The prevalence of CYP2C19 mutations in Turkish patients with dyspepsia.

Conclusions: This is the first study investigating CYP2C19 polymorphism in dyspeptic Turkish patients. Our investigation revealed that the most common CYP2C19 genotype was wild type CYP2C19 in dyspeptic Turkish patients. Dyspeptic Turkish patients are extensive metabolizers for proton pump inhibitors. This finding might have impact on the clinical consequences for the treatment strategies in dyspepsia.

Conclusions: In inflammatory bowel disease, transforming growth factor-B1 can be used as a marker for differential diagnosis of active ulcerative colitis patients and remission ulcerative colitis patients. Nevertheless, more studies with larger patient groups are necessary.

Cell death by apoptosis usually occurs in a regulated, limited fashion. Only selected target cells are deleted in the process of ontogeny, development and regeneration. In contrast, under pathophysiological conditions, apoptosis imposes as a massive, chaotic, non-selective event that may occur for periods or even up to decades. Conclusions: Addition of sodium nitroprusside and glutathione into the Wisconsin solution decreased levels of ICAM-1 molecule expression, which reflects lower levels of preservation injury. In this study, the addition of verapamil to the perfusate/preservation solution for reducing the intracellular calcium accumulation had no effect on tissue ICAM-1 molecule expression.

Conclusions: With this study, we have documented that in viral hepatitis patients, aflatoxin exposure is significantly higher than in healthy subjects in Turkey and it may play an important role in the development of hepatocellular carcinoma. Thus, large studies exploring the relation between aflatoxin exposure, viral hepatitis status, and risk of hepatocellular carcinoma development are needed.

Conclusions: The prevalence of fascioliasis was hypoendemic in the province of Mersin. There were no significant differences in the Fasciola hepatica prevalence between the groups with and without family history of fascioliasis. However, studies with larger sample sizes may reveal a difference.

In conclusion, computerized tomography/magnetic resonance imaging and octreotide scintigraphy may be insufficient to show disseminated disease and asymptomatic second primary malignancies. Therefore, positron emission tomography is a valuable promising option for the evaluation of gastroenteropancreatic neuroendocrine tumors and concomitant or metachronous malignancies. Lifelong follow-up by a multidisciplinary oncology team is needed so that a long-term survival can be achieved with integrated multimodal systemic treatment approaches.

We report a case of spontaneous hepaticoduodenal fistula following bile duct injury. Initially, Roux-en-Y hepaticojejunostomy had been planned for the patient, but as the patient did not show any symptoms or findings of biliary obstruction, we preferred a non-operative management. The fistula allowed adequate biliary drainage, and the patient has been followed regularly by the outpatient clinic with good clinical results for approximately five years.

We present three cases with giant intra-abdominal cysts (one mesenteric cyst, two ovarian) that were initially misdiagnosed as refractory ascites in another hospital. On ultrasound examination, displacement of bowel loops to outside the fluid without dispersion of fluid around the liver and spleen were the main findings suggesting the possibility of giant intra-abdominal cysts in our patients. In all three patients, the diagnosis of giant intra-abdominal cysts was confirmed after their total excision. Clinicians must consider processes other than ascites in the differential diagnosis of large abdominal fluid accumulation.

We herein present a patient referred to our clinic with the complaints of flatulence and left upper quadrant abdominal pain who was diagnosed to have pancreatic cystic neoplasia radiologically. The septated cyst was defined to be 9x12 cm in diameter by abdominal computed tomography and by ultrasonography originated from the tail of the pancreas. CONCLUSIONS: Friend leukemia virus integration-1 can be expressed in a variety of tumors, and is helpful in making the diagnosis of Ewing's sarcoma/primitive neuroectodermal tumor. We think that this protein is not expressed in gastrointestinal stromal tumors and it is not a part of the pathogenesis of this disease.

CONCLUSIONS: Our report is the first to describe the frequency of Celiac disease in patients with lymphoma in the southeast region of Turkey. In our study, there was no evidence that Celiac disease is a pre-malignant condition for lymphoma. Serological screening for Celiac disease in lymphoma patients does not seem to be necessary.

CONCLUSIONS: Transmission of hepatitis C virus by blood and blood products was better recognized in all groups tested, but the other means of infection were either overestimated (skin contact, sharing toilet and clothes) or under-recognized (blood-contaminated objects). More vigorous education programs are needed to increase awareness of hepatitis C virus in various risk groups in our country.

CONCLUSIONS: Baicalin and octreotide exert significant protective effects on severe acute pancreatitis-induced intestinal mucosa injury via a mechanism that is associated with inhibiting inflammatory mediators and inducing apoptosis. In comparison with the pharmacological action of octreotide, we believe that baicalin, as a new drug, has similar protective effects on the intestinal mucosa of severe acute pancreatitis rats, and therefore deserves further study and development.

CONCLUSIONS: Increased common bile duct diameter, biliary dilatation, biliary stent insertion, prolonged total procedure time, and hilar cholangiocarcinoma have higher post-endoscopic retrograde cholangiopancreatography cholangitis risk. Moreover, the presence of periampullary diverticula was found to decrease cholangitis risk.

CONCLUSIONS: Treatment with ethanolic extract of propolis improved the biochemical and histopathological findings in a rat model of experimental pancreatitis. Although our findings suggest that ethanolic extract of propolis might be considered an effective agent for the treatment of acute pancreatitis, this notion should be supported with further experimental and clinical investigations.

We describe herein a case of inflammatory myofibroblastic tumor of the liver with a review of the literature. A mass occupying the right lobe of the liver was excised in a 48-year-old woman, who previously presented with weakness, fever, progressive weight loss, and right upper abdominal pain. The lesion was an unencapsulated light brown tumor (largest diameter 6 cm) without necrosis or hemorrhage. Benign multicystic peritoneal mesothelioma is a rare tumor that occurs mainly in women in their reproductive age. It is characterized by the formation of multiple, thin-walled, multilocular cysts that frequently produce large, intra-abdominal masses. In conclusion, mesenteric heterotopic pancreatitis is seen very rarely and may be an unusual cause of acute abdomen. If the pathologic condition develops in the heterotopic tissue, as in the case of heterotopic pancreas, signs and symptoms of the disease may cause confusion in the clinical diagnosis. We agree that preoperative diagnosis of heterotopic pancreas is still difficult, even in a symptomatic patient.

We report the case of a 66-year-old female with recurrent episodes of rectal bleeding. She underwent a colonoscopy and was diagnosed with diffuse cavernous hemangioma of the transverse colon. A laparoscopic extended right hemicolectomy was performed. CONCLUSIONS: Although still within normal ranges, procalcitonin levels were slightly elevated in Crohn's disease but not in ulcerative colitis patients compared to healthy controls. Serum C-reactive protein is a reliable marker for disease activity in inflammatory bowel disease. Procalcitonin has no diagnostic value in determining disease activity.

CONCLUSIONS: When tumors of the biliary system are divided as intrahepatic and extrahepatic cholangiocarcinomas, MMP-2 expression was present in the extrahepatic cholangiocarcinomas including gallbladder carcinomas. Like the intrahepatic cholangiocarcinoma, Klatskin tumors also did not express MMP-2. This can be related with its characteristic growth pattern. MMP-9 and MMP- 14 were present in metaplasia, dysplasia carcinoma sequence in all of the bile tract tumors, suggesting that MMPs play an important role in carcinogenesis. Gastric infection by Strongyloides stercoralis: a case report.

We report a gastric infection case of S. stercoralis. A 72-year-old Turkish man was admitted to the Department of General Surgery with complaints of epigastric pain, nausea and vomiting for five days. Upper gastrointestinal endoscopy evidenced a diffuse infiltrate in the stomach, causing decreased distensibility. Microscopically, superficial chronic inflammation with hyperplastic reactive changes and damaged surface was observed. Histopathological examination of the gastric mucosa showed numerous cross-sections of adult worms, eggs and rhabditiform larvae of S. stercoralis developing in the gastric crypts.

We present a 34-year-old woman who had suffered from several episodes of melena for the past three years. Endoscopic examinations were normal. Computed tomography showed a large target lesion over the right abdomen and an image representing intestinal malrotation, which was supported by enteroclysis. In this report, we present a case with multiple inferior mesenteric arteriovenous fistulas, considered idiopathic due to no known cause related to the etiology, and associated ischemic colitis, which were clearly demonstrated by multidetector computed tomographic angiography. Surgical management of Mirizzi syndrome.

Conclusions: The essential part of the management of patients with Mirizzi syndrome is to determine the best surgical procedure in the preoperative period. In type I patients, simple cholecystectomy is generally enough, but sometimes T-tube insertion may be required, while the cases with types II-IV require more complex surgical approach, such as cholecystectomy and bilioenteric anastomosis. Roux-en-Y hepaticojejunostomy is an appropriate procedure with good outcome.

Conclusions: Although insignificant, some of the children with IBD had low levels of serum zinc and these patients had lower Z-scores than the others. Further studies including large numbers of patients may reveal a certain effect of zinc on bone development in patients with inflammatory bowel disease.

Does the severity of tissue eosinophilia of colonic neoplasms reflect their malignancy potential?

Conclusions: Our findings suggest that the intensity of tissue eosinophilia is most prominent in adenomas including serrated adenomas and is diminished from adenoma to carcinoma. This finding may be used as a diagnostic indicator.
