Mastering The Head-To-Toe Assessmentemail this articleEmail this article to a colleague. save this article to My ClippingsSave this article to My Clippings. discuss this articleDiscuss or comment on this article.
You probably practiced your head-to-toe assessment a bunch in your EMT class. Maybe more than any other skill in the EMT curriculum. If your class was or is anything like mine (as a student or a teacher) you performed the head-to-toe assessment again and again. As much as we practice this skill in EMT class, I often wonder why so many EMT’s have such bad head-to-toe skills out on the street. It seems that, once we get out on the street, the systematic, thorough head-to-toe assessment falls out of favor and quickly gets replaced with the faster, more direct focused assessment. That works just fine most of the time. ...

Ask people why the enter the lottery and they will usually tell you that “you’ve got to be in it to win it”. As far as it goes that’s true, but it still doesn’t get around the odds of you picking the right numbers being vanishingly (although not quite homeopathically) small at 14 million to 1 against for 6 numbers from a 1-49 selection. Compare their feelings about their chances of winning the lottery to succumbing to the toxic effects of their favourite tipple or a disease triggered by dietary whim and they may well respond, that such problems are more likely to happen to “other people&...

One of my favorite bloggers, Seth Godin, recently introduced me to the work of Scott McCloud, an author who’s written several classic books on understanding comic books. Scott dissects the comic medium and explains the ongoing allure of the comic book to the uninitiated. One of Scott’s observations is that comic books require imagination on the part of the reader because, in all great comic books, the action occurs between the frames. The artist only shows you snapshots of action and dialogue. Most of the story takes place in our heads. The real story is the stuff that we invent that happened in-between th...

CONCLUSIONS: This systematic analysis of a small sample of alcohol dependent individuals gives insight into their experiences during alcohol dependency and the journey to recovery. The findings suggest that denial of the problem to the outside world occurs simultaneously with individuals being aware of their problem. Participants felt the illness carries a stigma and their negative experiences of health professionals other than GP’s suggests that nurses and other health workers need to revise their understanding of alcohol dependence and their approach to it. AA was a significant factor in recovery for these partic...

Detoxification from alcohol abuse. The alcohol withdrawal syndrome (AWS) is a common management problem in hospital practice for neurologists, psychiatrists and general physicians alike. Although some patients have mild symptoms and may even be managed in the outpatient setting, others have more severe symptoms or a history of adverse outcomes that requires close inpatient supervision and benzodiazepine therapy. Many patients with AWS have multiple management issues; withdrawal symptoms, delirium tremens (DT’s), the Wernicke–Korsakoff syndrome, seizures, depression, polysubstance abuse, electrolyte dist...

This study asked 244 patients seen in eight GP practices in the UK to complete the two questionnaires, plus a number of other questionnaires – unfortunately these are not listed in this paper, although they are included in a previous paper (Hill, Dunn, Lewis et al, 2008). Correlations and discriminant analyses were carried out, and the results detailed in this paper. Only 53% of the patients actually completed the questionnaires. In this study, typically developing students were tested for their IQ and working memory at 5 years old and again when they were 11 years old. Findings and Educational Implications The findings revealed that a child’s success in all aspects of learning is down to how good their working memory is regardless of IQ score. Critically, working memory at the start of formal education is a more powerful predictor of subsequent academic success than IQ in the early years. This unique finding is important as it addresses concerns that general inte...

Postural sway among abstinent alcoholics can be improved up to a point Excessive sway during quiet standing is a common and significant consequence of chronic alcoholism, even after prolonged sobriety, and can lead to fall-related injury and even death. A new study of residual postural instability in alcohol-abstinent men and women shows that alcoholics improve with prolonged sobriety, but the improvement may not fully erase the problem of instability. In New Zealand, the national accident insurer, Accident Compensation Corporation, has been reviewing its pain management service contracts. The latest message from both the Government and ACC is the need to reduce costs (not that I’ve ever heard anyone say ‘let’s go for broke, let’s spend all we can!’) and one way to do this is to look at how to reduce the risk of long-term disability associated with chronic pain. So, in their pursuit of ways to do this, ACC has consulted with providers and reviewed opinions about services provided to claimants under the various pain services contracts and com...

During residency new doctors are faced with several assessments. The 360-degree feedback is just one of them. It involves the evaluation of performance on various tasks by, firstly, peers with knowledge of a similar scope of practice, secondly, co-workers from allied health professions and, thirdly, patients. Or: ‘letting future behaviour be guided by a systematic and critical analysis of past actions and their consequences’. How the 360-degree feedback is put into practice differs with were they work. Research Shows $100 Billion Ed. Stimulus Likely Hurting Economyemail this articleEmail this article to a colleague. save this article to My ClippingsSave this article to My Clippings. discuss this articleDiscuss or comment on this article.
Tomorrow morning, the president’s Council of Economic Advisers will release a report assessing the short and long-term effects of the stimulus bill on the U.S. economy. As with previous iterations, this report will attempt to forecast overall effects of the stimulus across its many different components and the different economic sectors it targets. In doing so, it ignores the clearest research findings available pertaining to a key portion of the stimulus: k-12 education. The president has committed $100 billion in new money to the nation’s public school systems, and required that states accepting the funds pro...

Physical restraint techniques and procedures are a subject of debate and controversy in EMS. Few agencies have taken the time and energy to research and develop a comprehensive restraint guideline for field providers to follow. When violent or aggressive patients show up (and they always do) EMT’s are left to fend for themselves. In these situations we take on a great deal of risk, both personal and legal, to bring the patient safely to the hospital. I’ve had my share of both good and bad take-downs. When things go well the call transitions smoothly from the street to the hospital. The patient stays protected,...

Safety behaviours – do they maintain kinesiophobia?email this articleEmail this article to a colleague. save this article to My ClippingsSave this article to My Clippings. discuss this articleDiscuss or comment on this article.
Let me start by saying this post is conjecture, but based both on observing patients, and after reading an interesting paper on ’subtle avoidance and safety behaviours relevant to social anxiety’. First some definitions: I hope you’re all familiar with the term ‘kinesiophobia’, or ‘fear of movement’ – it’s the fear and avoidance of movements that an individual believes will hurt or harm them. Safety behaviours: are strategies that may be used to reduce the anxiety of carrying out a behaviour – and are usually ‘logically’ linked to the underlying belief...
