Background: Although the application of damage control surgery for trauma has been widely reported and defined, similar approach in non-trauma patients has not been well detailed. Methods: A retrospective analysis of data from non-trauma patients who underwent emergency laparotomy between May 2006 and December 2008. Demographics, indications for surgery and outcome of patients who had definitive laparotomies (DL) and patients who had abbreviated laparotomies (AL) were compared. Appendectomies were excluded. Results and discussionTwo-hundred ninety-one patients (55% males) were included. Thirty-one (10.7%) underwent AL (58% ...

A protective role for early oral exposures in the etiology of young adult Hodgkin lymphomaemail this articleEmail this article to a colleague. save this article to My ClippingsSave this article to My Clippings. discuss this articleDiscuss or comment on this article.
The pattern of adolescent/young adult Hodgkin lymphoma (YAHL) suggests causation by a relatively late infection with a common childhood virus, but no causal virus has been found. Susceptibility is heritable and linked to lower interleukin 12 (IL12) levels, which can also result from fewer fecal-oral microbial exposures early in life. We studied twin pairs discordant for YAHL to examine exposures capable of altering the IL12 response and T-helper type 1 (Th1)–Th2 balance. One hundred eighty-eight YAHL-discordant twin pairs from the International Twin Study returned questionnaires (70% response). Exposure history of YA...

Transient neurological symptoms (TNS) after spinal anaesthesia have been reported most commonly in association with lidocaine, but have been observed with other local anaesthetics. The aim of this prospective, randomized, double-blind study was to investigate the incidence of TNS after spinal anaesthesia with either levobupivacaine or lidocaine. Patients undergoing inguinal hernia, appendectomy, varicose vein or minor orthopaedic operations were included in the study (60 patients; 47 male, 13 female, overall mean age 30 years). All patients had an American Society of Anesthesiologists score of I or II. The patients were ra...

Conclusions: In addition to clinical criteria, molecular studies for detecting disease-causing mutations are needed to establish the diagnosis of FMF. FMF patients who were homozygous for MEFV gene mutations had a higher symptom severity score and higher incidence of appendectomy. The broad spectrum of mutations may reflect intercultural interactions of ethnic groups in Anatolia. Nation-wide studies may help to determine the relationships among demographic, clinical and genetic features of FMF. (Source: Journal of Paediatrics and Child Health)

Computed tomography (CT) diagnoses appendicitis accurately, but few studies evaluate how often CT changes the management of appendicitis. Consultations for appendicitis were evaluated by surgeons and assigned to groups: high, indeterminate, and low suspicion. After assignment, CT was reviewed if completed or ordered if desired by the surgeon and changes in plans were noted. One hundred patients were evaluated for appendicitis, 70 received appendectomy. Our negative appendectomy rate was 4 of 70 (5.7%). In the high suspicion group, 63 patients had 23 CT scans performed and 2 CT scans were negative, avoiding unnecessary ...

Perioperative radial nerve injury is a rare anesthetic complication, and is rarely seen in association with the use of an automatic blood pressure monitor. As far as we know, only one case has been reported. Here, we report a 26-year-old healthy, lean female who sustained acute radial nerve palsy after appendectomy. A dropped wrist improved 5 days later. The cause of the radial neuropathy is discussed. We recommend that when an automatically cycling blood pressure monitor is used on a lean patient, caution should be taken against such a complication. However, SLCT with mucinous adenocarcinoma as heterologous element has been rarely documented. Acute appendicitis: Is removal of a normal appendix still existing and can we reduce its rate?email this articleEmail this article to a colleague. save this article to My ClippingsSave this article to My Clippings. discuss this articleDiscuss or comment on this article.
Conclusion:</b> In spite of the advances in the diagnostic and imaging techniques, the rates of negative findings on appendicectomy have not decreased much. Clinical judgment is still the most important factor in the management of patients with suspected acute appendicitis. The routine use of CT scan or diagnostic laparoscopy for all patients who are suspected to have appendicitis is neither cost-effective nor safe. (Source: The Saudi Journal of Gastroenterology)

We read with a great deal of interest the article recently published in your journal by Tycast JF et al (J Pediatr Surg 2008;43:2112-14). The authors present a case of infantile Amyand's hernia (AH) treated with a combined approach of laparoscopic appendectomy and open repair of the hernia. (Source: Journal of Pediatric Surgery)

We report a series of four patients with retrocecal appendicitis who presented with acute right upper abdominal pain. The clinical diagnoses at presentation were acute cholecystitis in two patients, pyelonephritis in one, and ureteric colic in one. Ultrasound examination of the abdomen at presentation showed subhepatic collections in two patients and normal findings in the other two. Computed tomography (CT) identified correctly retrocecal appendicitis and inflammation in the retroperitoneum in all cases. In addition, abscesses in the retrocecal space (n = 2) and subhepatic collections (n = 2) were also demonstrated. Emerg...

First single-incision type 7 total laparoscopic hysterectomy, adnexectomy, and appendectomyemail this articleEmail this article to a colleague. save this article to My ClippingsSave this article to My Clippings. discuss this articleDiscuss or comment on this article.
This study is a retrospective chart review and discussion of two patients who underwent a total laparoscopic hysterectomy, bilateral salpingo-oophorectomy, and appendectomy (Canadian Task Force Level III). Both patients underwent a type 7 total laparoscopic hysterectomy for benign indications in July 2007 and sustained no complications. The evidence from these two cases suggests that advanced laparoscopic procedures are possible using a single skin incision for multiple ports. Technological advances, including those in port structure, are needed to enable surgeons to employ strategies that effectively enhance instrume...

In this study, we report a retrospective analysis of 30 patients aged between 19 days and 8 years with an Amyand’s hernia operated in our institution from 1998 to 2009, and we reviewed the literature on the topic. Hernia repair without an appendectomy was performed in patients with normal appendix. Emergency appendectomy through herniotomy was performed in cases of inflamed and perforated appendices. Conclusion: Late presentation and perforated appendicitis were associated with high complication rates in emergency laparoscopic appendectomy. Parents of the patients should be informed about this before operation. The possibility of using alternative management in these situations can be considered. (Source: Surgical Practice)

Conclusions  The number of ARPs being performed in 36 free-standing children's hospitals is decreasing each year relative to several operations commonly performed at these institutions. Despite this overall annual decrease, there is tremendous variation between individual hospitals in how frequently ARPs are being performed relative to these procedures.
