Single Center Prospective Study of Cross-Clamp versus Balloon Occlusion in Robotic Mitral Surgery
CONCLUSIONS: Compared to aortic cross-clamp, balloon occlusion has similar perioperative and early postoperative outcomes. Additionally, it likely introduces a 10-minute reduction in total CPB time.PMID:38657703 | DOI:10.1016/j.athoracsur.2024.04.004 (Source: The Annals of Thoracic Surgery)

Bridge to Transplantation: Policies Impact Practices
Ann Thorac Surg. 2024 Apr 18:S0003-4975(24)00290-X. doi: 10.1016/j.athoracsur.2024.03.041. Online ahead of print.ABSTRACTSince the development of the first heart allocation system in 1988 to the most recent heart allocation system in 2018, the road to heart transplantation has continued to evolve. Policies were shaped with advances in temporary and durable left ventricular assist devices as well as prioritization of patients based on degree of illness. Herein, we review the changes in the heart allocation system over the past several decades and the impact of practice patterns across the United States.PMID:38642820 | DOI:1...

Intersection of Race, Rurality, and Income in Defining Access to Minimally Invasive Lung Surgery
CONCLUSIONS: Rural white populations living close to the federal poverty line may suffer a burden of disparity traditionally observed among poor Black people. This suggests a need for health policies that extend services to impoverished, rural areas to mitigate social determinants of health.PMID:38641193 | DOI:10.1016/j.athoracsur.2024.03.040 (Source: The Annals of Thoracic Surgery)

Hemodynamic Comparison of the On-X and Top Hat Mechanical Aortic Valve Prostheses
CONCLUSIONS: There was no difference between the two valves with respect to the AV area index at discharge, and hemodynamic function was stable during longer-term follow-up. The longer aortic cross-clamp time observed in the On-X group may indicate increased complexity in implantation compared to the Top Hat group.PMID:38636685 | DOI:10.1016/j.athoracsur.2024.04.003 (Source: The Annals of Thoracic Surgery)

Late Outcomes of Porcine and Pericardial Bioprostheses after Mitral Valve Replacement in 1162 Patients
CONCLUSIONS: We demonstrate similar rates of 10-year survival and structural deterioration with porcine and pericardial bioprosthetics in mitral valve replacement. This study suggests a lack of major improvement in durability of mitral bioprosthetic valves over time. Failure mode may have greater influence on surgeon decision-making of valve choice.PMID:38631662 | DOI:10.1016/j.athoracsur.2024.04.002 (Source: The Annals of Thoracic Surgery)

Perioperative Immunotherapy for Node-Negative Non-Small Cell Lung Cancer - Current Evidence and Future Directions
Ann Thorac Surg. 2024 Apr 13:S0003-4975(24)00280-7. doi: 10.1016/j.athoracsur.2024.03.035. Online ahead of print.ABSTRACTNeoadjuvant immunotherapy has gone from an idea to an indication in locally advanced lung cancer. Several phase III trials have demonstrated the superiority of neoadjuvant chemoimmunotherapy compared to chemotherapy in this setting. Although such progress has revolutionized the treatment of locally advanced disease, the unmet needs of stage I and stage II patients without lymph node disease have largely been under-represented in existing trials. Up-front surgery with few patients going on to complete adj...

Re-repair after translocation of an anomalous left coronary artery
Ann Thorac Surg. 2024 Apr 13:S0003-4975(24)00282-0. doi: 10.1016/j.athoracsur.2024.03.036. Online ahead of print.ABSTRACTWe repaired a left anomalous aortic origin of coronary artery (AAOCA) using coronary ostium translocation/re-implantation. Re-repair was required due to suboptimal flow pattern in postoperative transesophageal echocardiography (TEE), confirmed with coronary angiography. We emphasize the importance of early recognition, diagnosis and intervention to deal with a problem with the coronary repair.PMID:38621655 | DOI:10.1016/j.athoracsur.2024.03.036 (Source: The Annals of Thoracic Surgery)
