We describe here an easy method for poly-axial screw removal. Leaving or placing a small piece of rod, attached firmly by a nut, tightens the head of the poly-axial screw. We present here a patient with an AVM repeatedly treated at our Leksell Gamma Knife unit with radiographically confirmed obliteration of the AVM which subsequently began bleeding. This case demonstrates the possibility of late complications in radiosurgically treated AVMs even after their demonstrable obliteration. We present a case of a large vestibular schwannoma with heterogeneous contrast uptake on MRI. Using neuronavigation, histological samples were obtained during surgery from an area with homogeneous contrast uptake and from a central tumor portion without contrast enhancement on MRI. Intraoperative investigation found no evidence of necrotic tumor, and histopathological examination revealed an active tumor in both sections, with no central necrosis. We describe a patient with an intracavernous aneurysm in whom the area of increased aneurysm pulsatility as demonstrated by high-temporal resolution CTA was identical to the area of aneurysm growth on follow-up. We present a rare case of a metastasis to a pituitary adenoma and review 14 cases published in the literature to date. Metastases to pituitary adenomas most commonly present with rapidly progressing chiasm compression syndromes, headaches and oculomotor nerve palsies. The prognosis for this condition is poor. We report our experience in 3 consecutive cases of thalamic abscess, treated by stereotactic puncture as the first step, followed by histological analysis, external drainage and targeted intrathecal and systemic antibiotic therapy. Deep-seated abscesses seem to behave differently as they are associated with an increased risk of intraventricular rupture and antibiotic resistance, a fact which justifies a more aggressive and immediate surgical treatment. We present a preliminary report on the intra-operative use of a head-mounted microscope (“Varioscope” Leica HM500) in spinal neurosurgery. The Varioscope is a dynamic microscope mounted on a head-set. It weights 297 g and measures 73×120×63 mm (length×width×height). We present a case of a subarachnoid haemorrhage due to a ruptured basilar artery aneurysm in a 61-year-old woman. She presented with visual deterioration as the only clinical sign, no previous episodes of headache were reported. Fundoscopy showed a bilateral vitreous haemorrhage. A consecutively performed cranial computer tomography demonstrated subarachnoid bleeding. Further diagnostics revealed a basilar artery aneurysm that was successfully treated with coil embolisation.
