Conclusions. These are the first prospective data obtained from homosexual men to assess circumcision status as a risk factor for STIs. Circumcised men were at reduced risk of incident syphilis but no other prevalent or incident STIs. Circumcision is unlikely to have a substantial public health impact in reducing acquisition of most STIs in homosexual men.

Current General Medical Council (GMC) guidelines state that any doctor who does not wish to carry out a non-therapeutic circumcision (NTC) on a boy must invoke conscientious objection. This paper argues that this is illogical, as it is clear that an ethical doctor will object to conducting a clinically unnecessary operation on a child who cannot consent simply because of the parents’ religious beliefs. In their Comment on male circumcision and HIV risks and benefits for women partners (July 18, p 182), Jared Baeten and colleagues admit, albeit reluctantly, that circumcision of HIV-positive men does not reduce HIV risk for their partners. CONCLUSIONS PCT funding withdrawal for ritual circumcision had an impact on our unit's procedural case volume. This representeda cost saving to the trust, despite a higher rate of admissions for postoperative complications. There is an inequality inhealthcare provision throughout the British Isles for ritual circumcision, and we feel it is vital to offer support and training tomedical and non-medical practitioners who are being asked to perform a greater number of circumcisions in the community.

Purpose: We investigated a reconstructive method with better sensory and erectile function for partial penile defects and report our long-term results of surgical correction using scrotal skin flaps.Materials and Methods: We retrospectively analyzed the records of 18 patients with penile defects referred to us between 1992 and 2007. This article presents research findings based on the meaning of indigenous circumcision to Xhosa men in South Africa. In South Africa, male circumcision is a rite of passage from adolescence to adulthood. The country has experienced serious problems associated with the practice of this rite ranging from dehydration to death in the traditional "bush" circumcision schools. A qualitative, endogenous research design that is explorative, descriptive, and contextual was undertaken using individual interviews, focus group interviews, and community meetings discussing this ritual. Conclusion: There is a wide spectrum of lower genital lesion among girls in our setting. Treatment of these lesions may be challenging, but the outcome in most cases is good. Conclusion: Foreskin inflammation is increased with HIV and HSV-2 infections, higher HIV viral load and presence of smegma. Foreskin inflammation may have implications for HIV transmission and acquisition in uncircumcised men.
