Good data on background seroprevalence of major transfusion transmitted infections is lacking in Mali. We gathered data on the rate of positive donations of human immunodeficiency virus (HIV), hepatitis C virus (HCV), hepatitis B virus (HBV) and syphilis among blood donations in Mali for calendar year 2007. Donations with repeatedly reactive results on screening enzyme immunoassay (EIA) were considered to be seropositive. We report a female group A, D- patient with cerebrovascular disease who was induced to develop IgG immune anti-D which can be detected on the 15th day of hospitalization after the relatively regular time interval of transfusion of 1800ml apheresis FFP. Therefore, in making clinical decisions to transfuse FFP or apheresis FFP, the best choice is considered as the same or compatibility with RhD group, in case Rh immunization can be induced and trigger some potential serious problems in future emergent transfusion or pregnancy, especially in high incidence of D antigen countries.

CONCLUSION: Plasmodium represents the third risk of blood transmitted infectious agents after hepatitis B virus, syphilis, and before HCV and HIV in Senegal. The medical questionnaire is not useful enough for asymptomatic carriers deferral, and we propose to introduce Plasmodium screening. The screening for Plasmodium pLDH by Elisa technique seems to be the best tool in endemic area and the strategy of systematic screening is the most suited in terms of blood transfusion safety.

The ministerial decree of 12 January 2009 fixing the criteria for the selection of blood donors establishes the list of contraindications for blood donation according to the appendix to the European directive 2004/33/EC. Although a number of research have been realized in the aim to rationalize the use of platelet concentrates, these blood products remain absolutely necessary for patients with therapeutic aplasia and for some surgical patients. We will discuss in this work: the main rules of platelet transfusion procedures, threshold values for platelet transfusion, platelet doses, place of curative and prophylactic strategies, refractoriness to platelet transfusion, HLA immunization.

The preparation of labile blood products in a blood bank is in permanent technological progress. Many operations, such as blood centrifugation, components separation, etc. are now performed by automated devices. A new generation of equipments is able to prepare blood products by reducing the number of manual operations. Therefore, buffy-coat platelet concentrate preparation and whole blood preparation can be prepared by these automated systems. Consequently, this directly impacts working conditions of employees, quality of blood products and process management.

CONCLUSIONS: Even when sociodemographic variables and self-rated health are considered, socioeconomical status stays associated to blood donor status. Thus, this article highlights the importance of socioeconomic status in the blood donors' population structure. This study also shows that occasional donors, that should be resensitized, belong to the same socioeconomical categories than regular donors. Finally, this article provides research and sensitizing clues for some of the socioeconomic status analysed.

We report a case of Wilson's disease revealed by acute intravascular repeating hemolytic anemia associated with liver failure. The initially negative etiologic investigation was directed by occurred of liver failure. The genetic study allowed to discover an other similar case. The evolution was favourable under treatment with zinc sulfate and penicillamine. DISCUSSION: Diagnosis of Wilson's disease must be considered in case of acute hemolytic anemia associated with liver failure in young adults.

CONCLUSION: In about three cases out of four, transfusion-induced hypotension was associated with other clinical reactions. Indeed, hypotensive transfusion reactions were identified, having an incidence of 3.2 for 100000 blood units transfused. Furthermore, there was no explanation found for the incidence increase in our region during 2007. A national study was suggested to analyse the national data as well as a prospective study to clear out this type of transfusion reactions.

[Regionalization's experience of tranfusionnel advice.]

We represent an organization of transfusionnel advice at a regional level and we develop arguments and stages having allowed us to lead to this choice. This target was reached in two stages, which took place over 3 years. The regional transfusionnel advice leans on three fundamental points: a planned permanent organization, skilled and formed actors as well as adapted tools. Tending to a homogeneous organization between every blood bank center seems to be coherent towards the current configuration of blood donation in France.
