Contrast Enhancement and Histopathological Findings in Vestibular Schwannomaemail this articleEmail this article to a colleague. save this article to My ClippingsSave this article to My Clippings. discuss this articleDiscuss or comment on this article.
We present a case of a large vestibular schwannoma with heterogeneous contrast uptake on MRI. Using neuronavigation, histological samples were obtained during surgery from an area with homogeneous contrast uptake and from a central tumor portion without contrast enhancement on MRI. Intraoperative investigation found no evidence of necrotic tumor, and histopathological examination revealed an active tumor in both sections, with no central necrosis. This finding illustrates the surgical experience that “necrotic tumor areas” on MRI may not be consistent with intraoperative findings. Lack of contrast uptake in pre...

Zentralbl NeurochirDOI: 10.1055/s-0029-1202359AbstractThe formation of catheter tip granulomas is an increasingly observed serious complication of intrathecally administered medication. This complication, which is frequently associated with neurological disturbances, has previously been attributed to high dosages and high concentrations of intrathecal morphine. Much less commonly, intrathecal hydromorphone and intrathecal baclofen have also been associated with intrathecal granuloma formation. In the current case, we report a patient who developed her first catheter tip granuloma after 20 months of intrathecal morphine. Af...

We describe a patient with an intracavernous aneurysm in whom the area of increased aneurysm pulsatility as demonstrated by high-temporal resolution CTA was identical to the area of aneurysm growth on follow-up. A 35-year-old man presented with headaches and a sixth nerve palsy. CT including dynamic CTA demonstrated a pulsating intracavernous aneurysm. On follow-up one week later, the patient had developed hypesthesia in his V1 and V2 distribution and repeat imaging demonstrated aneurysm growth in the aneurysm part that demonstrated pulsatile movements on dynamic CTA. Stent-assisted coiling of the aneurysm was performed an...

Zentralbl NeurochirDOI: 10.1055/s-0029-1216362AbstractThe development of a space-occupying hemispheric infarction occurs in a subset of patients with ischaemic cerebrovascular stroke. It is a life-threatening condition with a high mortality rate of up to 80% with medical therapy alone. Previous retrospective and uncontrolled case series have suggested that decompressive hemicraniectomy can significantly reduce mortality to 20–30% compared to conservative treatment. This evidence has now been confirmed by the data of prospective randomised studies. The data also indicate that the reduction of mortality is not accompan...

We report our experience in 3 consecutive cases of thalamic abscess, treated by stereotactic puncture as the first step, followed by histological analysis, external drainage and targeted intrathecal and systemic antibiotic therapy. Deep-seated abscesses seem to behave differently as they are associated with an increased risk of intraventricular rupture and antibiotic resistance, a fact which justifies a more aggressive and immediate surgical treatment. Zentralbl NeurochirDOI: 10.1055/s-2008-1080942AbstractInjuries relevant to neurosurgeons in central Europe are rarely caused by wild animal attacks, whereas dog bites in urban areas and farm-related attacks from cattle and horses are well-known to cause sometimes fatal head injuries. The authors describe the first case of a perforating frontobasal injury caused by a wild deer's antler. Associated lesions involved the endonasal area and the left orbit. A multidisciplinary approach was used to address all lesions in a one-stage operation resulting in a full recovery of the patient. Apart from the problem of a possible contam...

Zentralbl NeurochirDOI: 10.1055/s-0028-1082055Abstract Recently, three-dimensional CT angiography (3D-CTA) has been used as a diagnostic method in postoperative evaluation after aneurysm clipping. A systematic analysis of possible predictors for good image quality needs to be conducted if 3D-CTA is to replace digital subtraction angiography in the postoperative setting. The aim of this study was to evaluate the influence of clip orientation on image quality and to assess titanium clip artefacts quantitatively on 3D-CTA.[...]© Georg Thieme Verlag KG Stuttgart · New YorkGet connected:Table of contents  |  Abstract  |...

Zentralbl NeurochirDOI: 10.1055/s-0028-1082061Abstract Surgery on the degenerative cervical spine disorders aims at decompression of the neural structures and restoring the physiological profile of the cervical spine. The aims of internal fixation are to gain primary stability, introduce a bony fusion and to correct the shape of the spine. The present study will give answers to the following questions: 1. What is the overall revision rate following an operative treatment of degenerative cervical disorders using common operative techniques? 2. Is there any influence of the fusion length to the overall revision rate and espe...

Fiber Tracking with Distinct Software Tools Results in a Clear Diversity in Anatomical Fiber Tract Portrayalemail this articleEmail this article to a colleague. save this article to My ClippingsSave this article to My Clippings. discuss this articleDiscuss or comment on this article.
Zentralbl NeurochirDOI: 10.1055/s-0028-1087212Abstract Fiber tract portrayal, based on diffusion tensor imaging (DTI), is becoming more and more important in functional neuronavigation. No standard exists to guarantee anatomically correct fiber tract depiction for neurosurgical purposes. Therefore, showing the anatomically correct extension of fiber tracts beyond the pure connection of functional areas remains an area of important research and investigation. Standards for fiber tracking software applications are elusive. The purpose of this study was to compare the performance of different fiber tracking software tools (FT...

Zentralbl NeurochirDOI: 10.1055/s-0028-1082321AbstractBasal encephaloceles in western countries occur in 1 of every 35 000–40 000 live births; with an incidence of less than 10% they are the least common of all encephaloceles. Certain subtypes such as transsphenoidal variants may be as rare as 1 in 700 000 live births. These rare encephaloceles are classified into five anatomic types: spheno-ethmodial, transsphenoidal, spheno-orbital, transethmoidal, and spheno-maxillary. Here we present an exceedingly rare variant of a non-midline basal encephalocele of the spheno-orbital type, which was treated by...

Zentralbl NeurochirDOI: 10.1055/s-2008-1077075Abstract Postoperative monitoring of the electrode position is important to evaluate the best stimulation site in deep brain stimulation. MR imaging is excellent for ruling out postoperative complications e.g. haemorrhage, but its accuracy in electrode localisation is still controversial. The reasons for this are the size of the artefact around the electrode and its unclear relation to the electrode position (concentric or eccentric). The goal of this study was to determine the relation and size of these artefacts to the electrodes by comparing the position of the electrodes in...
