Little is known about the role of skin color in the forensic sexual assault examination. The purpose of this study was to determine whether anogenital injury prevalence and frequency vary by skin color in women after consensual sexual intercourse. The sample consisted of 120 healthy (63 Black, 57 White) women who underwent a forensic sexual assault examination following consensual sexual intercourse. Experienced sexual assault forensic examiners using visual inspection, colposcopy technique with digital imaging, and toluidine blue application documented the number, type, and location of anogenital injuries. Although 55% of...

This study examines the validity and reliability of the British Society for Colposcopy and Cytopathology (BSCCP) objective structured clinical examination (OSCE). The BSCCP OSCE results obtained over eight OSCE circuits were analysed using SPSS 15. Face validity and content validity were established from expert opinion and blueprinting. Statistically significant difference was not shown in construct validity through level of experience (P=0.867, P=0.822, P=0.59, P=0.74, P=0.12, P=0.01; these are the P values for each of the patient interaction stations) however, concurrent validity was established against the gynaecology m...

Abstract The aim of this paper was to investigate the prevalence of human papilloma virus (HPV) infection in women under 30 years old
with cervical intraepithelial neoplasia (CIN) or cervical cancer, in order to provide a basis for cervical cancer prevention
and treatment. We recruited 2052 female cases from the education system in Futian District, Shenzhen city, from April 2006
to April 2008, with age ranging from 22 to 60 years old. Secondgeneration hybrid capture test was done for detection of female
genital tract HPV and cervical colposcopy for screening CIN or cervical cancer. The prevalence of HPV and ...

High-risk human papillomavirus (HR-HPV) testing for colposcopy triage of low grade squamous intraepithelial lesion (LSIL) is not recommended because of high positive rates in young women. It remains unclear whether HR-HPV testing may be useful for triage of older women. We compiled HR-HPV data for women aged [ge]30 years with LSIL for the period March 1, 2006 to February 28, 2008. Follow-up cervical biopsy information was collected for the period March 1, 2006 to August 15, 2008. We used the Hybrid Capture II test performed on residual material from liquid-based Pap tests. Of 735 women, 254 had HR-HPV testing, and of these...

Currently, screening programmes for cervical cancer usually rely on cytology as the first-line screen. Women who are found to be cytologically abnormal are triaged by carcinogenic human papillomavirus (HPV) DNA testing or by repeat cytology, or are referred directly for a colposcopic examination. At colposcopy, biopsies are taken from any apparent lesions. Treatment is decided on the basis of the combined cytological, colposcopic, and histological diagnoses during the patient's history. These decision processes are formalised in complex consensus management algorithms that narrow down the clinical management options to a s...

The family of human papilloma viruses (HPV) comprises more than 100 genotypes. Approximately 30 of the genotypes are responsible for infections in the human anogenital area. A precisely defined algorithm with the goal of diagnosing and removing HPV for a prolonged or indefinite length of time, as well as to protect the patient from any malignant ransformation, does not currently exist. The identification of HPV strains by PCR or DNA hybridization in lesional tissue provides higher security for the patients. In high-risk patients additional colposcopy, rectoscopy, and rethroscopy/cystoscopy increase the probability of prope...

Conclusions: Women experienced long-lasting negative effects on their mental health, but not on their physical health after referral for colposcopy. LLETZ had no measurable influence on QoL at any of the visits. This indicates that it is not the severity of the lesions or the treatment itself that affects women but the fact of having had an abnormal cervical smear identified. Depressive mood may be a major variable for the perception of low QoL. (Source: European Journal of Obstetrics, Gynecology, and Reproductive Biology)

Collaborative analyses of relevant trials are needed to explain the much smaller number of additional cases of cervical intraepithelial neoplasia grade 3 and above (CIN3+) detected by HPV testing in the ARTISTIC trial than in other apparently similar studies. One important factor mentioned by Sasieni and colleagues was that many cytologically normal women with persistent HPV were not referred to colposcopy before round 2 (the first cytology 26–54 months after entry), mainly because they did not attend for retesting at 12 months. However, the effect is reduced in the combined round 1 and round 2 results, as CIN3+ in late ...

Abstract: After Pap smear test, colposcopy is the most used technique to diagnose cervical cancer due to its higher sensitivity and specificity. One of the most promising approaches to improve the colposcopic test is the use of the aceto-white temporal patterns intrinsic to the color changes in digital images. However, there is not a complete understanding of how to use them to segment colposcopic images. In this work, we used the classification algorithm k-NN over the entire length of the aceto-white temporal pattern to automatically discriminate between normal and abnormal cervical tissue, reaching a sensitivity of 71% a...

This study investigates the clinical course of low grade squamous intraepithelial lesions (LSIL), HPV status and HPV16-specific immune response in a large prospective study of 125 women with LSIL followed cytologically, virologically and histologically. Women with low-grade abnormal smears were recruited and followed-up for one year. Colposcopy, cervical biopsy for histology and brushings for HPV typing was performed at recruitment, 6 months (no biopsy) and upon completion of the study at one year. HPV16-specific T-cell responses were analysed by interferon-[gamma] ELISPOT at entry, 6 and 12 months. Infection with multiple...

The benefits of referring women for immediate colposcopy or aggressive treatment instead of
cytological surveillance following detection of low-grade cervical abnormalities may not outweigh the risks of
overtreatment, according to three related studies from the Trial Of Management of Borderline and Other Low-grade
Abnormal smears (TOMBOLA) published online on July 28 in BMJ. (Source: Modern Medicine)

HPV infection is the most commonly diagnosed sexually transmitted viral infection . The cervical lesion is typically seen at colposcopy and is known as flat condyloma. Different treatment modalities for flat condyloma range from follow-up to local excision or destruction using the large loop electrosurgical excision procedure (LEEP), laser vaporization, or cryosurgery. Half of all patients treated with LEEP return to the clinic at some point for either recurrence or appearance of a new lesion . In the absence of specific antiviral treatment, the ability to enhance local immunity to overcome the viral infection is worth inv...

New data have emerged since publication of the American Society for Colposcopy and Cervical Pathology's 2001 consensus guidelines for management of abnormal cervical cytology and histology. The 2006 guidelines include recommendations for special populations (i.e., adolescents and pregnant women). Human papillomavirus testing is now included for management of atypical glandular cytology, for follow-up after treatment for cervical intraepithelial neoplasia, and in combination with cytologic screening in women 30 years and older. The preferred management of atypical squamous cells of undetermined significance in adult wom...

Conclusions Cervical punch biopsies and, especially, LLETZ carry a substantial risk of after-effects. After-effects are also reported by women managed solely by colposcopic examination. Ensuring that women are fully informed about after-effects may help to alleviate anxiety and provide reassurance, thereby minimising the harms of screening. Little is known about acquisition of human papillomavirus (HPV) and its outcome among older women with negative HPV testing and normal cytology. A longitudinal 3-yr follow-up of nested-cohort subjects (n = 8825) from a population-based cervical cancer screening study whose Pap and HPV tests were negative at baseline were conducted. Every active HPV-negative (n = 413) participant had 12-mo follow-ups of Pap smear and HPV testing. Colposcopy was performed if either HPV-positive or cytology was abnormal. The cytology and histology information of the remaining subjects (passive HPV-negative, n = 8412) was obtained from nationa...

We estimate the accuracy of colposcopy and visual inspection with acetic acid (VIA) while minimizing the effects of misclassification bias, and maximizing ascertainment of disease. VIA was performed by experienced physicians on a population-based sample of women aged 30 to 49 years in rural Shanxi province, China. Each woman received VIA, liquid-based cytology (LBC) and hybrid capture 2 (hc2, QIAGEN, Gaithersburg, MD; formerly Digene Corporation). Any woman who tested positive on any test had colposcopy, endocervical curettage (ECC) with directed biopsies as necessary and 4-quadrant random biopsies from normal-appearing ar...

(NaturalNews) Anne is a good patient. She sees her doctor for regular checkups, has yearly mammograms, Pap tests, and colon cancer screenings, and she even paid for a full-body CT scan out of her own pocket. She figures she's doing everything she can to make sure she doesn't get cancer.Truth is, Anne is doing nothing to prevent cancer. Although cancer screening is billed as a preventive service that saves lives, the best it can do is detect disease in its early stages, when it is supposedly easier to treat. Nevertheless, every year millions of Americans dutifully line up for their screenings, completely unaware that they m...

Understanding differences in genital injuries after nonconsensual and consensual intercourse is an important element of prosecuting sexual assault cases. In order to determine if the injury patterns and total surface area of genital injuries can differentiate between the types of intercourse (consensual or non-consensual), eighty women were examined after non-consensual (retrospective chart review, n = 40) and consensual (recruited, n = 40) intercourse within 48 hours using colposcopy, toluidine blue dye, and digital photography to document genital injuries. Differences between types of injuries found in the nonconsensual ...
