FDA Warns About Using Codeine After Tonsillectomy
Medicines that contain codeine should not be used for pain relief in kids after a tonsillectomy or adenoidectomy (surgery removing the tonsils or adenoids), because there is a chance of serious side effects or possible death, according to the U.S. Food and Drug Administration (FDA). A new boxed warning addressing the risk of codeine for post-surgery pain management in children will be added to medicines that contain codeine. A boxed warning is the FDA's highest warning level... FDA drug safety communication: Update of codeine use in children- new Boxed Warning and Contraindication on use after tonsillectomy and/or adenoidectomy
Source: FDA Area: News The FDA has notified the public about new actions being taken to address a known safety concern with codeine use in certain children after tonsillectomy and/or adenoidectomy   In August, it had issued an alert, following reports of children who developed serious adverse effects or died after taking codeine for pain relief after these procedures for obstructive sleep apnoea syndrome. These children (aged two to five) had evidence of an inherited ability to convert codeine into life-threatening or fatal amounts of morphine in the body. All children had received doses of codeine that were with...

Comparison of two different adenoidectomy techniques with special emphasize on postoperative nasal mucociliary clearance rates: Coblation technique vs. cold curettage
Abstract: Objective: To compare the cold curettage and coblation techniques for pediatric adenoidectomy by means of intraoperative blood loss, operation time, and pre- and postoperative nasal mucociliary clearance rates. Methods: The study included 60 consecutive patients undergoing adenoidectomy operation upon the diagnosis of adenoid hypertrophy ranging in age 4–8 years (mean age: 5.82±1.25 years). Two groups consist of 30 patients. The two different adenoidectomy techniques were compared by means of intraoperative blood loss, operation duration, preoperative and postoperative nasal mucociliary clearance rate (NMCR) va...

The TIMP-1 expression in germinal centers of hypertrophied adenoids in children
Conclusion: It is difficult to interpret our results. Our findings did not demonstrate changes in TIMP-1 expression according to age. This may indicate that the processes of hyperplasia, hypertrophy and atrophy of adenoid are not influenced by age and support our thesis that adenoid involution is rather the effect of changes in the number of lymphoid follicles that changes in them. However there is a need for further observational studies of TIMPs and MMPs in adenoid tissue. (Source: International Journal of Pediatric Otorhinolaryngology)

Hearing 25 years after surgical treatment of otitis media with effusion in early childhood
Conclusions: Hearing 25 years after surgical treatment of chronic OME is not different from age and gender matched normal hearing. In addition, treatment modality (myringotomy or ventilation tube) has no impact on the long-term hearing level. However, atrophy is associated with a hearing loss in tubed ears, whereas myringosclerosis is associated with a hearing loss in myringotomised ears. The hearing loss associated with pathology is of limited magnitude (up to about 5dB PTA). Even though pathology does occur more frequently and more extensively in tubed ears, the effect on the hearing at the group level is too small to ha...

Effects of guidelines on adeno-tonsillar surgery on the clinical behaviour of otorhinolaryngologists in Italy
Conclusions: The recommendations first developed in Italy in a 2003 policy document and then resumed in guidelines in 2008, were not implemented by ENT units involved in the survey. The study highlights the fact that the indications for adeno-tonsillar operations are based on the overall clinical presentation (comorbidity) rather than on a single symptom. Guidelines are necessary to give coherent recommendations based on both the findings obtained through randomized controlled trials and the data collected from observational studies. MHRA Drug Safety Update: European safety review of codeine-containing pain relief in children following post-surgical fatalities in ultra-rapid metabolisers
Source: MHRA Area: News The December issue of the MHRA's Drug Safety Update discusses a European review of the safety of medicines containing codeine licensed for pain relief in children (aged 0-18 years), which was started in October 2012. This review was triggered by recent concerns that there is an increased risk of morphine toxicity when certain susceptible children are given codeine for post-operative pain. These concerns follow the reporting of three fatalities, and one life-threatening case of respiratory depression in children given codeine after tonsillectomy or adenoidectomy in the treatment of obstructive sleep...

Use of purified fibrinogen concentrate for dysfibrinogenemia and importance of laboratory fibrinogen activity measurement
We report a patient with dysfibrinogenemia treated with purified fibrinogen concentrate who had discrepant post‐treatment laboratory values. The patient had mild bleeding symptoms and was diagnosed with dysfibrinogenemia based on fibrinogen activity of 51 mg/dl and antigen of 240 mg/dl. He was treated for an adenoidectomy with purified fibrinogen concentrate (RiaSTAP®) at a dose of 70 mg/kg. A discrepancy in post‐treatment fibrinogen activity was observed between the hospital and reference laboratories. Investigation revealed differences in laboratory assay and calibration methods. Fibrinogen concentrate may be ...

Children with severe or moderate obstructive sleep apnoea syndrome show a high incidence of persistence after adenotonsillectomy.
Conclusion: Persistent obstructive sleep apnoea syndrome (OSAS) occurs in approximately 20% of normal-weight children after adenotonsillectomy (T&A) and, in nearly 70% of them, it is caused by adenoid regrowth. Patients with severe or moderate OSAS showed a high incidence of persistent disease even after T&A. Reactive Tonsillar Enlargement Showing Strong 18F-Fdg Uptake During the Follow-Up of Follicular Lymphoma
We report a case of a 40-year-old man with a previous history of a nodal follicular lymphoma, stage IVA, treated with R-CHOP, which showed strong 18F-FDG uptake in the Waldeyer’s tonsillar ring during his follow-up, being considered highly suspicious of relapsed lymphoma. A surgical removal of the palatine tonsils and adenoids was performed, which showed reactive follicular hyperplasia. Furthermore, bone marrow biopsy revealed absence of neoplasia. Antibiotics for otitis media with effusion in children
Source: Cochrane Area: Evidence > Disease Focused Reviews Background Otitis media with effusion (OME) is characterised by an accumulation of fluid in the middle ear behind an intact tympanic membrane, without the symptoms or signs of acute infection. In approximately one in three children with OME, however, a bacterial pathogen is identified in the middle ear fluid. In most cases, OME causes mild hearing impairment of short duration. When experienced in early life and when episodes of (bilateral) OME persist or recur, the associated hearing loss may be significant and have a negative impact on speech development and b...
