Effect of Statin Use on Acute Kidney Injury Risk Following Coronary Artery Bypass Grafting
Acute kidney injury (AKI) is a serious complication of cardiovascular surgery. Although some nonexperimental studies suggest that statin use may reduce postsurgical AKI, methodologic differences in study designs leave uncertainty regarding the reality or magnitude of the effect. The aim of this study was to estimate the effect of preoperative statin initiation on AKI after coronary artery bypass grafting (CABG) using an epidemiologic approach more closely simulating a randomized controlled trial in a large CABG patient population. Health care claims from large, employer-based and Medicare insurance databases for 2000 to 20...

Population‐based trends in urinary diversion among patients undergoing radical cystectomy for bladder cancer
Conclusions
In this nationally representative sample of hospitals from 2001 to 2008, the use of continent diversion in RC gradually increased.
Although variations in urinary diversion exist by hospital teaching status, case volume, patient gender and primary health insurance, increased attention in expanding the use of continent diversions may help reduce these disparities for patients undergoing RC for bladder cancer. Provider payment in community-based health insurance schemes in developing countries: a systematic review
Conclusion CBI schemes in developing countries have used a wide range of provider payment methods. The existing evidence suggests that payment methods are a key determinant of CBI performance and sustainability, but the strength of this evidence is limited since it is largely based on observational studies rather than on trials or on quasi-experimental research. According to the evidence, provider payment can affect provider participation, satisfaction and retention in CBI; the quantity and quality of services provided to CBI patients; patient demand of CBI services; and population enrollment, risk pooling and financial su...

Prevalence and socioeconomic correlates of chronic morbidity among elderly people in Kosovo: a population-based survey
Conclusion:
This study provides important evidence on the magnitude and distribution of chronic conditions among the elderly population of Kosovo. Our findings suggest that, in this sample of elderly people from Kosovo, the oldest-old (especially women) and the poor endure the vast majority of chronic conditions. These findings point to the urgent need to establish a social health insurance scheme including the marginalized segments of elderly people in this transitional country. (Source: BMC Geriatrics)

Disparities in Transition Planning for Youth With Autism Spectrum Disorder
Health care transition services assist youth with special health care needs (YSHCN) in transitioning to adult care without gaps in services or health insurance coverage. Less than half of YSHCN receive anticipatory assistance in this transition; receipt of these services for youth with autism spectrum disorder is unknown.
Youth with autism spectrum disorder receive transition services half as often as youth with special health care needs. Quality of health care is associated with increased receipt of health care transition services. Presence of comorbid conditions decreased receipt of transition services. (Read the full ar...

Lost in translation: HMO enrollees in poor health have hardest time communicating with doctors
In the nation's most diverse state, some of the sickest Californians often have the hardest time communicating with their doctors. So say the authors of a new study from the UCLA Center for Health Policy Research that found that residents with limited English skills who reported the poorest health and were enrolled in commercial HMO plans were more likely to have difficulty understanding their doctors, placing this already vulnerable population at even greater risk.
The findings are significant given that, in 2009, nearly one in eight HMO enrollees in California was considered "limited English proficient" (LEP) and ...

Prescription of antiepileptics and the risk of road traffic crash.
Abstract
Studies assessing the impact of epilepsy and its medication on the risk of road traffic crashes have shown inconsistent results. The aim in this study was to assess this risk using French databases. Data from three French national databases were extracted and matched: the national health care insurance database, police reports, and the national police database of injurious crashes. Only antiepileptics prescribed predominantly in epilepsy were studied (phenobarbital, phenytoin, ethosuximide, valproic acid, vigabatrin, tiagabin, levitiracetam, zonisamide, and lacosamide). Arkansas and HHS Agree to Expand Coverage with Exchanges, Not Medicaid; 'A Whole New Ballgame'
If reports coming out of Arkansas are true, Gov. Mike Beebe (D.) and the U.S. Department of Health and Human Services have agreed to implement Obamacare’s coverage expansion in the state using the law’s subsidized private insurance exchanges, instead of with the Medicaid program. The devil is in the details, but if this deal holds, it could dramatically change the course of Obamacare in the states, in a way that will offer lower-income Americans far better—but costlier—coverage than they would have received from the health law otherwise. An exploratory study of the policy process and early implementation of the free NHIS coverage for pregnant women in Ghana
Conclusions:
Ghana has to be congratulated for its persistence in trying to address financial barriers. However, many themes from previous evaluations of exemptions policies in Ghana have recurred in this study -- particularly, the difficulties of getting timely reimbursement to facilities, of controlling charging of patients, and of reaching the poorest. This suggests that providing free care through a national health insurance system has not solved systemic weaknesses. The wider concerns about raising the quality of care, and ensuring that all supply-side and demand-side elements are in place to make the policy effective...
