We describe the management of a patient who presented with symptoms of severe congestive heart failure. A 48-year-old man was initially seen in the emergency room, admitted to the hospital, and worked up with a transthoracic echocardiogram, a transesophageal echocardiogram, and a computer tomography scan of the chest. All cardiac valves were normal, as was the left ventricular ejection fraction. Conclusion: Posterior pericardial annuloplasty with CABG in the treatment of IMR provides efficient mitral repair and significant decrease in the left atrium and left ventricle diameters, and provides a significant increase in left ventricular function. These results show IMR to be as effective as the other annuloplasty techniques. IMR is performed with autologuous material and therefore does not entail any risk of complications from prosthetic material and is highly cost-effective. CONCLUSION: Combining the data, we demonstrated that acute MI caused an early reduction in MMP-3 levels. The range of MMP-3 reduction is limited compared with other factors predicting MI, such as cTnI, which increases its usefulness. We demonstrated, however, that plasma fluctuation in MMP-3 levels could be used as a supplementary independent predictor of cardiovascular events in patients with stable coronary artery disease. We describe 2 cases of successful conversion from beating-heart (BH) to arrested-heart (AH) totally endoscopic coronary artery bypass surgery (TECAB). The preoperative computed tomographic (CT) scan of the first patient showed significant artherosclerosis of the femoral vessels in both groins. Therefore we tried to avoid inguinal cannulation by performing an off-pump BH-TECAB. We report 2 cases of minimal invasive revision for postoperative bleeding following totally endoscopic coronary surgery (TECAB). In the first case the revision was performed totally endoscopically without additional incisions. In the second patient revision was performed via enlargement of the camera port to a minithoracotomy. Both revisions were successfully conducted without sternotomy. We demonstrated that the technique of minimal invasive revision can further reduce the rate of conversion to sternotomy in TECAB operations. CONCLUSION: The procedure is safe, especially in patients with intact LIMA-to-LAD grafts needing lateral and inferior wall revascularization. Multislice computed tomography scanning allows better preoperative planning, especially regarding the site of implantation of the proximal graft, allowing a less invasive incision. The off-pump technique preserves cardiac and pulmonary function. The in-hospital death rate (1.8%) compares very well with the EuroSCORE-predicted mortality (14.2%). A selective left coronary angiography revealed a fistula connecting the left main coronary artery with the pulmonary artery in addition to severe left main coronary artery disease. We present the case of a 34-year-old woman with isolated PAPVC of the entire left lung. The patient underwent operation with a side-to-side left atrio-vertical vein anastomosis while on cardiopulmonary bypass and under cardioplegic arrest. She was discharged without complications and with a gradient of 2 mm Hg across the anastomosis. Left-sided PAPVC can be repaired with minimal morbidity and mortality. We describe an alternative technique of simple median sternotomy followed by reduction and wire fixation of the sternal fracture. The 2 patients described in this report had traumatic comminuted and displaced sternal fractures. CONCLUSIONS: In single-ventricle anatomy, cases of a bilateral SVC are more often associated with an IIVC than a single SVC. Patients who undergo bilateral CPAs with an IIVC have a difficult early postoperative course. We should look for IIVC and either exclude or prove IIVC in cases of bilateral SVCs. A 61-year-old man presented with multiple episodes of hemoptysis. A computed tomography scan revealed a 70-mm saccular aneurysm of the distal aortic arch. During a surgical repair via a midline sternotomy and a left thoracotomy, a saccular aneurysm was found to protrude into the lung, which had chronically healed. The patient underwent replacement of the Dacron graft, and he recovered well postoperatively. This experience prompted us to report the findings of this unique case.
