The lower esophageal sphincter (LES) and the upper esophageal sphincter (UES) play a central role in preventing regurgitation and aspiration. The aim of the present study was to evaluate the UES, LES and barrier pressures (BP) in obese patients before and during anesthesia in different body positions. Using high-resolution solid-state manometry, we studied 17 patients (27[ndash]63 years) with a BMI[ge]35 kg/m2 who were undergoing a laparoscopic bariatric surgery before and after anesthesia induction. Before anesthesia, the subjects were placed in the supine position, in the reverse Trendelenburg position (+20°) and in the...

Abstract Clinical trials evaluating low molecular weight heparin (LMWH) for the prevention and treatment of venous thromboembolism
and acute coronary syndromes have led to their regulatory approval for these indications in the general population. However,
certain patient populations have been excluded from these landmark clinical trials, including patients with renal insufficiency,

on pharmacokinetic studies often performed in healthy subjects, or small cohort studies which are generall...

Conclusions Brain available dopamine D2 binding appears to increase following GBP. This preliminary finding needs to be replicated in
a larger population but suggests that diminished D2 binding in the obese may be due to D2 receptor downregulation. Changes
in available dopamine receptor binding may play an important role in centrally mediated appetite suppression and resultant
weight loss after LGBP.

EndoscopyDOI: 10.1055/s-0029-1215269AbstractThe worldwide epidemic of obesity is forecast to worsen with a concomitant increase in the burden of co-morbid conditions. Bariatric surgery has some disadvantages, and intragastric balloons (IGBs) represent a generally safe, reversible and less invasive approach to weight reduction, based on occupying gastric space to enhance the sensation of satiety. A literature review found that the majority of the published data on IGBs relates to the Bioenterics intragastric balloon (BIB). The evidence for the efficacy of the BIB as a primary means of weight loss is still unconvincing, but ...

Recent estimates by the World Health Organization suggest that if current trends continue, obesity will affect an estimated 1.5 billion people by 2015. Indeed, the close association between obesity and the increased incidence and prevalence of obesity-related comorbidities, mortality, decreased quality of life, and increased health care expenditure has made obesity a worldwide public health priority. Behavioral and pharmacological treatment efforts have had done little to affect these trends, particularly in the setting of severe obesity (body mass index [BMI] ≥35 kg/m2) and super obesity (BMI ≥50 kg/m2). Conclusions In obese omental fat, saturable adipocyte LCFA uptake was greater than in controls, and expression of key genes involved in

Abstract: Background: Bariatric surgery leads to profound weight loss, but postoperative complications and psychosocial issues may impact long-term quality of life. The primary aim of this project was to examine whether such patients have better quality of life and self-reported functional status compared with obese adults who do not have bariatric surgery. Methods: This population-based study of patients evaluated for Roux-en-Y gastric bypass surgery involved a survey consisting of baseline and follow-up single-item overall quality-of-life items (Linear Analogue Self-Assessment Questionnaire; LASA), follow-up quality of li...

CONCLUSIONS: Intraoperative alveolar recruitment with a VCM followed by PEEP 10 cm H(2)O is effective at preventing lung atelectasis and is associated with better oxygenation, shorter PACU stay, and fewer pulmonary complications in the postoperative period in obese patients undergoing laparoscopic bariatric surgery.

Abstract Bariatric surgery dramatically alters the normal stomach anatomy resulting in a significant incidence of hiatal hernia and
gastroesophageal reflux disease. Although the majority of patients remain asymptomatic, many complain of severe heartburn
refractory to medical management and additional highly atypical symptoms. Here, we describe the diagnosis and treatment regarding
four cases of symptomatic hiatal hernia following bariatric surgery presenting with atypical symptoms in the University Hospital,
USA. Four patients presented following laparoscopic Roux-en-Y gastric bypass or duodenal switch/pancr...

Roux-en-Y gastric bypass (RYGB) has become the gold-standard bariatric procedure, partly because of the rapid resolution of accompanying diabetes. There is increasing evidence this is mediated by duodenal exclusion. We hypothesize that duodenal exclusion suppresses intestinal Na+/glucose cotransporter SGLT1-mediated glucose transport, improving glucose handling, and aimed to test this in a rodent RYGB model. Sprague-Dawley rats underwent sham procedure or duodenal exclusion by RYGB (10 cm Roux, 16 cm biliopancreatic limbs). Animals were maintained for 3 wk on a Western diet, before harvest at 10 AM, 4 PM, and 10 PM. Sectio...

We report a case of a 6-year-old girl suffering from morbid obesity, Blount`s disease, and significant social and functional
impairment who underwent a laparoscopic sleeve gastrectomy. One year later, she has shown remarkable improvement in all aspects
of her health emphasizing the success of the procedure. A follow-up laparoscopic Roux-en-Y gastric bypass or biliopancreatic
diversion (BPD) are options if she regains weight as she gets older. This case is noteworthy for several reasons. The age
of the patient is younger than any currently on record who has had this treatment. Additionally, the utilization of a sleeve
