Background. Intravenous thrombolysis has gained widespread acceptance during the last 10 years, and is the only specific treatment approved for cerebral infarction. Haukeland University hospital introduced thrombolysis in 1998. The aim of this paper is to summarize our experience with the first 100 patients who had stroke caused by middle cerebral artery (MCA) occlusion and were treated with thrombolysis. Materials and methods. Patients receiving intravenous thrombolytic treatment between 1998 and 2005 were prospectively included into our database. We registered demographic data, risk factors, time aspects, short and l...

Background. After the neonatal period, the incidence of arterial cerebral infarction is 1-2/100 000 children/year. Thrombolysis in cerebral stroke is recommended for adults, but is still controversial for children. The aim of this paper was to provide an overview of documentation on treatment with thrombolysis after arterial cerebral infarction in children. Material and methods. The article is based on literature identified through a non-systematic search in PubMed and own clinical experience in treating young adults with cerebral infarction. Results. In the western world cardiac disease, cardiac interventions and infe...

Background. Effectivization of the Norwegian out-of-hours primary care medical services has probably rendered the services less available to residents in nursing homes in many places. In Bergen, an out-of-hours medical service especially dedicated to residents in retirement and nursing homes and people with special needs living at home, was established in March 2006. We wanted to assess this medical service after its first year of activity. Material and methods. Data were prospectively recorded on when the services were used and characteristics of the users . Information about the institutions was also recorded. Result...

Background. Knowledge of autoimmune hemolytic anemia is rapidly increasing, especially on pathogenesis, associated disorders and aspects of transfusion immunology. New treatment options have emerged. Material and method. This review is based on selected publications, a non-systematic search in PubMed and the authors' own research. Results. Autoimmune hemolytic anemia is a heterogeneous group of diseases. The warm-antibody type is frequently associated with chronic lymphocytic leukemia or autoimmune systemic disease. Corticosteroids are still the first-line therapy and splenectomy is second-line, while rituximab has bec...

Background. Norwegian media often claim that a disproportionately high number of deaths in Norway are caused by overdoses. The police statistics in 2007 showed an increase in the number of overdose deaths, and many media postings focused on this in autumn 2008. In this paper we investigate the basis for those police statistics and discuss problems associated with calculating deaths caused by use of illegal substances. Material and methods. The police registries of overdose deaths were anonymized in the period 2005-2007. The deaths were coded according to type of substance involved. Results. In 2007, 62 deaths were code...

Phytoestrogens (plant oestrogens) have become popular alternatives to conventional oestrogen products, and various preparations are marketed as effective in the treatment of menopausal symptoms. A number of clinical trials have been conducted, but it is still difficult to assess the effectiveness of plant oestrogen treatment. If treatment is started, the same precautions should be taken as with conventional oestrogen treatment.

A man in his late fifties was treated with acitretin for psoriasis. The treatment was discontined when liver enzymes started to increase. He subsequently developed erythrodermia, weakness of proximal muscle groups, heliotrope rash, Gottron's papules and elevation of creatine kinase to more than 9000 U/L. However, histological examination of a muscle biopsy displayed neuropathic atrophy and only a few scattered necrotic fibers. Alpha 1 foetoprotein was substantially elevated (1600 kU/L) and computed tomography showed two hepatic tumors, lytic lesions in the spine and in the ninth rib. Although no chronic liver disease c...

The material comprises all patients treated for carotid stenosis, with either carotid thrombendarterectomy (CEA) or endovascular stenting, in the period 2001-2008. All procedures were prospectively recorded in a database. A neurologist examines the patients before, and 1 and 12 months after treatment. Results. 408 carotid stenoses were treated in the observation period. Median age (range) was 68 years (21-85), and 125 (31 %) patients were women. 206 (64.2 %) of the 321 stenos...

Background. In Norway, repair of abdominal aortic aneurysms is performed in more than 800 patients annually. The paper delivers an up to date discussion of infrarenal abdominal aortic aneurysm repair. Materials and methods. A systematic search was performed in Pubmed and literature containing the search terms <<abdominal aortic aneurysm>> and <<mortality>> (from 2004 to 2009) was retrieved. Background. At the beginning of the new millennium the world's leaders agreed on eight MDG's (Millennium Development Goals). This paper addresses the fourth goal: The number of children who die before their fifth birthday should be reduced by two thirds from 1990 to 2015. Material and methods. The article is based on literature identified through a non-systematic search in Medline and publications from WHO and UNICEF available on the internet. Results. From 1990 and up to now childhood mortality has decreased from about 90 to 70 per 1 000 live birth. In 2007, 9.2 million children died, 97 % of them in the 68 poorest co...

Material and methods. The article is based on literature identified through non-systematic searches in Medline, publications by WHO, UNICEF and Partnership for Maternal, Newborn & Child Health and the author's experience from neonatal medicine in Africa. Background. Due to long-term capacity problems in the psychiatric acute ward, we tried to canalise acute admissions due to life crises (and not serious mental disease) to a new short-term in-patient crisis unit. Our hypothesis was that the opening of this unit would lead to fewer admissions to the psychiatric acute ward and that this change would be reflected by an increase of patients with a more severe psychopathology. Material and methods. The study had a quasi-experimental design. Two patient groups in a psychiatric acute ward (from separate catchment areas) were compared before (2.1.2003-1.6.2003) and after (2.1.2...

Background. Congenital malformations of the kidneys and urinary tract may have changed over time. Material and methods. Data for diagnosis, treatment and results were retrospectively recorded in children born in one of three Norwegian counties 1987-2006; their age at time of recording was from 1 to 21 years. Results. 389 of 142 986 (2.7 per 1000) live born children had malformations of the kidneys and/or urinary tract. The prevalence was higher for children born in the period 1997-2006 (241/70 217; 3.4 per 1000) than in 1987-1996 (148/72 769; 2.0 per 1000), p< 0.0011. It is well documented that patients suffering from asthma have rhinitis more often than those without. Likewise, those suffering from nose and sinus disease, e.g. allergic rhinitis, sinusitis and nasal polyposis,...

We present two hypotheses on mechanisms of drug-resistant epileptic seizures. Material and methods. The article is based on our own clinical experience and literature identified through a PubMed search using the words: refractory, intractable, pharmacoresistant, drug resistant epilepsy/seizures. Results. There is no international consensus on the definition of drug-resistant epilepsy. Some epilepsy types appear to respond to drug treatment, while others are more resistant. Animal models and refractory patients have been studied to find mechanisms that can explain the lack of drug response in some types of epilepsy. Current...
