Central venous catheter fracture presenting as pain on flushingemail this articleEmail this article to a colleague. save this article to My ClippingsSave this article to My Clippings. discuss this articleDiscuss or comment on this article.
We report two patients who developed localized pain on flushing the catheter. Radiological investigation revealed fluid leakage from the CVC, from a fracture within the skin tunnel in one case, and from the intravascular segment, with venous backflow probably related to a sleeve thrombus, in the other. Summary: Background & aim: Growth hormone (GH) is an important regulator of growth and body composition. It has been shown that GH release can be promoted by administration of various amino acids, such as arginine and lysine, that are present in soyprotein. Soyprotein ingestion increases GH secretion more than the ingestion of a combination of arginine and lysine. The GH promoting effect of soyprotein however is not present when soyprotein is ingested combined with carbohydrate and fat. The aim of this study is to investigate whether carbohydrate or fat eliminates the GH promoting effect of soyprotein. Methods: We compared ...

Oral feeding, by stimulating salivary secretion with its formidable antibacterial properties, may have advantages over enteral tube feeding which bypasses this process. Oral proprietary feeds may be used to provide the patient's entire nutritional requirements or, more commonly, to supplement the diet when the patient is unwilling or unable to take enough normal food. This is particularly common among the elderly. The use of sip feeds, of course depends upon the ability to swallow and the absence of esophageal or gastric obstructions. Once the route of feeding has been chosen and the solution has been identified, it is necessary to decide how it can best be given to the patient. A multi-professional approach is essential to ensure that all clinical constraints e.g. treatments, nursing schedules are considered. It is also important to involve the patient/carers in this decision particularly if long-term feeding is likely. (Source: e-SPEN, The European e-Journal of Clinical Nutrition and Metabolism)
