Advice on dosage adjustment in renal impairment in Appendix 3 of the latest BNF (58) will now be expressed in terms of eGFR rather than creatinine clearance in the majority of cases. This is further qualified with information on the limitations of eGFR, such as:
. eGFR should not be used to adjust dosages of nephrotoxic drugs or drugs with a narrow therapeutic index; in such cases, use plasma-drug levels to adjust dose or creatinine clearance calculated using the Cockcroft and Gault formula.

This paper is concerned with early development of transformed epithelial cells (TECs) in the presence of fibroblasts in the tumor micro-environment. These two types of cells interact by means of cytokines such as transforming growth factor (TGF-beta) and epidermal growth factor (EGF) secreted, respectively, by the TECs and the fibroblasts. As this interaction proceeds, TGF-beta induces fibroblasts to differentiate into myofibroblasts which secrete EGF at a larger rate than fibroblasts. Coral throughout the world are under threat. To save coral via cryopreservation methods, the Symbiodium algae that live within many coral cells must also be considered. This study examined the heritability of acute appendicitis.The study included twin pairs with known zygosity born between 1959 and 1985. Individuals with acute appendicitis were found by record linkage with the Swedish Inpatient Register. Comparing monodizygotic and dizygotic twins, the similarity and relative proportions of phenotypic variance resulting from genetic and environmental factors were analysed. In an effort to explore the role of family of origin (or equivalent) issues as they affect a couple’s relationship, the authors introduce the Birth Order Research-Based Questionnaire (BORQ). Couples will also be asked to participate in other interview questions to assess each individual’s birth order effects and family of origin. A brief review of the birth order literature is included as well as an appendix documenting the birth order traits for oldest, only, middle, and youngest.
