemic control in hospitalised patients have recently been published. It was therefore of interest to assess whether glycaemic control of diabetic subjects in our hospital adhered to these guidelines. No recent data are available on the glycaemic control of hospitalised diabetic patients in Switzerland. M...

Despite five decades of intensive research, mechanisms initiating and stabilising atrial fibrillation (AF) are still not fully understood. Nevertheless, mapping studies, next to clinical trials and research on cellular electrophysiology, have provided key information that has led to a much more profound understanding of the arrhythmia. Contact mapping using multi-electrode arrays (MEAs) is the gold standard for high-resolution mapping in basic research and clinical trials, and continuously contributes to a better description of mechanisms perpetuating AF. It thereby provides information needed to target and test new ph...

MicroRNAs (miRNAs) are small non-coding RNAs that are expressed in higher eukaryoates and have even been found in viral genomes. They usually act as endogenous repressors of target genes by either inhibiting translation, causing mRNA degradation, or by a combination of both mechanisms. More than 850 mature miRNA sequences have been identified in humans, and although this accounts for less than 2% of human genes, it is predicted that 30% of mRNAs are targeted by miRNAs. miRNAs play critical roles in most cellular processes including development, differentiation, and the homeostasis of both a cell and an organism. The functional capacity of the human lung as gas exchanger is to a large extent determined by structural design. Quantitative structure-function correlations can be established by morphometry. A very large surface of air-blood contact, together with a very thin tissue barrier, are required to permit adequate oxygen uptake under work conditions. However, these design features also pose problems, such as how to ventilate and perfuse this large surface evenly and efficiently, or how to ensure mechanical stability against surface forces with a minimum of supporting tissue. The discussion focuses on the extent to which nove...

Atrial fibrillation is the most common sustained cardiac arrhythmia in the general population. Unfortunately, current treatment strategies aiming at the elimination of atrial fibrillation have limited long term success rates and significant risks. In this context, recent publications have provided many insights on potentially treatable risk factors for the occurrence of atrial fibrillation, such as alcohol, blood pressure, obesity, inflammation and nutritional factors. In this review, we summarise the current evidence on these risk factors and indicate areas in need of further investigation. The current evidence shows ...

This review aims to provide physicians with an overview of the potential of procalcitonin to guide antibiotic therapy in respiratory tract infections and in sepsis. Knowledge of the strengths and weaknesses of procalcitonin are prerequisites for a rational and safe use in clinical routine. In most infections a true gold standard for diagnosis does not exist, therefore physicians must remain sceptical towards observational studies evaluating procalcitonin. Interpretation of procalcitonin levels must always include the clinical setting and knowledge of assay characteristics, particularly the setting of specific cut-off r...

The precise diagnosis of a severe haemorrhagic disorder is usually not too problematic. However, physicians are most often faced with individuals consulting for mild haemorrhagic symptoms, particularly occurring after a surgical intervention. The problem here is to evaluate whether this person really has a bleeding disorder requiring special investigations and treatments, particularly if another invasive procedure is planned. The key point is to ask the appropriate questions to discriminate bleeding occurring in normal subjects from that occurring in patients with haemostatic disorders. Recently, bleeding questionnaire...

CONCLUSIONS: Both prescribing physicians and;patients provided with assistance to die quite often mentioned pain and other concerns, many of which were objectively assessable and related to unbearable suffering or unreasonable disability. Concerns referable to autonomy and individual judgement were more often noted by people seeking help than by the prescribing physicians.

CONCLUSIONS: CysC was low at diagnosis of hypothyroidism and significantly increased following T4 replacement in patients with primary as well as central hypothyroidism. T4 replacement decreased Cr levels in patients with primary hypothyroidism whereas Cr remained unchanged in;patients with central hypothyroidism. CysC may not accurately reflect kidney function in patients with primary and central thyroid dysfunction.

Food allergies have increased over the past decade and are an important problem in daily clinical practice. They affect 6% of children and 3 to 4% of adults. Furthermore, around 20% of the population falsely believe that they are allergic to some foods and follow unnecessarily restrictive diets. For infants, the problem is even more acute as they need appropriate feeding in order to achieve normal growth and avoid bone and metabolic problems. Although any food can cause a reaction, few foods are responsible for the large majority of the symptoms: i.e., milk, eggs, wheat, peanuts, nuts, fish, shellfish. Of these, cow's ...
