CONCLUSIONS: During advancement of the tracheal tube, alterations of the shape of the visualized breath sounds using the Visual Stethoscope appeared before the changes of the breath sounds were detected by auscultation. Bilateral breath sounds disappeared when the tip of the tracheal tube was advanced beyond the carina in both groups.

We present a case of TBM with refractory lower respiratory infection and haemoptysis.The patient was a 53-year-old man with recurrent haemoptysis and without fever, wheezes, chest pain, weight loss or any respiratory disease. Chest helical computed tomography (CT) scan showed tracheomegaly with transversal diameters of the trachea of 50mm. A bronchoscopy was performed and revealed markedly enlarged airways and traces of bleeding arising from the right airways. The haemoptysis was stopped only after bronchial artery embolization. Conclusion: Flexible bronchoscopy provided rapid and definitive diagnosis for our patients with persistent wheezing without any major complications. This study is one of the largest studies concerning persistent wheezing. Early bronchoscopic evaluation can reduce cost by providing rapid and accurate diagnosis and preventing unnecessary investigations and drug administration. Conclusions: For children with FBA who experience severe and life-threatening complications from FBA, our recommendation is that emergency measures should be available during FBs removal. CONCLUSIONS: Semi-quantitative cultures of endotracheal aspirate are poorly concordant with quantitative cultures obtained via non-bronchoscopic BAL. Although the performance of endotracheal aspirate improves when antibiotic treatment is considered, guiding therapy on the basis of semi-quantitative cultures may still result in failure to identify potentially multiple-drug-resistant pathogens, and would also tend to promote excessive antibiotic usage. Our data support the use of quantitative cultures in diagnosing VAP.

We describe three cases of bronchopleural fistula that developed after surgery for lung cancer in 1 patient and for bronchiectasis in 2 patients. All were successfully treated endoscopically by direct application of albumin-glutaraldehyde tissue adhesive (BioGlue; Cryolife Inc, Kennesaw, GA) through a rigid bronchoscope. Complete resolution was obtained in each patient within 24 hours.

Abstract After the recovery of pneumocystis pneumonia, amebiasis, and cytomegalovirus duodenal ulcer in a patient with AIDS, antiretroviral
therapy was initiated. Fever was first noted on the 11th day of administration of the highly active antiretroviral therapy
(HAART), and chest radiography showed infiltration into the right lower lobe of the lung on the 19th day. Bronchoscopy was
performed, with a tentative diagnosis of pulmonary tuberculosis, but Mycobacterium kansasii was eventually identified. M. kansasii may also be a pathogenic organism causing immune reconstitution inflammatory syndrome.

Microarray technology provides a powerful tool for defining gene expression profiles of airway epithelium that lend insight into the pathogenesis of human airway disorders. Successful Endobronchial Seal of Surgical Bronchopleural Fistulas Using BioGlue [CASE REPORTS]

We describe three cases of bronchopleural fistula that developed after surgery for lung cancer in 1 patient and for bronchiectasis in 2 patients. All were successfully treated endoscopically by direct application of albumin-glutaraldehyde tissue adhesive (BioGlue; Cryolife Inc, Kennesaw, GA) through a rigid bronchoscope. Wegener’s granulomatosis (WG) is a multisystem autoimmune vasculitis of small to medium sized blood vessels. Etiology is unknown. Nectrotising granulomatous vasculitis with destruction of the walls and narrowing of the vessel lumen leads to disturbanses of appropriate organs function. Clasical WG includes symptoms of upper and lower respiratory tract and kidneys, but WG can affect any organ. We report the case of a patient who received a heart transplant (HT) because of cardiac amyloidosis and who developed pseudomembranous AT. Possible risk factors concurrent in this case were splenectomy, lymphocytopenia, and previous cytomegalovirus infection. Chest computed tomography scan showed thickening of the left bronchi and a 'tree-in-bud' pattern with multiple small nodules. Increased cardiac index due to terbutaline treatment aggravates capillary-alveolar macromolecular leakage in oleic acid lung injury in dogs

Conclusions:
As cardiac index increased with terbutaline infusion, we speculate that terbutaline recruits leaky capillaries and increases FITC-D70 leakage after OA injury. These findings suggest that therapies inducing an increase in cardiac output and a decrease in pulmonary vascular resistances have the potential to heighten the early increase in protein transport from plasma to alveoli within the acutely injured lung. Conclusion Aspiration and ingestion of dental foreign objects are infrequent, but they can occur at large multidisciplinary dental procedures.
These episodes have the potential to result in acute medical and life-threatening emergencies since the beginning of the event
or at a late stage in proceeding in the underdiagnosed patient. Prevention of such incidents is, therefore, the best approach
via the mandatory use of precautions during all dental procedures, and in case of suspicion with no retrievable material,
patient must always be submitted to a radiographic study.

CONCLUSIONS: A diagnosis of lipoid pneumonia should be considered in patients with chronic refractory pneumonia or TB, especially if there is a history of mineral oil ingestion. Bronchoscopy with multiple bronchoalveolar lavages was an efficient treatment for the clearance of mineral oil from the lung parenchyma and the prevention of fibrosis. The objective of this study was to describe the use of VB in two patients with suspicion of FBA, as well as to review the literature regarding this topic. The two patients presented with sudden onset of respiratory symptoms and history of cough or choking with foods before these symptoms. Both patients were submitted to VB.
